    Is a prior concussion associated with an increased risk of long-term disability following a motor vehicle crash?     Brain Injury Insider Episode 147: Toddler Concussion Symptoms: What Parents Should Know After a Fall

    When people hear “concussion,” they often think of teen athletes. But concussions are also common in toddlers and preschoolers — and in young children, they most often happen because of falls.In this episode of The Brain Injury Insider, we discuss new research published in Pediatrics on early childhood concussion. The study, “Frequency and Predictors of Persisting Symptoms 1 Year After Early Childhood Concussion,” looked at children ages 6 months to 6 years old and found that about one in four developed prolonged symptoms after a concussion.These lingering symptoms can matter because they may affect learning, behavior, and development later in childhood.Because toddlers and preschoolers cannot always explain how they feel, parents should watch for changes such as:• Behavior changes• Excessive irritability• Appetite changes• Less social engagement• Stomach aches• Increased clinginess• Changes in sleep, mood, play, or daily routinesA concussion is not “just a bump on the head.” If your child has a head injury and something seems off afterward, it is important to report the injury to your child’s doctor, describe the changes you are seeing, and ask what follow-up may be needed.This episode also covers practical tips for parents, including tracking symptoms over the first days and weeks, documenting changes, and asking about follow-up care if symptoms persist or interfere with daycare, preschool, or normal routines.Thanks for joining us this week on The Brain Injury Insider.     Brain Injury Insider Episode 145: New Outpatient Guidelines for Traumatic Brain Injury: A Major Shift in TBI Care

    Today on The Brain Injury Insider, I want to call your attention to an important breakthrough in the management of traumatic brain injury:  The development of outpatient clinical practice guidelines designed to standardize care for the millions of Americans recovering from TBI.Although protocols exist for emergency care and hospital management of people with a brain injury, no protocols exist regarding outpatient management following emergency care or inpatient hospital care, and no guidelines exist for individuals who never were seen in a hospital. Patient care following a traumatic brain injury is inconsistent, fragmented and often inadequate.  This includes people suffering from concussions as well as other types of brain trauma. Less than half of individuals with a TBI receive any follow-up care after they leave the emergency department.    Failing to provide follow up care contributes to prolonged symptoms, unnecessary disability, and missed opportunities for early intervention.To address this, the Action Collaborative on TBI Care, part of the National Academies of Sciences, Engineering, and Medicine has released evidence-based guidelines that focus on the first six months after injury.     Brain Injury Insider Episode 144: New Study on Subdural Hematoma; When Surgery Isn’t Always Better

    Today, I’d like to discuss a new study examining treatment options following the development of a traumatic subdural hematoma, a condition when blood collects rapidly on the surface of the brain. This bleeding may cause pressure on the brain and lead to worsening injury, cognitive impairment, and even death.Falls are the leading cause of traumatic brain injury, particularly among older adults and children. A slip, trip, a misstep can lead to a blow to the head, or the sudden deceleration of the brain when it moves inside the skull, causing damage to brain tissue and blood vessels within the brain.In an article published in the JAMA Journal entitled “Acute Surgery vs Conservative Treatment for Traumatic Acute Subdural Hematoma,” researchers looked at data from 18 Level-1 trauma centers across the U.S. They focused on over 700 patients who had developed an acute subdural hematoma after head trauma.     Brain Injury Insider Episode 138: Can Heading a Soccer Ball Damage Your Brain?

    Today I want to discuss a recent study with potentially wide implications for anyone who plays soccer — even at the amateur level — and even for those who have never been diagnosed with a concussion.  Heading a soccer ball repeatedly might damage your brain, even if you never are knocked out, see stars, or diagnosed with a concussion. In one of the largest studies yet to look at the effects of soccer headers in non-professional players researchers found: Players who had repeatedly headed a soccer ball showed changes in a part of the brain called the gray-white matter interface, an area located in the orbitofrontal region.  An area just behind the eyes.  When the skull is struck and then the brain bounces around the interior of the skull, this region of the brain is sensitive to injury.   The brain changes found on testing were related to memory and learning.  Importantly, many of these players had no history of diagnosed concussion.     Brain Injury Insider Episode 135: Research Reports That TBIs May Speed Up The Aging Process in Post 9/11 Veterans

    Today I want to share some fascinating—and concerning new research.  The research reports that traumatic brain injuries may speed up the aging process in post-9/11 veterans.  Researchers looked at blood samples from over a thousand veterans, most in their 30s.     Brain Injury Insider Episode 133: CTE, Concussions & the Midtown Manhattan Tragedy

    Today, I would like to discuss the tragic midtown Manhattan shooting on July 28, 2025, in which the shooter—Shane Devon Tamura, a 27‑year‑old former high school football player left a suicide note claiming he suffered from CTE, or chronic traumatic encephalopathy, and blamed the NFL for concealing the dangers of football. He asked for his brain to be studied post‑mortem.Tamura never played in the NFL—he only played in high school—and there’s conflicting information regarding a history of head trauma or concussions. CTE can only be diagnosed via autopsy.CTE is a degenerative brain disease marked by abnormal tau protein build‑up.Symptoms typically appear years after repeated head impacts including memory loss, confusion, mood swings, depression, aggression, and suicidal thoughts.CTE is not exclusive to football. Repetitive sub concussive hits—or those without obvious symptoms—can be harmful.It affects not only professional athletes but also high school and college players, as well as athletes in hockey, boxing, soccer, rugby, and even military veterans.Repetitive head trauma, early exposure to head trauma and concussions, or even repetitive head trauma—especially when not fully recovered from—pose serious risks, even at amateur levels, and can contribute to the development of CTE.Athletes, coaches, parents, and officials need to be educated to recognize the symptoms of a concussion including headaches, confusion, dizziness, sensitivity to light/sound, and emotional changes.If there’s any suspicion of a concussion—“when in doubt, keep them out”. No matter the level or sport. Slow, conservative return‑to‑play protocols must be enforced; rest and medical clearance should be required before resuming activity.My partner Shana De Caro and I opposed the NFL settlement before the Court of Appeals and the United States Supreme Court. We showed the NFL settlement failed to compensate the vast majority of players who suffered the effects of repetitive head trauma.Discussing CTE we informed the Court, the existing NFL concussion settlement placed an unfair deadline: only players diagnosed with CTE by a certain date could qualify.     It’s summertime and today we’re taking a hard look at a fast-growing threat to public safety—electric scooters and e-bikes.  They may be convenient, but they’re causing chaos on our streets—and most people aren’t even aware of the risks. According to a recent article in USA Today, the statistics are alarming.  Emergency rooms are reporting a spike in injuries, particularly head trauma and traumatic brain injury.  As one trauma surgeon told USA Today, 'We're seeing people come in with fractured skulls, concussions, and brain bleeds—just from what looked like a simple fall.' And these aren't just riders.     Our armed service members will get some more protection under the National Defense Reauthorization Act passed by Congress in the closing days of 2024.Weapons must be changed to reduce blast exposure to troops operating these weapons and those in close proximity.  Development of future weapon systems must consider brain safety.Brain injury caused by the sound waves produced during a blast continues to plague our service members.  Amid a growing body of evidence of injuries and devastating symptoms caused by enemy attacks and from routine military activities, Congress and the Department of Defense need to take brain injury our service members seriously.The signature wound of military service needs to be addressed.     Brain Injury Insider Episode 120: What Is The Risk of Stroke After a Traumatic Brain Injury?

    The increased risk of stroke following any type of traumatic brain injury is well known.A new comprehensive review of the medical literature in an article entitled Risk of Stroke After a Traumatic Brain Injury, a systematic review and meta-analysis published in the ACTA Neurological Journal this November, confirms this link.Traumatic brain injury is a leading cause of death and disability in the United States and is associated with increased risk of neurological disorders including seizure disorders, Parkinson’s Disease, ALS, dementia, and Alzheimer’s disease. This comprehensive literature review confirms the relationship between traumatic brain injury and stroke.  The review found across the spectrum of all types of TBI, mild, moderate, and severe, patients were at an increased risk of both ischemic strokes caused by a blood clot in the brain and hemorrhagic stroke caused by bleeding within the brain.     Screening of patients in hospital emergency departments for concussions and other types of traumatic brain injury is challenging due to the many ways symptoms may manifest.Repeated studies have shown that emergency departments often fail to properly diagnose concussions leading to the inability of individuals to receive timely or proper treatment.And even when a concussion is diagnosed, there is often a failure to assess the patient for a risk of future consequences previously called the post-concussion syndrome and now known as persistent post concussive symptoms.A new study emphasizes the need to screen patients for ocular difficulties in the emergency department as an accurate tool to assess the risk of developing long term post-concussion consequences.The study examines screening for convergence insufficiency symptoms.Convergence insufficiency affects the ability of both eyes to work together to form a single image resulting from damage to the nerves controlling eye muscles.Common symptoms include visual fatigue, double vision, headaches, or trouble concentrating.
