And there is a specific biological explanation for all of it that most people in recovery are never given.The main culprit is called neuroinflammation. When you use alcohol or drugs heavily over a long period of time, the immune cells in your brain — called microglia — go into a prolonged state of activation. They are essentially treating your brain like it is under attack. This produces inflammation inside your skull, and that inflammation causes the exact symptoms you are experiencing: fatigue, difficulty concentrating, emotional instability, and a general feeling that something is wrong even when you cannot name what it is.The second piece is your stress hormone system. Alcohol, opioids, and stimulants each disrupt the HPA axis — the system that controls your cortisol levels and your body's response to stress. After long-term substance use, this system gets stuck in a high-alert state. Your cortisol stays elevated even when there is nothing to be stressed about. This is why people in early recovery often feel chronically on edge, unable to relax, easily overwhelmed by things that would not have bothered them before.The important thing to understand is that both of these — the inflammation and the stress hormone disruption — are temporary. They have a documented recovery timeline. The body is not permanently broken.
