Learn more about everything at our home page www.diabetes-connections.com  Transcript & links:    Okay.. our top story this week: XX A large Swedish study found that the increased risk of being diagnosed with type 1 diabetes after COVID-19 infection is mostly limited to the first 30 days after infection and does not continue long term. Researchers followed nearly the entire Swedish population under age 80 from 2020 through 2023 and found that while SARS-CoV-2 infection was linked to a temporary rise in new type 1 diabetes diagnoses, the risk declined over time. The study also found no evidence that COVID-19 vaccination increases the long-term risk of developing type 1 diabetes. Vaccination did not significantly change the relationship between COVID-19 infection and diabetes risk, and any small increase in diagnoses seen among adults shortly after a first vaccine dose was not seen after later doses or during longer follow-up. The researchers concluded that their findings do not support changing current COVID-19 vaccination recommendations because of concerns about type 1 diabetes risk. https://www.infectiousdiseaseadvisor.com/news/covid19-infection-may-increase-short-term-type-1-diabetes-risk/ XX Two new studies are challenging the traditional view that type 1 diabetes develops solely because the immune system attacks insulin-producing beta cells. Researchers from Indiana University found evidence that beta cells themselves may play an active role in determining whether they survive or succumb to the stresses that lead to type 1 diabetes. In the first study, scientists discovered that some healthy human beta cells can quickly activate an antiviral defense system when exposed to interferon-alpha, an immune signal often produced during viral infections. This response relies on molecules called reactive oxygen species (ROS), which are usually associated with cell damage but, in this case, appeared to help switch on protective antiviral genes. Researchers found this defense program in healthy cells and in people at risk for type 1 diabetes, but not in beta cells from people who already had the disease. The findings suggest that losing this built-in defense mechanism may make beta cells more vulnerable during the development of type 1 diabetes. The second study focused on autophagy, the process cells use to recycle damaged or worn-out components. Using a new imaging technique, researchers observed that beta cells in a mouse model of type 1 diabetes showed defects in autophagy before blood sugar levels began to rise and even before a full immune attack was underway. This suggests that problems inside the beta cells may occur early in the disease process rather than being caused entirely by the immune system. Together, the studies point to a more complex picture of type 1 diabetes. While they do not show that beta-cell defects cause the disease, they suggest that differences in how beta cells respond to stress, viral signals, and cellular damage may influence who develops type 1 diabetes and how the disease progresses. https://medicalxpress.com/news/2026-06-beta-cells-players-diabetes.html XX Researchers have created the most detailed map yet of how the human pancreas develops during childhood, offering new clues about why children are especially vulnerable to developing diabetes. The study, published in Nature Communications, examined pancreatic tissue from 123 children without diabetes, ranging from newborns through age 10. Using advanced imaging techniques, scientists tracked how insulin-producing islet cells grow and mature during the first decade of life. The researchers found that pancreas size varies dramatically at birth, with some infants having pancreases nearly four times larger than others. They also discovered that insulin-producing beta cells grow more slowly after birth than previously thought, suggesting that much of a person's lifelong beta cell capacity may be established before birth and during early childhood. Other findings showed that insulin-producing cells mature earlier than glucagon-producing cells and that new hormone-producing cells may continue to form after birth. The researchers hope this new understanding of pancreas development will help scientists identify diabetes risk earlier and develop better strategies for prevention and treatment in children. https://news.vumc.org/2026/06/29/unlocking-diabetes-secrets-pediatric-organ-donors-help-map-a-path-to-a-cure-and-prevention/   XX At the American Diabetes Association annual meeting, 2-year results from the SUPPRESS-EARLY trial showed that initiating tirzepatide (Mounjaro, Zepbound) early in the course of type 2 diabetes led to substantially higher rates of near-normal glycemic control and also led to broader metabolic improvements compared with intensive conventional therapy.   In this MedPage Today video, investigator Stefano Del Prato, MD, of the University of Pisa in Italy, discusses the findings.     Following is a transcript of his remarks:   What happened is that in the tirzepatide-treated arm, 85% of the population at the end of the second year was on the maximum dose of tirzepatide 15 mg. And then the remaining 15 with the different doses.   Interestingly, in the population that had been treated with the intensive conventional approach, 85% of them ended up to have, on top of metformin, a GLP-1 receptor agonist, mainly represented by subcutaneous semaglutide [Ozempic, Wegovy], 60+%, another 15% on oral semaglutide [Rybelsus], and the remaining on dulaglutide [Trulicity].   And I have to say that maybe the recommendation to really push along the line to try to achieve and to strive to achieve [glycemic] control was successful in these individuals. Because the population that had been recruited in the study started off with a baseline A1C of 7.8% and it went down to 6.3% in the conventionally intensive treatment, which is not bad at all, on average is below the target of 6.5%.     However, when we look at the effect of tirzepatide, the final level of A1C at the end of the second year was 5.6%, which is on average below the upper limit of the normal range for A1C, 5.7%. This also translates into more people not only achieving normal glycemia, if we can define normal glycemia as A1C below 5.7%, greater than what we observed in conventionally treated individuals. So it was around three times more people achieving an A1C of 5.7%, in the range of around 65%, as compared to 28% with people on a conventional optimized treatment.   Now, this is not surprising knowing the potency of tirzepatide. But again, going back to the rationale of the design, can we change what is the natural history of the disease? This seems to be at least of interest and it's possibly changing the trajectory of the disease for glycemic control, as I mentioned, but also in terms of the body weight and waist circumference because both body weight and waist circumference went much lower with tirzepatide compared to the conventional treatment.   Tirzepatide also was associated with an improvement in the lipid profile, in particular with the LDL, triglycerides, and the triglyceride concentration and non-HDL cholesterol, and also with a statistically significantly lower systolic blood pressure with a numerical reduction in the diastolic blood pressure.   And also the other thing that probably will ... become more apparent with the study continuing is that the investigators were allowed to add on any other treatment ... needed to achieve their target. So tirzepatide was just metformin and tirzepatide. In the control group, there was already 10% of people who were receiving two drugs on top of the metformin.   So another potential result of the trial is that it's possible to achieve and maintain better glycemic or better metabolic control over the time without really needing to increase the number of medications in order to achieve that goal. And we know that type 2 diabetes is a progressive condition often requiring intensification of the treatment.   So these initial results really stand for a great opportunity with tirzepatide. Of course, we need to wait for the 4 years just to confirm that this is indeed the case, but the initial result seems to point along that line. https://www.medpagetoday.com/meetingcoverage/adavideopearls/121967 XX A study from the University of Virginia found that high blood pressure is extremely common among people with diabetes, even among those who believe their blood pressure is under control. Researchers measured blood pressure in 172 adults with type 1 or type 2 diabetes during routine eye clinic visits and found that only 8% had normal readings. About half had stage 2 hypertension, and more than 10% had blood pressure levels high enough to be considered a medical emergency. The study also revealed that many patients were unaware of how serious their blood pressure problems were. Among those who thought their hypertension was well controlled, more than half still had stage 2 hypertension. Nearly 60% of participants were advised to contact their primary care provider, and one patient required an emergency department referral. Most patients supported blood pressure screening during eye exams, leading researchers to suggest that routine blood pressure checks in ophthalmology clinics could help identify undiagnosed or poorly controlled hypertension before it leads to serious complications such as heart attack, stroke, or worsening diabetic eye disease. https://medicalxpress.com/news/2026-06-routine-eye-exams-reveal-stage.html XX What is going on with retatrutide? This is the next generation GLP-1 but it's not authorized outside of clinical trials. Big investigation by CBS shows retatrutide is for sale all over the internet, a phenomenon they say has no modern precedent. CBS News identified more than 120 websites selling or promoting retatrutide, including more than 50 clinics staffed by licensed medical professionals. After being contacted by CBS News, at least 21 clinics abruptly removed retatrutide from their websites or changed the language to state they don't offer it. Others defended prescribing it, saying they're confident enough in results from clinical trials sponsored by drugmaker Eli Lilly that they didn't need to wait for the FDA's independent, rigorous review. An FDA spokesperson said retatrutide "has not been found safe or effective for any condition," adding that it "cannot be manufactured or distributed except for investigational use." The Justice Department is prosecuting two cases – in Utah and Florida – involving the sale and prescription of retatrutide. But the first line of enforcement is often at the state level. Ohio's Board of Pharmacy has taken action against several pharmacies and clinics providing retatrutide, and just last month, Alabama's Medical Board warned physicians against prescribing research-grade medications. The FDA has sent 14 warning letters to companies that have advertised retatrutide since 2024. Of these, at least six have continued to offer it online, including a business called Pink Pony Peptides. A TikTok account associated with the firm responded to the warning in April by taunting the FDA, boasting that the business "just had the best 24 hours ever." In May, Eli Lilly announced that participants in a large clinical trial taking the highest dose of retatrutide lost an average of 28% of their body weight over 80 weeks. Side effects – including nausea, diarrhea, constipation and vomiting – were comparable to similar therapies, the company said. "Anyone purporting to sell retatrutide to consumers is breaking the law," an Eli Lilly spokesperson said https://www.cbsnews.com/projects/2026/experimental-weight-loss-drug/ XX Pioneering research has developed a new way of creating carbohydrate-based medicines, which could ultimately replace costly drugs for common health conditions, using two cheap basic ingredients – table sugar and vinegar. These medications include SGLT2 inhibitors, widely prescribed drugs used to treat type 2 diabetes, heart failure and chronic kidney disease. Co-lead author Professor Phil Baran, Dr. Richard A. Lerner Endowed Chair at Scripps Research, in San Diego, California, said: "The point of this is to show that anyone in a garage can make an SGLT2 inhibitor with reagents that are widely available. At the moment, the Scottish Medicines Consortium does not have an appraisal of Tzield on the go, so, there is likely to be a disparity in access within the UK for the time being. In Northern Ireland, access will depend on a review and adoption of NICE guidance.  Sanofi is expecting to see an uptick in momentum thanks to two subsequent FDA approvals, one in children as young as one with stage 2 T1D, and a second to delay the decline in endogenous insulin production in children aged eight to 17 years recently diagnosed with stage 3 T1D. btw you might here more people referring to stage 4 diabetes. They've added that to include people diagnosed with type 1 who've been on insulin for a longer period of time – basically long enough to not be eligible for the current guidelines for Tzield. https://www.bbc.com/news/articles/ce8mzd94r76oXX XX Obesity Association, a division of the American Diabetes Association® (the association), announced the next section in the Standards of Care in Overweight and Obesity, "Screening, Diagnosis, Evaluation, and Staging of Obesity in Adults," published in Diabetes, Obesity, and CardioMetabolic CARE® and BMJ Open Diabetes Research & Care.  Key highlights of the guidance: Early screening: Annual BMI screening with emphasis on tracking weight trends to identify risk earlier, including a longitudinal life-event weight graph tool for standardized assessment. Enhanced diagnosis: Combines BMI with waist measurements and population-specific thresholds to improve accuracy. Notably, the guidelines recommend that BMI in the overweight range together with central adiposity measurements warrant a formal obesity diagnosis. Comprehensive evaluation: Holistic assessment including medical, behavioral, and social factors. Offers a fully integrated obesity diagnostic algorithm. Risk stratification: Use of tools like the Edmonton Obesity Staging System to guide care. Chronic care approach: Ongoing monitoring and follow-up to support long-term management. San Diego-based Dexcom plans to formally begin the new app rollout in July for Apple iPhone and Android users in the U.S. Dexcom said its reimagined app aims to make glucose insights easier to understand and act on. It hopes to help build awareness of how food, activity, sleep and stress influence overall wellbeing.   The company also reiterated plans to launch Stelo internationally. It expects to bring the sensor to the UK, Australia, New Zealand and South Korea later this year, continuing into 2027. https://www.drugdeliverybusiness.com/dexcom-launches-enhanced-stelo-app/ XX MannKind Corporation recently announced it received a grant from Breakthrough T1D to support the INHALE-1 clinical study of Afrezza, its ultra rapid-acting inhaled insulin, in newly diagnosed pediatric type 1 diabetes patients aged 10 to under 18 years. The French Open champion returns to the All England Club looking to build on his first Grand Slam title and gain ground on Carlos Alcaraz in the race for the No. 2 spot in the ATP rankings. Zverev has an opportunity to make up points quickly after a first-round exit at Wimbledon last year. But his final tune-up before Wimbledon came with an unexpected challenge. During his semifinal loss to Taylor Fritz at the Halle Open, Zverev said a malfunctioning glucose sensor led to serious diabetes management issues on court. The sensor incorrectly showed his blood sugar was high when it was actually low, causing him to take more insulin than needed. "I had huge problems with the sugar because the sensor I use gave me a completely incorrect reading," Zverev said after the match. "During the match, or rather during the first 45 minutes, I had to consume about 350 grams of sugar. I felt absolutely terrible." Despite feeling unwell, Zverev pushed the match to three sets before falling 6-7(4), 6-4, 7-5 to Fritz. He credited his opponent with playing the better match and said the diabetes-related issue was not an excuse for the result. Zverev, who was diagnosed with type 1 diabetes at age 4, uses Medtronic diabetes technology to help manage his glucose levels while competing on the ATP Tour. He said the sensor error was the first major problem he has experienced after nearly a decade of using the technology. The German said the incident should not affect his Wimbledon preparations. It's the stage where the body can no longer produce enough insulin on its own to manage blood sugars you need to start insulin. This approval is for kids ages 8-17 within 8 weeks of a stage 3 T1D diagnosis. The drug is designed to prevent the body from rejecting transplanted cells while avoiding some of the side effects associated with standard anti-rejection medications. You've probably heard about this as the Eledon study – many of the participants have been very active on social media. It was presented at ADA. transplants.https://www.breakthrought1d.org/news-and-updates/tegoprubart-islet-transplant-all-participants-off-external-insulin/ XX New data suggest that acmopatide (ack-MOW-puh-tyd) (CT-868), an experimental once-daily dual GLP-1/GIP receptor agonist, may help people with type 1 diabetes improve blood sugar control, lose weight, and reduce insulin use. Across all doses, participants lost up to 7% of their body weight and reduced insulin use by as much as 15%. The study lasted just 16 weeks, so researchers say longer-term data will be needed to determine whether the benefits can be maintained and whether lower insulin requirements can be achieved without increasing the risk of hypoglycemia. XX A new combination therapy that pairs an amylin analog with semaglutide improved both blood sugar levels and weight loss in several groups of people with type 2 diabetes. The once-weekly injectable, known as CagriSema (KAG-ruh-SEM-uh), was evaluated in three Phase 3 REIMAGINE studies. In people early in the course of type 2 diabetes, researchers reported A1C reductions of up to 1.8 percentage points and significant weight loss compared to placebo after 40 weeks of treatment. That's , the company's upcoming hybrid closed-loop system for people with type 1 and type 2 diabetes. The main difference between the Omnipod 6 and Insulet's current Omnipod 5 patch pumps is that the new system has a lower glucose target of 100 mg/dL and better Bluetooth connectivity Insulet also shared progress on a fully closed-loop system designed specifically for type 2 diabetes. It  does not require carb-counting or insulin bolusing ahead of meals. Physicians also don't need to program the starting settings.   XX Abbott shared new research highlighting challenges in identifying and managing diabetic ketoacidosis (DKA). The studies coincide with the company's development of Libre Duo, a dual glucose-ketone sensor that continuously tracks both measurements. Abbott reported that DKA can be difficult to recognize when patients first arrive at the hospital, based on data from more than 100,000 people. The company has submitted the dual sensor to the FDA and recently received CE Mark approval in Europe. XX   Dexcom announced its acquisition of Nutrisense, a company that combines continuous glucose monitoring with nutrition coaching and behavioral support. At ADA, the company also presented results from the CONNECT study showing significant A1C reductions and improved glucose control in people with type 2 diabetes not using insulin. The findings add to growing evidence supporting CGM use beyond intensive insulin therapy. We did an episode with CEO Jake Leach at ADA about these announcements as well as updates on G8, their hospital product and much more. XX Sequel Med Tech reported positive clinical results evaluating its twiist automated insulin delivery system in people with type 2 diabetes. The study showed improvements in A1C and time in range over 13 weeks XX Senseonics presented new real-world data supporting the performance of its Eversense 365 implantable CGM. The analysis included more than 12,000 sensors and demonstrated sustained accuracy and effectiveness in both open-loop and automated insulin delivery settings. Researchers also evaluated Eversense use with Sequel Med Tech's twiist system. The findings support broader use of long-term implantable CGM technology.   -- MiniMed used ADA 2026 to spotlight two recently cleared diabetes management systems. The MiniMed Flex pump offers a smaller, smartphone-controlled insulin pump option, while MiniMed Go combines the InPen smart insulin pen with Abbott's Instinct sensor. The products received FDA clearance earlier this year. XX Tandem Diabetes Care highlighted data supporting the use of its Control-IQ automated insulin delivery technology during pregnancy. Results from the CIRCUIT trial showed users spent approximately three additional hours per day in the recommended pregnancy glucose range compared with standard therapy. The findings helped support recent regulatory approvals for pregnancy use in both Europe and the United States. Tandem also expanded indications for adults with type 2 diabetes. XX Beta Bionics presented real-world data from the first three years of iLet Bionic Pancreas use. The company reported a 25% improvement in time in range among users, along with positive feedback from clinicians about simplified diabetes management. The iLet system requires only a user's weight to begin therapy and eliminates carbohydrate counting. Beta Bionics also highlighted growing access to near-real-time outcomes through its public data dashboard. XX MannKind presented new findings supporting its Afrezza inhaled insulin at ADA 2026. A post-hoc analysis of the INHALE-1 study found that pediatric users reported greater treatment satisfaction compared with those using rapid-acting injected insulin. The results come shortly after FDA approval expanded Afrezza's indication to include children. We did a bonus episode with one of the lead investigators of the study that lead to that approval. XX Adaptyx presented early clinical data supporting a wearable sensor that continuously measures cortisol levels. The device successfully tracked cortisol changes during both controlled testing and overnight monitoring in first-in-human studies. Company leaders say cortisol plays a major role in conditions including diabetes, hypertension, and depression. The technology uses synthetic DNA-based molecular switches to generate real-time readings.   XX Biolinq shared new clinical findings for its Shine continuous glucose monitoring system. The needle-free device combines glucose monitoring with activity and sleep tracking .The system received FDA clearance in 2025. They're also looking at measuring lactate through the sensor. XX Long-time T1D advocate Kent Schnakenberg died last week. Schnakenberg was known in his community for using his love of bicycling to raise awareness of Type 1 diabetes. He also advocated for improving the lives of those living with the disease. Inspired by his niece, Michelle, who was diagnosed with juvenile diabetes when she was 13 years old, since 2014 he has traveled around the country cycling thousands of miles, speaking to hundreds and hundreds of kids and raising Money. According to Schnakenberg's family, he suffered a head trauma incident in his home on Wednesday. I spoke to Kent years ago – I believe the first year of the podcast. The FDA okayed MannKind's afrezza for children 6 and older with type 1 and type 2 diabetes. MannKind says its proprietary Technosphere drug delivery platform enables the rapid absorption of insulin into systemic circulation. This follows FDA approval earlier this year for an update that revises recommendations for the starting mealtime dosage when patients switch from subcutaneous mealtime insulin regimens. MannKind also completed enrollment in February for a study evaluating the initiation of Afrezza therapy shortly after type 1 diabetes diagnosis in pediatric patients.   The company said it made Afrezza available for eligible patients for $35 or less per month. Desmond Schatz, professor of pediatrics at the University of Florida College of Medicine, said: "Mealtime insulin can be especially challenging for children because eating and snacking patterns, activity levels, and daily settings like school and sports often vary. With its rapid onset and dosing at the start of a meal, Afrezza may help clinicians better match insulin therapy to how children and families live day to day, while offering a needle-free mealtime option." Finerenone (fy-near-uh-known) is currently approved in the US for adults with CKD associated with type 2 diabetes and for adults with heart failure with left ventricular ejection fraction of 40% or greater. Chronic kidney disease (CKD) is present in over one-third of adults with diabetes, and because it's such a serious condition, interventions are needed to reduce its incidence and help people live a long and prosperous life. https://www.docwirenews.com/post/fda-grants-priority-review-to-finerenone-snda-for-type-1-diabetes-associated-ckd XX Abbot gets European approval for the world's first dual glucose‑ketone sensing technology for people with diabetes. They're calling this Libre Duo and Libre Duo 10 Day, and it's designed to continuously measure glucose and ketone levels every minute. Abbott plans to begin launching Libre Duo systems in select European countries later this year. Libre Duo delivers up to 15 days of wear and will be offered to adults ages 18 and older. Libre Duo 10 Day offers up to 10 days of wear and is intended for people ages 2 and older. Abbott is also working with leading pump companies to allow automated insulin delivery (AID) systems to connect with the sensors. https://abbott.mediaroom.com/2026-05-27-Abbott-secures-CE-Mark-for-worlds-first-dual-glucose-ketone-sensing-technology-for-people-with-diabetes   XX Huge recall for Omnipod. Insulin says a manufacturing issue through ongoing product monitoring that could result in insulin under-delivery  with specific lots of its Omnipod 5, Dash and Eros pods. Insulet said the scope of this action reaches approximately 7 million pods. This issue is separate from the March recall that affected certain Omnipod 5 lots. According to the Acton, Massachusetts-based company, some of its affected pods may have a small tear in the tubing (cannula) just above the skin. This tear lands between the pod and the point where the cannula enters the body. If this occurs, insulin may leak outside of the device instead of being fully delivered into the body as intended. This may lead to under-delivery of the therapeutic.   Individuals using an affected pod may notice wetness on the skin or pod adhesive or detect the smell of insulin. However, some cases may prove difficult to detect and go unnoticed. Of the approximately 7 million pods included in the action, approximately 60% have been consumed or are expired. The pods affected by the correction represent approximately 8.5% of the 2025 global Omnipod pod prodcution. Insulet says it has sufficient supply to replace affected pods. It expects no disruption to product availability. The company said it has notified the FDA and all other relevant regulatory authorities of its action.   The full list of affected pod lots can be found here. https://www.massdevice.com/insulet-another-omnipod-5-recall-dash-eros/ XX Dexcom is warning that certain scrapped glucose sensors have been stolen and resold. Dexcom said it has not received any reports of severe adverse events associated with the stolen product. One lot of scrapped devices carries a risk of infection for sensors that are not properly sterilized, and another lot had an elevated internal testing failure rate, meaning users would have an increased risk of having no sensor readings available. Dexcom said the affected sensors were stolen during the destruction process and then sold by third parties. The company routinely scraps sensors that do not meet its standards. The sensors are sent to a third-party vendor for destruction and recycling.   Dexcom said it traced sales of the stolen devices to Pharmsource, which is not an authorized Dexcom distributor but supplies some independent pharmacies and U.S. durable medical equipment distributors. Because of this, pharmacies that purchase products from Pharmsource should review their inventory, Dexcom said.   People with sensors from the affected lots should not use those sensors and can call customer support to request replacements. Dexcom has set up a website to help users check if their devices are affected. https://www.medtechdive.com/news/dexcom-warns-of-scrapped-glucose-sensors-being-resold/821139/ XX XX   Beta Bionics plans to debut its first insulin patch pump by the end of the second quarter of 2027, subject to Food and Drug Administration clearance. The device, called Mint, would be compatible with Beta Bionics' interoperable automated glycemic controller, a software that allows for the pump to automatically adjust insulin delivery based on readings from a glucose sensor. Beta Bionics first unveiled the prototype for Mint last year at the American Diabetes Association's Scientific Sessions. The device is expected to have a similar size and wear time, at three days, to Insulet's patch pumps on the market. It would have a 200-unit insulin reservoir.   Mint differs by containing a mix of reusable and disposable components. Beta Bionics plans to make the device exclusively available in the pharmacy channel, building on its existing agreements for its current iLet insulin pump. Beta Bionics is one of several diabetes tech companies developing patch pumps to compete with market leader Insulet. Tandem Diabetes Care and Medtronic spinoff MiniMed have also announced planned patch pumps. XX A new study suggests that higher long-term exposure to food colouring additives — including both synthetic and natural colourings commonly found in processed foods and beverages — may be associated with an increased risk of developing type 2 diabetes. Researchers analyzed data from more than 108,000 adults in the French NutriNet-Santé cohort between 2009 and 2023, following participants for a median of just over eight years. During that time, 1,131 participants developed type 2 diabetes. The study found that people with the highest intake of total food colouring additives had a 38% higher risk of developing type 2 diabetes compared with non- or low-consumers.   Several specific additives were linked to increased risk, including caramel colouring additives such as total caramel (E150 family), plain caramel (E150a), sulphite ammonia caramel (E150d), and beta-carotene (E160a). Additional associations were observed for curcumin (E100), anthocyanins (E163), paprika extract (E160c), lutein (E161b), and cochineal-derived colourings (E120). "Our findings revealed positive associations between widely consumed food colouring additives and type 2 diabetes incidence," the authors wrote, adding that further research is needed to better understand the mechanisms behind the findings and whether food colouring regulations should be reevaluated. https://www.medscape.com/viewarticle/use-common-food-colours-tied-high-type-2-diabetes-risk-2026a1000hes XX Big news for Australia – their Therapeutic Goods Administration (TGA) approves Tzield. Tzield is now approved in Australia to delay the onset of stage 3 (or clinical) T1D in people aged eight years and older with stage 2 T1D – the early, pre-symptomatic stage of the condition, where changes in blood glucose levels have begun but insulin therapy is not yet required. According to a study, the drug metformin doesn't just help patients to effectively manage their type 2 diabetes. it may also give older women a better chance of living to 90. Scientists in the US and Germany used data from a long-term US study of postmenopausal women.   Records for a total of 438 people were selected – half of whom took metformin to treat diabetes, and half of whom took a different diabetes drug, sulfonylurea.   While there are some caveats and asterisks to the study, those in the metformin group were calculated to have a 30 percent lower risk of dying before the age of 90 than those in the sulfonylurea group. The study used age 90 as the marker for 'exceptional' longevity. However, scientists aren't yet sure that the drug extends lifespan, especially in humans – which is part of the reason for this study. RCTs could follow further down the line to dig deeper into these results, the researchers suggest. We've been talking about this with CEO Jake Leach for a while now – it will be a 50% smaller with what they're calling advanced sensing capabilities. According to Leach, G8 will adapt to the physiologic variability of each user. It has additional technology built in, based on a new silicon chip design and algorithm. 15 day wear is now the baseline for all Dexcom sensors moving forward. At launch the G8 will only measure glucose but the plan is for a multi-analyte version to follow. That would measure ketones and potassium. Ketones we know – but potassium is very important for people with kidney and possible for people taking some diabetes meds. It's an interesting space to watch.. btw, analyte is just a medical word for the specific thing you're measuring – the target of the test you're running. we're going to hear that word a lot I think..   Looks like an FDA submission for the G8 next year.. with an outside the US launch the following year. https://www.drugdeliverybusiness.com/dexcom-unveils-next-gen-g8-cgm/ XX Glucotrack has submitted its implantable continuous blood glucose monitor (CBGM) for FDA IDE, that's investigational device exemption and would enable the company to initiate a U.S. clinical study for the fully implantable technology. Rutherford, New Jersey-based Glucotrack's device features no on-body external component. The company aims to offer it for three years of continuous, accurate blood glucose monitoring for a more convenient, less intrusive solution. Unlike traditional CGMs that measure glucose in interstitial fluid, the CBGM measures glucose levels directly from the blood. The implant goes five centimeters within the subclavian vein. Glucotrack's active implantable device has a small battery and some electronics that go just under the skin in the pectoral region. However, this system would be one device that features both the pump and CGM technology.   PharmaSens and SiBionics also have a collaboration aimed at developing the all-in-one solution. They are jointly developing the next-generation Niia insulin patch pump with a SiBionics CGM. PharmaSens expects a second feasibility study in the second quarter to evaluate the next-generation pump with SiBionics' CGM.   PharmaSens says the clinical feasibility study of Niia demonstrated for the first time ever that the combined offering is, in fact, feasible. It believes its device addresses the need for alternatives to multi-device diabetes management. systems.   Aggregated MARD for the investigational device came in at 11.6%. A total of 4088 patients receiving GLP-1-based therapies (median age, 43 years; 34.3% men) were propensity score matched with an equal number of patients not receiving the treatment. The findings from SURMOUNT-MAINTAIN and ATTAIN-MAINTAIN, were presented at the 33rd European Congress on Obesity (ECO) and published in The Lancet and Nature Medicine, respectively.   "Weight regain remains one of the biggest challenges in obesity care, and is often the result of treatment interruptions that cause biology to work against patients, undoing the progress they've made," said Louis J. Aronne, M.D., FACP, DABOM, founder and Chair Emeritus of the American Board of Obesity Medicine, former president of The Obesity Society, Fellow of the American College of Physicians, world-renowned obesity specialist and Lilly consultant. These lab-grown cells not only respond strongly to glucose but were also able to restore blood sugar control when transplanted into diabetic mice. When transplanted into diabetic mice, the cells gradually restored the animals' ability to regulate blood sugar. Published in Nature, the study describes a potential method for directly reducing glycogen, the stored form of sugar in the body. These scientists discovered that glycogen can be directly regulated by ubiquitin, a protein best known for marking damaged proteins for recycling or removal. The study is the first to show that ubiquitin can regulate glycogen in humans, overturning more than 50 years of scientific understanding. Researchers found that the mix of hormone-producing cells in the pancreas varies widely from person to person, and that variation plays a central role in how the body regulates blood sugar. The study involved a deep dive into islet cell function that is linked to donor traits associated with observable characteristics, or phenotype, such as sex, race and ethnicity, as well as genetic information, or genotype, including predicted ancestry and genetic risk for both type 1 and type 2 diabetes. The findings highlight that islet cell composition, rather than the physical size and shape of islets, is a key factor in regulating hormone release. The team found that the makeup of pancreatic islets plays a major role in how effectively they release insulin and glucagon — key hormones that regulate blood glucose. Islets with a higher proportion of insulin-producing beta cells showed stronger insulin secretion in response to various stimuli, while higher levels of alpha and delta cells were generally linked to reduced insulin output. In addition, the researchers found that islet hormone secretion is affected by donor traits, such as sex, race and ethnicity and their genetic makeup, including ancestry predicted from genetic testing and genetic risk for type 2 diabetes. Combined, the findings of the study have significant implications for understanding the factors that may predispose people to diabetes. "This study is the tip of the iceberg," said Dr. Evans-Molina. "We hope this dataset becomes useful to the entire diabetes research community and that researchers use it to answer questions about the genotype-phenotype correlation within these data."   https://www.nih.gov/news-events/news-releases/nih-funded-study-maps-human-pancreatic-islet-cells-offering-new-clues-diabetes-risk XX XX XX Research published recently in JAMA Network Open offers illuminating evidence suggesting there is a positive association between GLP-1 agonists—drugs commonly used to treat obesity and diabetes—and better outcomes among breast cancer patients.   "This study suggests that GLP-1 drugs may offer protective benefits potentially improving survival and recurrence risk in some female patients with breast cancer – whether this is related to weight control, improve cardiovascular health or other mechanisms remains to be studied," said study senior author Bernard F. Fuemmeler, Ph.D., MPH, associate director for population sciences and the Gordon D. Ginder, M.D., Chair in Cancer Research at VCU Massey Comprehensive Cancer Center.   Breast cancer patients who are also obese or have type 2 diabetes experience more aggressive cancer growth and worse outcomes. Prior studies have shown that weight loss treatment and surgery following a breast cancer diagnosis are associated with improved heart health and increased survival.   What are GLP-1 drugs? Glucagon-like peptide-1 receptor agonists (GLP-1 RAs). Approved to treat type 2 diabetes in 2005 and weight management in 2021. Impacts on breast cancer survival and recurrence are still unclear. Since 2020, the use of these drugs has increased dramatically, where approximately 12% of Americans have used GLP-1s for weight loss, according to a RAND report.   The research findings Through a retrospective cohort study examining the electronic health records of more than 840,000 breast cancer patients who were diagnosed between 2006 and 2023, the results suggest there is a potential link between GLP-1 RAs and improved outcomes among breast cancer patients who are also obese or have type 2 diabetes.   GLP-1 RA use was associated with an overall lower risk of death from any cause over a 10-year follow-up period among breast cancer patients. Additionally, breast cancer survivors who used GLP1-RAs for diabetes or obesity had a significantly lower risk of their cancer returning over 10 years following their initial treatment.   "Our findings align with emerging preclinical research and contribute to a growing body of literature related to GLP-1 RA use in oncology settings," said study lead author Kristina L. Tatum, PsyD, MS, of the VCU School of Public Health.   What's next? Further studies are needed to understand the biological mechanisms, if any, between GLP-1 RAs and breast cancer outcomes. The research team intends to further evaluate these correlations through randomized clinical trials.   "Our study underscores the potential of GLP-1 RAs as an adjunct strategy for improving cancer-related outcomes among patients with breast cancer, although clinical trials are needed to inform effective therapeutic approaches and clinical decision making," Fuemmeler said. https://www.oncology-central.com/could-glp-1-receptor-agonists-improve-outcomes-for-breast-cancer-patients-with-obesity-or-with-type-2-diabetes/ XX Researchers at UC Riverside gave cannabis to obese mice and found that not only did the rodents lose weight, but when given a concentrated cannabis oil, the mice also saw striking benefits in their metabolic function. DiPatrizio said his team studied the issue to better understand why cannabis users show significant reductions in weight and risk for diabetes compared with nonusers. "We would think that chronic cannabis users would be eating more and weigh more, but it's just the opposite," DiPatrizio said. Scientists are increasingly examining the possibility that cannabis compounds could fight obesity or metabolic disorders like diabetes. Cannabinoids interact with the body's endocannabinoid system, which partially controls nearly every aspect of our physiology, including metabolism and appetite. The approval expands the previous indication from those aged eight and above and was granted under a priority review process. This decision is supported by one-year data from the PETITE-T1D Phase IV study, which evaluated the safety and pharmacokinetics of Tzield in children under eight years old. Tzield was approved for use in individuals 8+ in stage 2 T1D in 2022. Since then, we have been working to expand the eligible population. Semaglutide was the most commonly-used GLP-1 (65.5% of GLP-1 users) followed by tirzepatide (23.5%). The rest were using the older-generation drugs: liraglutide or dulaglutide. Health Canada said this generic, like existing products, is indicated to be used for the "once-weekly treatment of adult patients with Type 2 diabetes to manage blood sugar levels." With three generics on the market, Tadrous said the price could drop to about $100 or less, depending on their dose. The Company received Investigational Device Exemption (IDE) approval in March 2026 from the U.S. Food and Drug Administration (FDA). The Company plans to submit a 510(k) filing to the FDA in 2027 and launch its FCL AID system for T2D in 2028.   XX insulet Corporation (PODD) has initiated a voluntary recall of certain lots of its Omnipod 5 insulin delivery Pods in the U.S. after detecting that some devices had a manufacturing defect that causes insulin leakage.   Patients using the affected devices could risk experiencing high blood glucose levels due to insufficient insulin delivery, the Acton, Massachusetts-based MedTech disclosed in a statement late Thursday. The company has already notified the FDA about the recall, which it said will affect nearly 1.5% of Omnipod 5 pod units it manufactures annually. In a real-world analysis presented at the International Conference on Advanced Technologies & Treatments for Diabetes, researchers found that CGM metrics for patients who switched from the Guardian 4 sensor (MiniMed) to Instinct by Abbott were able to maintain a time in range of greater than 75%. "When it comes to the automated insulin delivery system ... I think the sensor matters less and the system matters more," Viral N. The launch comes a few months after the company picked up CE mark for the long-term, implantable CGM in January. Eversense 365 is the world's first 365-day CGM system. It also holds clearance as an integrated CGM (iCGM) system, meaning it can work with compatible medical devices. Those include insulin pumps as part of automated insulin delivery systems, like the Sequel Med Tech twiist system. The company said it made Eversense 365 available to the first patients in Sweden. It plans to bring the sensor to Germany, Spain and Italy in the coming weeks https://www.drugdeliverybusiness.com/senseonics-launches-eversense-365-europe/ XX A machine learning model can improve genetic prediction of type 1 diabetes by as much as 10%, show results from a University of California, San Diego study.   The researchers used the machine‑learning model T1GRS to improve on a gold standard polygenic genetic risk score used to predict who is likely to develop the condition called GRS2. The GRS2 polygenic risk score has been widely tested and can be used to predict newborns who are at high risk of developing type 1 diabetes. While early prediction can't necessarily stop the disease it can help to prevent emergencies like diabetic ketoacidosis at diagnosis, allow families time to prepare and could allow use of therapies to delay onset of the condition. Mark Warner (D-Va.) said Monday that he will return to the upper chamber this week after taking time off for the death of his daughter, Madison.   The Virginia senator wrote on the social platform X, "As we remember our incredible daughter, Maddy, my family has been deeply touched by the outpouring of support we've received. Thank you to everyone for your kind words." Madison Warner, 36, died earlier this month after a decades-long battle with juvenile diabetes and other health issues. Mark Warner and his wife, Lisa Collis, wrote in a statement last Monday that they were "heartbroken beyond words" by their daughter's passing.   On Monday, the former Virginia governor said his daughter "was a deeply empathetic and engaged person" and that "as recently as the day she passed, she was full of ideas and suggestions" for him, including how he could improve his social media presence.   "She used to say to me: 'Dad, you have the power — you have to use it.' Okay.. our top story this week: XX Metformin may be linked to the slower progression of age-related macular degeneration (AMD). Among people with diabetes who were older than 55, those taking metformin had a 37% lower chance of developing intermediate AMD over a five-year period compared with individuals who were not using the medication. It's one of the leading causes of vision loss in the US and many other western countries. These researchers now say a clinical trial is the next step. https://scitechdaily.com/scientists-discover-surprising-eye-benefit-of-widely-used-diabetes-drug/ XX new study suggests people with type 1 diabetes may be nearly three times as likely to develop dementia compared with people without diabetes. Similarly, people with type 2 diabetes may have roughly twice the risk of dementia compared with those without diabetes. However, the study found an association rather than proof of causation, meaning diabetes was linked to dementia risk but was not shown to directly cause it.   https://www.usnews.com/news/health-news/articles/2026-03-19/both-types-of-diabetes-increase-dementia-risk XX Researchers in Japan say they've developed an insulin pill… in mice.  The study, published in the journal Molecular Pharmaceutics, tested the delivery of oral insulin by building a carrier peptide called DNP-V. This peptide helps to transport insulin through the small intestine, where protein drug absorption is usually poor. The result was a rapid and significant drop in blood glucose, as well as a sustained (longer-term) decrease. The mice's blood sugar was reduced to near-normal levels. Although the researchers are optimistic about the findings translating to larger therapeutic models, they noted that the results in mice do not guarantee the same outcome in humans, and that more research is needed. https://www.foxnews.com/health/needle-free-diabetes-management-could-horizon-study-suggests   XX   Lilly says it's next-generation obesity drug retatutride cleared its first late-stage trial on Type 2 diabetes patients. The drug lowered hemoglobin A1C by an average of 1.7% to 2% across different doses at 40 weeks compared with placebo, and helped patients lose an average of 16.8% of their weight. Retatrutide also met the study's second goal, helping patients at the highest dose lose an average of 16.8% of their weight, or 36.6 pounds, at 40 weeks, when evaluating only patients who stayed on the drug. When analyzing all participants, including those who discontinued treatment, the highest dose of the drug helped patients lose 15.3% of their weight. The company was also "very pleased" with the relatively low discontinuation rates due to side effects, which were up to 5%, he added. But Lilly has yet to file for approval of the drug for obesity or diabetes. The company expects to report findings from seven additional phase three trials on the drug by the end of the year. Still, retatrutide's A1C reduction doesn't appear to be the greatest Lilly has seen within its portfolio: The highest dose of Zepbound lowered the measure by more than 2% at 40 weeks in two separate trials on diabetes patients. Dubbed the "triple G" drug, retatrutide works by mimicking three hunger-regulating hormones – GLP-1, GIP and glucagon – rather than just one or two like existing treatments. Thi is a new design from the company formerly known as Medtronic. At commercial launch, MiniMed Flex™ will support the company's newest sensor portfolio, including Simplera Sync™ sensor and the Instinct sensor, made by Abbott. MiniMed also announced the MiniMed™ Forward Program, which allows customers who start on the MiniMed™ 780G system to upgrade to the MiniMed Flex™ system for $0. MiniMed Flex™ is cleared for individuals ages 7 and older with type 1 diabetes, and for individuals 18 years and older with insulin-requiring type 2 diabetes. https://www.prnewswire.com/news-releases/minimed-announces-fda-clearance-of-minimed-flex-the-companys-smallest-insulin-pump-featuring-its-first-smartphone-controlled-design-302716864.html   XX Lots of new out of the recent ATTD conference.. some headlines: New study from the UK shows that Ketone Monitoring Could Significantly Reduce DKA Risks in people with type 1 and type 2. This was a study by Abbott which recently submitted a continuous dual glucose-ketone monitor to the FDA for clearance – if approved, it could be available in the U.S. later this year. -- The first modified insulin producing cells are still working 14 months after transplant – without the need for immunosuppressive drugs. This is from Sana which now plans a study of a new therapy.. same gene-editing strategy with lab-grown, stem-cell-derived insulin-producing cells. -- Protein looks like it helps avoid lows during exercise. Both high and low doses of whey protein before exercise were effective, significantly reducing the risk of hypoglycemia by five to 10 times.   Researchers noted that the body's response to protein was rapid (within 20 minutes), which suggests taking it close to the beginning of exercise could be beneficial for preventing hypoglycemia. Though more research is needed, there was also evidence showing protein intake could be beneficial for prolonged fasting and preventing overnight lows. -- More info about type 1 and GLP medications. Researchers at ATTD presented the results of a small, seven-month study assessing the effectiveness of semaglutide for people with type 1 diabetes and obesity. During the trial, 36% of participants taking semaglutide spent more than 70% of their time in range, less than 4% of their time below range, and lost more than 5% of their body weight compared to those not taking semaglutide.   Treatment with semaglutide was also associated with reductions in cholesterol and blood pressure. Based on all of these changes, the researchers calculated that the participants who received semaglutide had significantly reduced their risk of heart disease over the next 10 years.   Other studies show that since 2020, prescriptions of GLP-1 medications have grown exponentially for adults with type 1 diabetes between the ages of 18 and 85. https://diatribe.org/diabetes-research/top-diabetes-news-attd-2026   XX Lots of talk about fully closed loops.. CamDiab unveiled theirs.. called Liberty.. which the company says it's the world's first fully closed loop commercial launch. CamDiab offers the FDA-approved mylife CamAPS FX app for automating insulin delivery in MyLife's (formerly Ypsomed Diabetes Care's) insulin delivery pumps. The mylife CamAPS FX on iOS has full compatibility with leading continuous glucose monitors (CGMs). Those include the FreeStyle Libre 3 and Libre 3 Plus from Abbott and the Dexcom G6 so customers can use their preferred device. https://www.drugdeliverybusiness.com/camdiab-unveils-fully-closed-loop-insulin-feature/ XX Insulet reported data on a fully closed-loop automated insulin delivery system in people with Type 2 diabetes. The 24 people in the trial spent 24% more time in the target blood glucose range using the system than when receiving standard injection therapy. Insulet plans to start a pivotal study this year and aims to launch in 2028. Rival insulin pump manufacturer Tandem is on a similar course. Tandem CEO John Sheridan told investors on an earnings call last month that his team plans to start a pivotal trial this year to support a filing with the Food and Drug Administration in 2027. Medtronic disclosed the start of a pivotal trial of its Vivera fully closed-loop algorithm last month, shortly before spinning off the program as part of the MiniMed initial public offering. Okay.. our top story this week: XX A biotech company developing stem-cell treatments for type 1 diabetes has announced a new research partnership aimed at improving the survival of transplanted insulin-producing cells. NewcelX, a clinical-stage company based in Switzerland, said it will work with Eledon Pharmaceuticals to study a combination approach. The goal is to help transplanted cells survive longer in the body by reducing the immune response that often leads to transplant rejection. If successful, the strategy could support longer-lasting islet cell replacement and move the therapy closer to becoming a functional treatment for people with type 1 diabetes. However, the companies have not yet released any safety or effectiveness data on the combination treatment, and financial details of the partnership were not disclosed. The research agreement is focused on exploring whether combining stem-cell-derived islets with targeted immune therapy can lead to longer-lasting cell transplants and improved outcomes for people with type 1 diabetes. https://www.stocktitan.net/news/ELDN/newcel-x-announces-strategic-collaboration-with-eledon-d10l1vqdofls.html XX Debate this week in the UK on whether testing for type 1 diabetes should become mandatory when children present with symptoms. The Westminster Hall debate, scheduled for 9 March, will consider calls for routine testing of babies, toddlers and young children who show signs associated with the condition. It follows a petition backing the move, dubbed 'Lyla's Law', which passed 121,000 signatures in December 2025. The campaign was launched by John Story after his two-year-old daughter, Lyla, died from diabetic ketoacidosis (DKA) on 3 May 2025, 16 hours after being diagnosed with tonsillitis. https://www.nursinginpractice.com/clinical/diabetes-and-endocrinology/diabetes-community-urged-to-call-on-mps-to-attend-lylas-law-debate/ XX     A new study suggests that people with gestational diabetes who adjust their own insulin doses may reach healthy blood sugar levels faster than those whose doses are adjusted by clinicians. Half of the participants were assigned to adjust their own insulin doses using a simple rule: increase the dose by two units if fasting blood glucose was above 95 mg/dL, decrease it by two units if it dropped below 70 mg/dL, and keep the same dose if levels fell in between. The other half had their insulin adjusted by clinicians through weekly reviews. By the end of pregnancy, both groups had similar average fasting glucose levels before delivery: about 89 mg/dL in the patient-led group and 90 mg/dL in the clinician-led group. However, those adjusting their own insulin reached their blood sugar targets more quickly, averaging 1.8 weeks compared with 2.5 weeks for those managed by clinicians. The study also found lower risks of certain complications among the patient-led group. https://www.medscape.com/viewarticle/self-insulin-dosing-leads-control-gestational-diabetes-2026a1000729 XX   A blockbuster anti-obesity and diabetes drug could cost as little as $3 per month to manufacture once it goes off patent later this month, researchers said Friday, providing a major opportunity to boost health in low and middle-income countries.   Semaglutide, the active molecule in Novo Nordisk's Ozempic and Wegovy will lose patent protection in countries such as Brazil, China, and India later this month, and researchers identified 150 countries where it was never patented. These researchers estimated it will cost as little as $3 to produce a month's supply of semaglutide, which in its branded form sells for around $200 a month in the United States. Another of the study's authors, Professor Francois Venter at the University of Witwatersrand in South Africa, said drugs to treat HIV, TB, malaria, and hepatitis are now available at prices close to production costs but still sufficient for generic manufacturers to operate. https://www.sciencealert.com/weight-loss-drugs-could-cost-just-3-a-month-to-make-as-patents-end XX Here in the US the FDA is stepping up its efforts to combat widespread GLP-1 drug compounding. In its latest offensive, the agency has unleashed a fresh set of 30 warning letters targeting telehealth companies it says make "false or misleading" claims about compounded versions of popular obesity drugs. The FDA says Compounded drugs can be important for overcoming shortages or meeting unique patient needs—but compounders should not try to compound drugs in a way that circumvents FDA's approval process." https://www.fiercepharma.com/pharma/fda-ramps-crackdown-glp-1-drug-compounders-fresh-batch-30-warning-letters XX   Check your infusion sets for an issue: Unomedical, a subsidiary of Convatec and a supplier of insulin infusion sets to diabetes tech firms, has received a warning letter from the FDA. Inspectors raised concerns with leaking infusion sets, following a regulatory assessment of Unomedical's facility in Reynosa, Mexico, last summer. Unomedical supplies infusion sets to insulin pump makers including Medtronic, Tandem Diabetes Care and Beta Bionics. In a Feb. 3 statement, Convatec said the letter focuses on reporting procedures and quality protocols and does not place restrictions on producing, marketing or distributing any of Unomedical's products.  Unomedical told the FDA in its responses that it plans to conduct a retrospective review of complaints involving serious injury or death by January and conduct additional training on complaint handling by May.   https://www.medtechdive.com/news/fda-warns-insulin-infusion-set-maker-unomedical-over-leaks-mishandled-comp/813503/ XX Nearly four in ten people with type 2 diabetes do not take their medications as prescribed, according to a new research review published in Diabetologia in November 2025.   Researchers examined existing studies on medication adherence, including how often patients miss doses, why it happens, and what strategies may help. They estimated that about 38% of patients with type 2 diabetes are not fully adherent to their medications.   Adherence rates vary depending on the type of medication. About 63% to 68% of patients take oral glucose-lowering drugs as directed, while adherence drops to 43% to 54% for injectable GLP-1 medications and 41% to 64% for insulin.   Poor adherence can lead to serious consequences. One retrospective study cited in the review found that patients who consistently took their glucose-lowering medications had a 31% lower risk of hospitalization or emergency department visits.   The review also highlighted ways to improve adherence. Simplifying medication routines can help, such as using fixed-dose combination pills, which combine multiple drugs into a single tablet. Studies show these combinations are linked to better adherence and improved blood sugar control.   Pharmacists can also play an important role by providing education, reviewing medications, setting up reminders, and helping patients organize their treatment plans. The researchers noted that support should be tailored to each patient. Older adults may benefit from simpler systems and caregiver support, while younger patients may respond better to digital tools like app-based reminders.   The authors also found that measuring adherence is challenging and recommend using multiple methods, such as pharmacy records, patient interviews, and objective tests when possible.   Overall, the review concludes that personalized, multi-step approaches lasting at least three months are most effective in helping people with type 2 diabetes stay on track with their medications. https://www.pharmacytimes.com/view/type-2-diabetes-medication-adherence-rates-remain-low-and-pharmacists-can-help XX New clinical trial shows metformin does not directly reverse insulin resistance in people with type 1 diabetes. The results may help physicians refine treatment strategies and reduce the daily demands placed on people who rely solely on insulin therapy. "Insulin resistance is a growing problem in type 1 diabetes. Medtronic  acquired MiniMed 25 years ago announed last May that it would spin its diabetes business off. The former CEO is back in his position as executive chair of the Board, he'd stepped away for a medical leave. Sayer's return to the board comes just days after Dexcom announced a new board member. The release says: Built on Sequel's proprietary iiSure™ Technology, the system enables earlier detection of delivery issues, alerting users to blockages up to nine times faster than other AID systems1, potentially reducing the risk of unexplained high glucose and giving you time to take action before experiencing severe high blood sugar or DKA2. Designed to expand access to automated insulin delivery, twiist is available through pharmacy channels with a flexible access model, XX Tandem Diabetes Care's Mobi automated insulin delivery system is now available with Android devices. In November, Tandem announced that it received FDA approval for the Android version of its Mobi mobile app. The pump, which pairs with Tandem's Control-IQ+ algorithm, previously worked with iOS software. At the time of the clearance, it said it would commence a limited rollout before the full launch — now underway — this year.   Tandem launched Mobi in the U.S. in February 2024. It initially received FDA clearance for people with diabetes ages six and up in July 2023. The system then received expanded clearance for pediatric indications in April 2024, then later won CE mark in May 2025.   Mobi features a 200-unit insulin cartridge and an on-pump button to provide an alternative to phone control for insulin boluses. It comes in at less than half the size of the flagship Tandem pump system, the t:slim X2 pump. All the info is over at diabetes-connections.com events/     Okay.. our top story this week: XX An "immune system reset" eliminated Type 1, diabetes in mice in a study conducted at Stanford Medicine without immune suppressant medications. This was a combined transplant of blood stem cells and insulin-producing pancreatic islet cells from a donor whose immune profile did not match the recipient. The dual transplant approach both restored insulin production and retrained the immune system. For the full six months of the experiment, the animals did not need insulin injections or immune suppressive medications. Challenges remain using this approach to treat Type 1 diabetes. Pancreatic islets can be obtained only after death of the donor, and the blood stem cells must come from the same person as the islets. It is also unclear whether the number of islet cells typically isolated from one donor would be enough to reverse established Type 1 diabetes. But the researchers are working on solutions, which could include generating large numbers of islet cells in the laboratory from pluripotent human stem cells, or finding ways to increase the function and survival of transplanted donor islet cells. https://scitechdaily.com/stanford-scientists-cure-type-1-diabetes-in-mice-without-insulin-or-immune-suppression/ XX An electronic implant interlaced with islet cells is being looked at to treat type 1. Researchers at the University of Pennsylvania School of Medicine worked with engineers at Harvard University to combine stem-cell biology with soft electronics. They inserted an ultrathin, flexible mesh of conductive wires — thinner than a human hair — into developing pancreatic tissue. As the cells assembled into clusters, the mesh became woven through them. The electronics can record the faint electrical signals produced by the cells that control insulin release. They can also deliver small pulses of electricity back to the cells.   After several days, the cells began to behave more like mature islets. Last fall we told you about promising results from Eledon's drug to prevent islet transplantation rejection in type 1 diabetes. The first six patients no longer had to inject or infuse insulin.. the trials continue and this month one of the patients – Katie Beth Hand – began posting about her experiences one month in, on social media, she says she's off basal insulin already and in range 99 percent of the time. She is also encouraging people to learn more about support the islet act https://lnkd.in/e8pQ7_Y7 XX This is a bill introduced last November which would change the wording on pancreatic cell transplants. The problem is that islets are classified as drugs rather than organs, making transplantations difficult for medical teams and centers to preform due to accessibility. Insurance companies are also less likely to provide reimbursements for treatment, which can cost hundreds of thousands of dollars. The official Journal of The Transplantation Society estimates the cost at about $140,000. The bill went to the senate committee of Health, Education, Labor, and Pensions in early November. No other action has been taken since then. https://www.wtoc.com/2026/02/19/bluffton-family-advocates-islet-act-help-diabetic-son/ XX Big change for the obesity drug Zepbound – now available in the multi dose KwikPen. This is a month's worth of doses in a single pen.. and it's multi dose – you can adjust it. Cash-paying patients can get the multi-dose device, called KwikPen, on the company's direct-to-consumer website, LillyDirect. Prices start at $299 per month for the lowest dose level. Until now, you could only get zepbound in a single dose auto injector or a sing dose vial. In a release, Lilly said the Food and Drug Administration approved a label expansion for Zepbound to include the multi-dose device. The KwikPen is already used for other drugs, such as Lilly's popular diabetes medication, Mounjaro – which is the same medication as zepbound, they're both tirzepitide. https://www.cnbc.com/2026/02/23/eli-lilly-launches-zepbound-obesity-drug-pen-one-month-doses.html   XX For years, researchers have observed that people who live at high elevations,  tend to develop diabetes less often than those at sea level. Although the trend was well documented, the biological explanation behind it was unclear. Scientists now say they have identified the reason. Their research shows that in low oxygen environments, red blood cells begin absorbing large amounts of glucose from the bloodstream. Their work showed that when oxygen is limited, red blood cells use glucose to generate a molecule that helps release oxygen to tissues. This process becomes especially important when oxygen is in short supply. The researchers also found that the metabolic benefits of prolonged hypoxia lasted for weeks to months after mice were returned to normal oxygen levels. They then evaluated HypoxyStat, a drug recently developed in Jain's lab that mimics low oxygen exposure. HypoxyStat is taken as a pill and works by causing hemoglobin in red blood cells to bind oxygen more tightly, limiting the amount delivered to tissues. In mouse models of diabetes, the medication completely reversed high blood sugar and outperformed existing treatments. https://www.sciencedaily.com/releases/2026/02/260221060952.htm XX Watching this one closely – Portal Diabetes gets FDA breakthrough device designation for its implantable insulin pump system. This is a system that includes not just a device that's implanted into the abdomen, but also a new, temperature stable insulin. It will work with – quote – "modern" CGM technology with a fully closed loop - and aims to deliver a functional cure for type 1. While reports say Portal's system is the first in the US – there was an implantable pump developed and used by about 500 people worldwide, including about 100 in the US – by MiniMed. Medtronic bought the company and in 2007 they stopped that program. Portal Diabetes expects to begin clinical trials on its combination system around the fourth quarter of 2027. https://www.drugdeliverybusiness.com/portal-diabetes-fda-breakthrough-implantable-insulin-pump/ XX Sequel Med Tech and Senseonics (NYSE:SENS) today announced the full U.S. launch of their CGM and insulin pump integration. That's the eversense cgm and twist pump. Sequel said its full launch with Eversense 365 makes twiist available with two compatible CGMs. twiist also pairs with the Abbott FreeStyle Libre 3 Plus sensor. Eversense 365, an implantable system, rests under the skin for the duration of a year. Users can change its external, silicone-based adhesive daily with almost no skin reactions. https://www.drugdeliverybusiness.com/sequel-senseonics-full-launch-twiist-eversense/ XX Right back with a Dexcom update, and a look at which type of diet reduces insulin use overall.. right after this: -- Back to the news.. Dexcom is watching for expanded Medicare coverage of its continuous glucose monitors to people with Type 2 diabetes who don't take insulin. CEO Jake Leach told investors on Thursday that the company has been "sitting here waiting for a coverage decision" from the Centers for Medicare and Medicaid Services Dexcom started to see commercial coverage unlock for Type 2, non-insulin users toward the end of last year, Leach said. He expects broader Medicare coverage for that group would allow nearly 12 million people to access CGMs.     In the meantime, the American Diabetes Association updated its guidelines last year to recommend clinicians consider using CGMs for Type 2 diabetes when patients are taking glucose-lowering medications other than insulin. Leach said that real world data the company has been generating supports that decision, and that Dexcom has launched a registry for non-insulin users. https://www.medtechdive.com/news/dexcom-seeks-expanded-medicare-coverage-of-cgms-for-type-2-diabetes/812223/ XX Medtronic's separation of MiniMed is not yet complete.. but continues to move forward. The company has submitted their next pump – MiniMed Flex – to the FDA. This is a pump smaller than the 780G but uses the same reservoirs and infusion sets. It will also work with both the Simplera Sync and Instinct sensors. Medtronic also began a U.S. pivotal study for Vivera, its third-generation algorithm for automated insulin delivery. It also remains set to submit its MiniMed Fit patch pump system to the FDA by the coming fall. https://www.drugdeliverybusiness.com/medtronic-submits-minimed-flex-fda-q3/ XX A study modelling how genes may influence a child's body mass index over time has found that BMI at age 10 and overall growth rate between ages one and 18 might be important factors, as the two are more likely linked to diabetes, high cholesterol, and heart disease in later life. Nearly 66,000 BMI measurements from around 6,300 children and adolescents aged one to 18 were analysed to understand the role of genes.     "Future research is needed to help identify the most effective ages to prevent obesity or poor growth for long-term benefit." https://www.ndtv.com/health/bmi-at-age-10-growth-rate-up-to-age-18-are-important-factors-for-diabetes-heart-disease-study-11125146 XX A low-fat vegan diet—without cutting calories or carbs—may help people with type 1 diabetes significantly reduce how much insulin they need. In a new analysis published in BMC Nutrition, participants following the plant-based plan lowered their daily insulin use by 28%, while those on a portion-controlled diet saw no meaningful change. Researchers say the reduced insulin requirement likely reflects improved insulin sensitivity. The original 2024 study reported additional benefits from the vegan diet. Participants lost an average of 11 pounds and showed improvements in insulin sensitivity and glycemic control. Cholesterol levels and kidney function also improved among those following the plant-based plan. https://www.sciencedaily.com/releases/2026/02/260212234212.htm XX Interesting little tidbit from the Winter Olympic Games.. the World Anti-Doping Agency (WADA) was monitoring GLP drug use. An advisory group that makes recommendations about WADA's list of prohibited substances discussed the status of GLP-1 medications, and added semaglutide (Ozempic, Wegovy, Rybelsus) and tirzepatide (Mounjaro, Zepbound) to its monitoring program That means patterns of use of these drugs will be tracked both in and out of competition.  The finding will be used to make recommendations about whether GLP-1 agonists should be added to the prohibited list, the spokesperson explained. Hannah Schmidt competes in ski cross for Canada – she was diagnosed with Type 1 diabetes at age 12 years old.  Anna FarnSchadt Fernstäd a Czech skeleton racer diagnosed in 2022 after she'd already been to several Olympics. Instead, it allows people to look up their drugs and then navigate to buy them elsewhere, either from a major drug company or a pharmacy. The 43 drugs listed on the site have prices ranging from $3 to over $5,500. TrumpRx does include warnings that the site may not be the best option to save money on prescriptions. Each product page advises: "If you have insurance, check your co-pay first — it may be even lower." For now, the website says its prices are for people paying with their own money, rather than going through insurance. The only insulin listed right now is Lilly's insulin lispro – and it's the same price as you'd find through Illy's insulin value program. I looked up diabetes meds.. For example, if you have an insurance co-pay of $25 a month for Farxiga, a drug often used for diabetes, you would be paying $182 on TrumpRx. As you can imagine, though ,this is complicated and as with most of our healthcare system, it may be good in some cases and not much help in other.  I'd suggest calling your local pharmacist or checking with your human resource dept. https://www.nytimes.com/2026/02/06/health/trumprx-prescription-drug-prices-consumers.html XX Novo Nordisk will launch some doses of its oral semaglutide for diabetes under the brand name Ozempic pill in the second quarter of this year. The company said the U.S. Food and Drug Administration has approved Ozempic tablets in three different doses. Novo says The new Ozempic name is intended to help patients and health care professionals more easily recognize the available treatment options for type 2 diabetes Semaglutide tablets have been available under the brand name Rybelsus Ruh BELL sis for diabetes since 2019 but with different dosing. The pill is also approved to reduce the risk of certain cardiovascular conditions in adults with type 2 diabetes who are at high risk for these events. The FDA had approved the new doses based on a bioequivalence study and the clinical trial data for Rybelsus, Novo said. https://www.reuters.com/business/healthcare-pharmaceuticals/novo-launch-ozempic-pill-diabetes-second-quarter-this-year-2026-02-04/ XX https://www.contemporarypediatrics.com/view/early-screening-for-type-1-diabetes-found-effective-in-children XX Possible new way to identify and track the progress of type 1 diabetes before clinical onset. A recent study published in Science Advances described the application of subcutaneous microporous scaffolds. These are inserted and have been shown to  identify changes in cancer, multiple sclerosis, and T1D by capturing changes of immune cells over the course of a disease. This is a proof of concept study in mice.. so very early days. https://www.news-medical.net/news/20260204/Implantable-immune-scaffold-predicts-type-1-diabetes-weeks-before-symptoms.aspx XX A large global genetics study shows that many key drivers of Type 2 diabetes operate outside the bloodstream. In a major international project led in part by the University of Massachusetts Amherst and Helmholtz Munich in Germany, researchers linked hundreds of genes and proteins to the disease. The work, published in Nature Metabolism, points to a key challenge in diabetes research: the biology behind rising blood sugar does not play out the same way in every part of the body. It also shows why including people from many backgrounds matters, since genetic clues that stand out in one population may be faint or invisible in another. Huge study, 2.5 million people worldwide comparing patterns across seven tissues tied to diabetes and four global ancestry groups, then asked a simple question: what do you miss if you only measure blood? Across the seven tissues, the researchers found causal evidence pointing to 676 genes. Yet overlap with blood was limited: only 18% of genes with a causal effect in a primary diabetes tissue, such as the pancreas, showed a matching signal in blood. At the same time, 85% of genetic effects observed in diabetes-relevant tissues were completely absent from blood-based analyses. The findings lay out a roadmap for future research aimed at understanding the biological pathways underlying Type 2 diabetes and developing more effective treatments. https://scitechdaily.com/massive-global-study-rewrites-the-biology-of-type-2-diabetes/ XX Express Scripts settled the U.S. Federal Trade Commission's claims its insulin pricing practices violated antitrust and consumer protection laws, and agreed to changes aimed at lowering costs for patients, insurers and small pharmacies The settlement, first reported by Reuters, fits with that goal, and allows the FTC to pare down a case brought by the former Biden administration against Cigna's Express Scripts, UnitedHealth Group Inc's (UNH.N), Optum unit and CVS Health Corp's (CVS.N), CVS Caremark. The case against Optum and Caremark is ongoing. Pharmacy benefit managers, which set how drugs are covered by health insurance, have faced a decade of scrutiny from regulators and lawmakers over pricing practices. While the industry has already made reforms, the settlement gives the FTC power to enforce broader changes at Express Scripts. The 10-year agreement restricts Express Scripts' ability to engage in practices critics say contribute to high costs, like pocketing rebate payments from drugmakers based on the list price of drugs. Congress has passed bipartisan legislation to extend and strengthen the Special Diabetes Program (SDP), a cornerstone of Federal investment in type 1 diabetes (T1D) research. The President signed the legislation and it is now law. Extends the SDP through December 31, 2026, and increases funding from $160 million to $200 million annually. Strengthens overall funding for the National Institutes of Health (NIH) by $415 million. Increases diabetes research funding at the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) by $10 million. Created by Congress and administered by the NIH, the SDP has contributed nearly $3.6 billion to T1D research and has played a role in nearly every major breakthrough in the field. Expanded Smart Food Logging including a comprehensive nutrition database of more than 1M meals that provides a breakdown of calories, carbohydrates, protein, fat, dietary fibers, and more. More ways to meal track including text search, barcode scanning or taking a photo of the meal, creating a seamless and intuitive meal tracking solution. A redesigned Daily Insights feature which will introduce a new interface with more personalized recommendations. The newest features will launch nationwide in the coming weeks.  XX Beta Bionics has received a warning letter from the Food and Drug Administration following an inspection last year, the company disclosed on Friday. The diabetes technology company said in a securities filing that the warning letter concerns non-conformities with the company's quality management system, medical device reporting, and correction and removals. The warning letter has not yet been posted by the FDA.   The company said in the filing that it has already taken actions to improve the processes described in the warning letter, and it is working on a written response to the FDA.   The firm does not expect the warning letter to affect the planned launch of a new insulin patch pump by the end of 2027. Beta Bionics unveiled a prototype of the device, called Mint, last year at the American Diabetes Association's Scientific Sessions. Kamen has not been accused of any wrongdoing. We told you about this recall late last year, these numbers are an FDA update.     Abbott said the sensors can provide incorrect glucose readings over extended periods, which could lead to users making dangerous treatment decisions, including eating excessive carbohydrates along with skipping or delaying insulin doses, potentially leading to serious health risks. Senseonics plans to launch Eversense 365 in Germany, Italy, Spain and Sweden in the coming months. All the info is over at diabetes-connetionss.com events/ Okay.. our top story this week: XX   All UK children could be offered screening for type 1 diabetes using a simple finger-prick blood test, say researchers who have been running a large study. This is the ELSA study - Early Surveillance for Autoimmune diabetes, a first of its kind UK study. They tested blood samples from 17,931 children aged 3-13 for autoantibodies, markers of type 1 diabetes that can appear years before symptoms. Families of children found to have early-stage type 1 diabetes received tailored education and ongoing support to prepare for the eventual onset of type 1 diabetes symptoms and to ensure insulin therapy can begin promptly when needed, reducing the chances of needing emergency treatment. Those with one autoantibody also received ongoing support and monitoring.   Some families were also offered teplizumab, the first ever immunotherapy for type 1 diabetes, which can delay the need for insulin by around three years in people with early-stage type 1 diabetes. The second phase has launched and will expand screening to all children in the UK aged 2-17 years, with a focus on younger children (2-3 years) and older teenagers (14-17 years). The research team aims to recruit 30,000 additional children across these new age groups. ELSA 2 will assess how screening can be scaled across the NHS and evaluate its cost-effectiveness. https://www.birmingham.ac.uk/news/2026/childhood-type-1-diabetes-screening-is-effective-and-could-prevent-thousands-of-emergency-diagnoses XX At the J.P. Morgan Healthcare Conference Dexcom CEO Jake Leach says they're going to launch a new product outside the US. I'll link up that interview, The full quote: "When you look at the outside the U.S., there are a lot of structures that are tiered. Patients have access to different types of products, so we've got a new one that we want to introduce that will add flexibility there. It's based on the G7 platform, just like Dexcom ONE+, but it has a unique experience that's tailored for a subset of users that, today, don't have access to Dexcom." Your guess is as good as mine, but sounds more like a pricing or ordering issue than a new bit of hardware or software. Dexcom will also bring Stelo to some international markets this year. And plans a new mobile app experience for the wearable biosensor meant for people who don't dose insulin. Leach also says G8 will be much smaller and with more capability. but is a few years away. https://www.drugdeliverybusiness.com/dexcom-ceo-jake-leach-2026-roadmap-jpm/ XX A new international consensus statement provides guidance for the use of diabetes technology during pregnancy for women with type 1 diabetes (T1D), type 2 diabetes (T2D), or gestational diabetes (GD).   Organized by the diaTribe Foundation, the document was based on evidence where available, as well as opinion from an international group of experts in endocrinology, diabetes technology, and obstetrics & gynecology, among others.   This is the first set of recommendations specifically addressing the use of diabetes technology in pregnancy – and we'll link it up. https://www.medscape.com/viewarticle/new-consensus-statement-addresses-diabetes-tech-pregnancy-2026a100020d XX Bariatric surgery beats GLP-1s for type 2 diabetes across income levels. This study was published this month, looking at nearly 300 patients are 4 medical centers. Success here is measured by lower blood glucose levels, higher weight loss (28% vs. 10%), less use of diabetes medications, remission of diabetes to the point of no longer needing to inject insulin, and reduced risk factors for cardiovascular disease.     Bariatric surgery was better than medical therapy across all social backgrounds, they found, and not just in areas of higher deprivation. The ancillary study was smaller, and some of the participants randomized in earlier stages crossed over from medical to surgical treatment, and the reverse. The authors acknowledged and accounted for these limitations, along with the rapid development of more powerful obesity drugs not fully captured in the study. This was a long term study – more than 12 years – and by the end of the study more people were choosing GLP1 medications. One dividing line: If someone hopes to lose 100 pounds, that's more likely with surgery than with medications.  "Ultimately, we need large, long-term, well-designed studies to clarify the best strategy for a given patient." https://www.statnews.com/2026/01/19/diabetes-study-bariatric-surgery-better-than-glp-1s/ XX Researchers at the University of California, Riverside have reported for the first time that a father's exposure to microplastics (MPs) can lead to metabolic problems in his children, including diabetes. This is a mouse study, but it looks at a previously unrecognized way in which environmental pollution may influence the health of future generations. MPs are extremely small plastic fragments, measuring less than 5 millimeters, that form as consumer products and industrial materials break down. Metabolic disorders describe a group of conditions that include elevated blood pressure, high blood sugar, and excess body fat, all of which raise the risk of heart disease and diabetes. The team found that female offspring of male mice exposed to MPs were far more prone to metabolic disorders than offspring of unexposed fathers, even though all offspring received the same high fat diet.     The research team hopes the findings will guide future investigation into how MPs and even smaller nanoplastics affect human development.   https://scitechdaily.com/microplastics-can-rewire-sperm-triggering-diabetes-in-the-next-generation/ XX The FDA has finalized four new recalls for certain lots of Abbott's FreeStyle Libre 3 and FreeStyle Libre 3 Plus sensors due to ongoing safety concerns. We told you about this in November when Abbott says some of its continuous glucose monitoring (CGM) sensors were providing incorrect low glucose warnings. Internal testing identified the issue—carbon building up in the sensors during the manufacturing process—and determined that approximately 3 million CGM sensors were affected. The sensors were distributed in the United States, Canada and several European countries.  When Abbott shared that announcement, the FDA was still reviewing the situation. No recalls had yet been finalized. The Pod recycling program, offered at no cost to customers, enables users to request a recycling kit online. This allows them to return their used Omnipods. Insulet then decontaminates the returned Pods before transporting them to a company specializing in recycling for electronics and medical products. Insulet began recycling pilot programs in Mass and California and are rolling it out nationwide. Insulet also has "Pod takeback" programs outside the U.S. in several international markets. These programs enable customers to request a takeback kit by contacting their local customer support team.   https://www.drugdeliverybusiness.com/insulet-expands-us-pod-recycling-program/ XX Up next a new resource for a population at three times the risk for diabetes, but without a lot of access to health information. I The first diabetes information website primarily in ASL has launched. The site includes GIFs and videos on diabetes management and an ASL glossary of diabetes-related terms. This is from University of Utah Health – Called Deaf Diabetes Can Together. Deaf and hard of hearing people are at three times higher risk for diabetes, but access to health information in ASL is limited. https://healthcare.utah.edu/newsroom/news/2026/01/first-diabetes-information-website-asl-launches XX Novo Nordisk ended all work on cell therapies, including a Type 1 diabetes program, in October – and now has found a buyer. Aspect has acquired rights to the assets and giving Novo an option to reengage for later-stage development and commercialization. Novo is helping bankroll Aspect's development of the assets, investing in the company and providing research funding. The arrangement gives Novo a chance to profit from the programs down the line. Novo is eligible for royalties and milestone payments on future product sales and, having handed the reins to Aspect for now, can expand its role in later-stage development and commercialization. This is an In the News episode.. we're doing things a little differently this year, but we are still bringing you the top diabetes stories and headlines happening now. Okay.. our top story this week: XX Positive results for a phase 1 clinical trial of encapsulated islets in people with type 1 from Encellin. This first-in-human trial is assessing (1) safety and adverse events, (2) cell survival within the device, and (3) fibrosis, or scarring, around the implants. As planned, ENCRT devices were removed from the initial five participants after 4 months of implantation. Analysis of the removed devices show: minimal to no fibrosis (formation of scar tissue around the device); robust formation of blood vessels around the device; viable islets inside the device. These results indicate that the device has the potential to host islet cells while maintaining sufficient oxygen and nutrient flow for the cells to work effectively. Breakthrough T1D's Role: Encellin's technology was originally developed within the Lab of Dr. Tejal Desai (University of California San Francisco) with funding from Breakthrough T1D, which was crucial in the formation of Encellin as a company and attracting subsequent venture capital. https://breakthrought1d.ca/cell-therapy-trial-encellins-device-delivers-promising-early-results/ XX The US Food and Drug Administration has accepted Tzield for priority review to expand the current age indication from eight years and above, to as young as one year old and above to delay the onset of stage 3 type 1 diabetes (T1D) in patients diagnosed with stage 2 T1D. The target action date for the FDA decision is April 29, 2026.   "This priority review emphasizes the urgent need for innovative therapies like Tzield which has the potential to prevent the natural progression of T1D by delaying the loss of endogenous insulin production. This might be particularly significant in this young population, as it is well documented that the autoimmune attack that drives this disease in many cases, begins, early in life," said Christopher Corsico, Global Head of Development at Sanofi. "If approved, Tzield could represent an important advance for delaying the onset of stage 3 type 1 diabetes in early childhood, which would benefit patients and caregivers alike."   Interim data for the PETITE-T1D phase 4 study was presented at the 51st Annual Conference of the International Society for Pediatric and Adolescent Diabetes and simultaneously published in Diabetologia.   Priority review is given to regulatory applications seeking approval for therapies that have the potential to provide significant improvements in the treatment, diagnosis, or prevention of serious conditions.   The safety and efficacy of Tzield in the PETITE-T1D population has not been approved by any regulatory authority. https://www.globenewswire.com/news-release/2026/01/05/3212420/0/en/Press-Release-Sanofi-s-Tzield-accepted-for-priority-review-in-the-US-for-young-children-with-stage-2-type-1-diabetes.html XX Sanofi said the U.S. Food and Drug Administration accepted a priority review to potentially expand the current age range for its Tzield type-1 diabetes drug to include children as young as one year old.   The French pharmaceutical company said Monday that the FDA's review could amend the lower end of the range for the drug, which is currently approved for children as young as eight years old. Tzield would be the first disease-modifying therapy that works to delay stage three type-1 diabetes for children aged one and older—who currently have stage two of the disease, according to Sanofi.   The review comes after positive data from a Phase 4 study, and the anticipated action date for the FDA decision is April 29, Sanofi said. https://finance.yahoo.com/news/sanofi-says-fda-agrees-review-064000083.html   Separately, The European Commission approved Sanofi's Teizeild (teplizumab) to delay the onset of stage 3 type 1 diabetes (T1D) in adult and paediatric patients eight years of age and older with stage 2 disease, the company announced Monday. A positive opinion had been issued by the EMA's Committee for Medicinal Products for Human Use in November. https://firstwordpharma.com/story/7057996   XX The FDA clears the MiniMed Go app for multiple daily injections (MDI). MiniMed Go, a smart MDI system, integrates the InPen smart insulin pen with the Instinct sensor made by Abbott. Medtronic and Abbott entered a collaboration in August 2024 to pair insulin delivery and continuous glucose monitoring (CGM) technologies. Medtronic (soon to be a standalone company called MiniMed) launched the MiniMed 780G automated insulin delivery system with the Instinct sensor made by Abbott late last year. Now, the technology can pair with Medtronic's other insulin delivery offering, the InPen smart insulin pen. MiniMed Go received clearance for individuals with insulin-requiring type 1 and type 2 diabetes aged seven years and older. It also received the nod for children aged 2-6 under the supervision of an adult caregiver. Compatibility for Medtronic's own Simplera sensor with MiniMed Go remains under FDA review. Medtronic expects to launch MiniMed Go in the U.S. in the coming spring. https://www.drugdeliverybusiness.com/medtronic-fda-clearance-minimed-go-inpen-instinct/ XX A new UNC-led study, published in the Journal of Diabetes and Its Complications, is the first to highlight diabetes distress that people over the age of 65 can encounter when managing T1D, including the unique stressors in this age group. While the researchers found that markedly elevated levels of diabetes distress were less prevalent among older adults than in younger age groups, understanding the sources of that distress – and the people most likely to experience it – will help health care and public health experts improve treatment strategies for this unique population. Around 36% of older adults surveyed in the study reported elevated levels of diabetes distress warranting further assessment and treatment. Older adults who identified as women, had higher hemoglobin A1C levels and had been to the emergency room in the past year were most likely to experience elevated levels of diabetes distress. The most common stressors included financial worries, T1D management difficulties and worries about complications.  An unexpected finding was that people who had been diagnosed with T1D at earlier ages or had lived with the condition the longest had lower levels of diabetes distress.   "Our group hopes to continue exploring the specific factors and experiences that contribute to diabetes distress in this population, and this study has highlighted areas for potential next steps. The guidance builds on the agency's existing policy classifying low-risk wellness tools, such as fitness apps and activity trackers encouraging exercise, as non-medical devices exempt from tight regulation, provided they don't associate claims related to disease diagnosis or treatment. When asked about the accuracy of these general wellness devices Makary told Fox News, "If they're not making claims that they are medical grade, let's let the market decide. Let's let doctors choose from a competitive marketplace which ones they recommend for their patients." I'm watching this closely, curious to see which will be the first diabetes related product through the gate. This is the first pump pairing with the implantable one-year CGM. The system features what they're calling iiSure technology, which includes four checkpoints to provide accurate delivery and alert users to blockages quickly. Cleared for ages six and up with type 1 diabetes, it offers a personalized way to manage the condition. You've heard of GIP – it's in drugs like terzepatide along with GLP-1, these are hormones for blood sugar control and weight loss. These Danish researchers tested whether giving a GIP intraveneoiusly would level out blood sugar after meals in settings that put the men at high risk for low blood sugar, like exercise. This is a long acting insulin interchangeable with Lantus. Called Civica insulin glargine-yfgn. It will have a "CalRx" label in California. Max recommended price: $55 per box of five pens. Your cost may be lower depending on insurance. This is the lowest list price for long-acting insulin, without hidden rebates or markups. Yes. If you already have a prescription for insulin glargine (e.g., Lantus), you do not need a new one because Civica insulin glargine-yfgn is interchangeable. Marie Schiller announced a new resource for the diabetes community – a company called Locasana and a consumer brand called Health Sites. Schiller wrote on LinkedIn that she'll be CEO of the company that seeks to development an at-home diagnostic concept to assess tissue and skin health. Healthy Sites, a consumer brand dedicated to helping people care for their skin before, during, and after therapy. Many people struggle with issues like "pump bumps," redness, bruising, scarring, and irritation, and have had few product-level solutions available to them. Schiller co-founded the T1D Exchange and served as Vice President, Connected Care and Insulin Product Development and Site Head, Cambridge Innovation Center at Eli Lilly. She's lived with type 1 for more than 40 years. XX Very sorry to hear about the death of David Mitchell, the founder of Patients for Affordable Drugs. Mitchell died on January 2nd. I hope your December is going well and that you know somehow you're able to take some time for yourself in the middle of all the holiday rushing around this time of year can be magical and stressful and exhausting and wonderful, and you know, all the things. And it's the same thing over here, super busy getting all this stuff done before the end of the year. Love it. But, you know, getting podcast episodes out, writing all the things we write and planning for next year, as they say, We're staying booked and busy.   But quick behind the scenes here to better explain this episode, I taped this interview way back over the summer during the ADA Scientific Sessions conference. I had some technical problems. I actually thought I lost this interview. There were two interviews that seemed to have gone missing. We're going to air the other one very soon. But thankfully, I do have backups upon backups. So all the info that you're going to hear today is still relevant.   This product, a small A1C test, is still in development. The only dated bit is about their follow up event that took place in August. Orange Biomed was launched in 2021 in South Korea, with its US headquarters in Providence, Rhode Island. Its founders are two Duke University alums, and they're my guests, CEO Yeaseul Park and Co-President Unghyeon Ko, We are also joined by Janice Dru-Bennett. She is a senior advisor at the company. Now, English is not the first language of two of these three speakers. And we have so many questions for you, but Janice, let me ask you, you're there as everybody's setting up at the kind of beginnings of the show. What is it like right now for people who aren't familiar with ADA,   Janice Dru-Bennett  03:42 yes, you can hear the hammers in the background, although, but not on this podcast, but there's a lot of noise and people walking by. We're just setting up this the day before the exhibit hall opens and Dr Cole will be presenting at the Innovation Hub tomorrow, which is where we're sitting right now, with tables of innovators will be showcasing their diabetes innovations, and   Stacey Simms  04:04 there's a lot to get to. Dr Koh, I know you're presenting, but yes, let me ask you, like, what why? I know you said it's your third year, but why is orange biomed at ADA, what is your goal   Yeaseul Park  04:16 for us? ADA, is for a learning experience. As well as a platform to share. We come to see how all those around the world are fighting against diabetes, whether through clinical research, digital tools or technologies or community programs. At the same time, you're so proud to hear what orange biomat is building anytime, and eight months exhausting. That makes diabetes monitoring not accessible, not so many. And this year is especially exciting because Dr ko our co founder of orange buying at the group of speaking at ADA brand new program the innovation Hall.   Stacey Simms  04:58 That's awesome. So Dr Koh, tell me. Little bit about this, the Innovation Hub is pretty cool, but what are you going to be talking about?   Unghyeon Ko  05:05 Yeah, actually, I'm talking about the engineering part. I mean our technology, so our orange biomed, we are trying to solve a simple but a serious problem about the A1C accessibility. So to increase the A1C accessibility. So we are, we are developing at home device to measure the A1C level. So I'm, I'm talking about how difficult to increase the accessibility of A1C, but our technology is handled that difficult problem. So we now he's so agreed. So I'm going to introduce our technology and emphasize the importance of the A1C measurement at home.   Stacey Simms  05:49 Yeah, so A1C, it's interesting. My son was diagnosed at two, and in the pediatric world, you know, they'll just prick a finger generally and have that A1C right away. But my husband lives with type two, and he gets his labs drawn. And then it takes forever. So tell me a little bit before we go further about what you're hoping to do and making this easier for the patient,   Speaker 1  06:10 the frequent monitoring of A1C is so important to prevent the diabetic complications. So the money, so if you there is some so I can say that there is a research that if you measure the A 1d the four times a year, the People's A1C level is decreased like 3.8% but if you measure the A 1d at one per year, Then the A1C level is increased 1.5% so the frequent A1C monitoring is so important to prevent the diabetes complications. But problem is A1C measurement is only available at clinical site at this moment, so most of the A1C monitoring is done by the clinical side. So that's why people are difficult to monitoring A1C, because they have to visit the clinics forever. So is so like four times, or even eight times visit the clinics or hospital is quite difficult, especially in the people living in the far area from the hospital. So that's why the home A1C test is required. So I think that's why the accessibility of the A1C is one of the important things in managing the diabetes complications.   Stacey Simms  07:39 Dr Koh, is there evidence that, I mean more frequent A1C testing, I think would give many people peace of mind, perhaps. But is there evidence that it really does help in your health?   Speaker 1  07:51 Oh, yes, it is actually like from there is the research, like the famous research about the A1C level, like the research name this t and this research proved that the A1C is the one of the strongest predictor of diabetes complication. So A1C is completely related with the risk of diabetes complication. So like keeping A1C on the 7% dramatically lower the risk of diabetes complications. And also, there is another research in UK, the UK PDS study, and that study said they are A1C. Lowering A1C by just 1% can reduce overall mortality by 15% and microvascular complication by 37% so the roaring A1C is the goal of the treatment of the   Yeaseul Park  08:47 diabetes. So   Stacey Simms  08:48 when I think of at home diabetes tests, blood tests, seem like they're they're really sensitive, right? You have to be very careful with things like that, although we do, we did finger sticks at home for years and years. Are there challenges with at home A1C testing that that people like me could mess up,   Yeaseul Park  09:06 sure actually when I was doing pandemic outside system? So it's a new   Yeaseul Park  09:19 box of mustard with five or six needles inside, and we need to collect this blood to sound the left result. But then I really tried to collect the requirement matter blood, which is like it was like bleeding. Oh, it's not just retiring in one block, one drop of block, but it's like you need to try, yeah, many times, not just in one spot, to collapse in the block. And the other way you. Built female in, built a lot more broadly, to store your venous blood, and that's features like discomfort.   Stacey Simms  10:10 Would you mind taking us through your experience with the A1C testing? You were talking about how much blood it took? Yeah.   Yeaseul Park  10:17 So it, it requires many, many drops of blood. So I felt like it's like bleeding, and you make a lot of mess around the table. And so I felt, even though it's it was a topic time it was pandemic. So that's the only option I had at that time, but I wanted to make it simple and easier. And the other types of point of care devices only use a drop of blood still have some limitations, because we all don't want to bleed too much, so sometimes we try to finger stick very small and just squeeze to get enough blood. But if you squeeze to get enough sample volume, that's make your other liquid, like sweats, can also mix with your blood, and that actually affects the accuracy of the testing usually so many point of care devices also not recommend you to squeeze to get enough blood, so that means you need to treat a little too deep to get enough. So we really wanted to make this whole process or simpler and more problem.   Stacey Simms  11:43 Can you share a little bit about what the device looks like, what the patient experience is when they use it?   Speaker 1  11:48 Dr, CO, so our device is a palm size. Is in most like, like self, self poem, so it's a palm size device. So our device has no switch, but there is only a slider in the front of the device. So if you slide that, you can the device is turned on and you can insert the cartridge, and the cartridge is disposable cartridge. So after that, you just collect your blood and dilute it in the collecting tube and drop the sample into the cartridge, then analyze the A1C like automatically. So it's quite similar with the covid by covid test kit. So the covid test kit collect the sample in your nose and mix with the Rickett and drop it right? And it's quite similar.   Stacey Simms  12:45 So do you do a finger stick to put on the cartridge? How much blood to yells? Point, you know? How much blood do you need?   Speaker 1  12:52 Our devices for home use device, so it's quite we use a very different technology, because our device analyze the red blood cell one by one. So actually, we don't need exact unlike like five micro or 10 micro, we don't need the exact sample block. So we just need one drop of blood. So if the one drop is big, or if the one drop is right or small, it's fine for us. So one drop of blood, mix with their sample and drop one drop onto the cartridge. So maybe you can, you can drop one more than one drop, but we recommend one drop. So one drop of blood sample my dinner the rest yesterday pointed out that the skeezing the finger of blood is a problem for other device because, because in our body, there is a body wicked inside your under, under your skin. So if the body wicked is mixed with the blood samples, so it might be a problem because it dilute the blood sample. But our device, we analyze the Red Cross itself. So if it is diabetes, I think so we will find so you just puncture very best, and if you scale it, and it's totally fine for us. So it's, it's one of the good point of our device.   Yeaseul Park  14:20 How long does it take to get the results? It takes like, five minutes. Okay, yeah, and that's all at home. Yeah. Yezel, who do you see using this? Who is this for? Basically, it's for everyone. I think whoever has pre diabetes, diabetes type one, type two, and especially, I think who has limited access to primary care or lab testings. You know, many people who are older, tends to have more, especially the people who has limited access to primary care or lab testings. We believe this device can give more value to them. Yeah, and especially some people who have limited mobility, if they are older, or if they have experienced that amputation or something like that, they cannot go to the hospital by themselves. They need a caregivers to drive them to the hospital for the simple lab testing. But now I think it empowers patients who has that limitation still can take control their health by using this kind of home use device. How accurate   Stacey Simms  15:33 is it? I assume you have studies, and you've done some trials on how on the accuracy?   Yeaseul Park  15:37 Dr, CO, do you want to add that?   Speaker 1  15:40 Oh, yeah. So we are preparing the clinical trial. So the official clinical trials will be done within this year, but so that's our plan. But we we tested our device already using the in in last year. So last year, feasibility studies show that our device is quite similar with other point of care devices, and hopefully because at that time, our device, our especially our cartridge sensor, we just manufacturing our own like our in our lab. So this time, the official clinical trial in in this year, we are going to manufacturing in the factories so it might be more precise. So we hopefully we trying to chase the hospital accurate.   Stacey Simms  16:30 And I have to ask, where more and more people with diabetes are wearing a CGM and looking at time and range. What would you say to people who would tell you, well, we don't really need A1C anymore. We have time and range. Dr Cody, I see you nodding. Go ahead, yeah, yeah.   Speaker 1  16:47 So that's a very important point, but because the timing range is also important, and the CGM is very great technology for diabetes people. But problem is, like the A1C and C GM target different, like the CGM target the hypothesemia, but the A1C targeting the diabetes complications. So like, if you measure the timing range and you can manage your average glucose more nicely, but it might be prevent your hypothenia. But if you want to assess your diabetes management, you might be measure A1C. So if you measure timing range, but you also have to measure the A1C. So A1C is for everyone's and so. And also, the point is, if you don't treat the insulin, or if you don't treat the any medications, then you don't need to actually using the CGM, that's the ADH recommendation. So, but in in that case, you need the A1C as well. So A1C for everyone, and the CGM is for the people who treated the insulin. That's the ADA guide, right? And then,   Stacey Simms  18:12 yes, let me just ask you. You know, you came all this way. As you say, this is your third time at ADA. Trials are starting soon. What's your hope here? Is this something you see in homes of everyone who has any kind of diabetes? What's the big goal for Orange biomed?   Yeaseul Park  18:28 Every time we talk to a day, we can feel what's going on here in diabetes industry. It's a huge maybe first year, I the most frequently hard keyword was aid system. But after that, we now have GLP one, and now we hear more keyword around obesity. So that's a little slightly different trend I can feel. And once you come and join this full sessions, then I can see there's make everyone is making a progress, and we are all together. Want to fight against diabetes in their own way or with their own expertise, whether it's pharmaceutical, whether it's medical device or diabetes, sex, sometimes any other community programs that really support this patient and families, the community, and it's Really this whole atmosphere actually really motivates our team and myself, and we can feel the value. I can really feel this we are doing something valuable to patients and our community, and that's the most great thing, like the greatest thing that I can take when I come back to home with a. After the ADA. And for sure, we want to have opportunity to make voice what we are doing at Orange biomed, and want to deliver this value to the patient and other healthcare professionals. Otherwise, even though we are working hard to make this progress, no one knows, and that makes any changes the world. So that's the important purpose we are coming here. I'm Stacey Simms and every other Friday I bring you a short episode with the top diabetes stories and headlines happening now. Our top story this week: XX Certain glucose monitors from Abbott Diabetes Care are providing users with incorrect glucose readings, an error that has been linked with the deaths of at least seven people and more than 700 serious injuries worldwide, according to an alert from the US Food and Drug Administration.   Incorrect glucose readings can lead to improper treatment. Abbott warned that about 3 million FreeStyle Libre 3 and FreeStyle Libre 3 Plus sensors are affected, but no other Libre products. Patients can visit FreeStyleCheck.com to see if their sensors are affected and to get a replacement for free.   The FDA has also published specific information about the affected products in its alert. The agency considers this to be a "potentially high-risk issue" and will continue to update its website as information becomes available.   "Patients should verify if their sensors are impacted and immediately discontinue use and dispose of the affected sensor(s)," the FDA said.   https://www.cnn.com/2025/12/02/health/abbott-diabetes-glucose-monitors https://www.freestylecheck.com/us-en/home.html XX Omnipod 5 is getting some enhancements.. and Omnipod 6 is announced. The FDA cleared updates including  a lower, 100 mg/dL target glucose option and what they call a more seamless automated experience. "This is the most significant algorithm advancement to our Omnipod 5 System since its launch in 2022," said Eric Benjamin, Insulet EVP and COO. Insulet said the new 100 mg/dL target glucose expands Omnipod 5's customization range. It now features six settings between 100 mg/dL and 150 mg/dL in 10 mg/dL increments. The company said this flexibility allows healthcare providers to tailor insulin delivery more precisely. It supports individuals seeking tighter glucose management or aiming to meet specific glucose goals. Omnipod 5's latest upgrades also help users stay in "Automated Mode" with fewer interruptions, even during prolonged high glucose events. Insulet plans to launch the updates to the algorithm in the first half of 2026. The company announced plans for an Omnipod 6 – without a lot of detail - at the company's Investor Day event in November. They also talked about a new, fully closed-loop pump for the type 2 diabetes population. https://www.drugdeliverybusiness.com/insulet-fda-clearance-omnipod-5-algorithm-enhancements/ XX Dexcom, the global leader in glucose biosensing, announced today that the Dexcom G7 15 Day Continuous Glucose Monitoring (CGM) System will launch in the United States on Dec. 1, making it the longest-lasting CGM system with 15.5 days of wear.   Dexcom G7 15 Day will first be available through durable medical equipment (DME) providers on Dec. 1 with full retail launch in the coming weeks. Dexcom G7 15 Day will also be covered for Medicare beneficiaries.   Dexcom G7 15 Day's industry-leading wear-time will provide fewer sensor changes, less disruption and more time for people with diabetes to benefit from life-changing CGM technology.   New with Dexcom G7 15 Day:  Longest lasting CGM system with 15.5 days of wear. Best-in-class accuracy1 with an overall MARD of 8.0%. Easier glucose management with fewer monthly sensor changes and reduced monthly waste. This follows yesterday's announcement – the FDA has cleared Dexcom Smart Basal, the first and only CGM-integrated basal insulin dosing optimizer designed for adults 18 and older with Type 2 diabetes using long-acting insulin. Dexcom Smart Basal will use Dexcom G7 15 Day sensor data and logged doses to calculate personalized daily recommendations to guide users towards a more effective long-acting insulin dose, as directed by their healthcare provider. For 72 hours in a hotel setting, they ate heavy carb-and-fat meals, skipped all meal announcements, and didn't give any mealtime insulin boluses. The system handled almost everything automatically. Researchers said the device stayed in closed-loop mode 97% of the time and there were no incidents of diabetic ketoacidosis or severe hypoglycemia reported. While using the Freedom system, participants spent a median 61% of the day in the glucose target range — slightly higher than the 56% achieved with their usual pump at home. But the biggest improvement came overnight: time in range jumped to 96% with the closed-loop system compared to just under 70% during their home-pump week. With almost zero time spent below 70 mg/dL, researchers concluded that the fully automated Tandem system was both safe and effective even with unannounced, high-impact meals — hinting at a future of diabetes management that demands less effort from users.   XX Novo Nordisk reported promising mid-stage results for its experimental drug amycretin (AM-ee-creht-in) in diabetes patients on Tuesday. Amycretin, targets both GLP-1 and amylin hormones. In this study, it helped patients with type 2 diabetes lose up to 14.5% of their body weight over 36 weeks with weekly injections, far outperforming a placebo. The oral version delivered weight loss of up to 10.1%. Rival Eli Lilly  is surging ahead with its own amylin-based drug, eloralintide, which is advancing to late-stage testing after helping patients shed as much as 20% of their weight in a mid-stage trial. https://www.cnbc.com/2025/11/25/novos-next-gen-obesity-drug-shows-positive-results-heads-to-late-stage-testing.html XX The U.S. Medicare health plan said on Tuesday that newly negotiated prices for 15 of its costliest drugs will save 36% on those medications compared with recent annual spending, or about $8.5 billion in net covered prescription costs. The prices go into effect in 2027, including a monthly price of $274 for Novo Nordisk's popular GLP-1 drug semaglutide, sold as Wegovy for weight loss and Ozempic for diabetes. medicare's recent net price for Ozempic, opens new tab was $428 a month, according to an analysis published in the Journal of Managed Care and Specialty Pharmacy. Medicare put the drug's list price, before confidential rebates and discounts, at $959 a month. Based on such nondiscounted list prices, Medicare said savings on the 15 drugs ranged from 38% to 85%. The annual price negotiations were established under President Joe Biden's signature Inflation Reduction Act (IRA) of 2022. The CEO says, "This balance sheet restructuring provides a stronger foundation for LifeScan to support our base business, advance new growth strategies, and commence our journey to become one of the most comprehensive players in the glucose management space." https://www.drugdeliverybusiness.com/glucose-monitor-lifescan-emerge-from-bankruptcy/ XX An artificial intelligence (AI)-led Diabetes Prevention Program (DPP) was as effective as a traditional human-led program in achieving recommended goals for weight loss, A1c reduction, and physical activity, according to a randomized trial of adults with prediabetes and overweight or obesity. One example of a push notification: "Looks like you're at the grocery store, Rita! Want a quick list of high-fiber snacks or smart swaps to stay on track this week?" The app also provided location- and goal-based education, with gamification elements to promote engagement. Approximately one third of participants in both the AI and human-led groups achieved the primary outcome (31.7% and 31.9%, respectively). Results were consistent across sensitivity analyses and individual components of the composite endpoint. "As more AI-based programs emerge, head-to-head comparisons among different AI-DPPs will be informative. I'm Stacey Simms and every other Friday I bring you a short episode with the top diabetes stories and headlines happening now. It's world diabetes day! It is marked every year on 14 November, the birthday of Sir Frederick Banting, who co-discovered insulin along with Charles Best in 1922.   WDD was created in 1991 by International Diabetes Federation (IDF) and the World Health Organization and became an official United Nations Day in 2006 with the passage of United Nations Resolution 61/225. There will be a ton of stuff in your feeds today and that's great! I'm going to keep this to a pretty normal in the news episode.. although I do have my own World Diabetes Day announcement – I want YOUR community commercials. XX The Primary Oral Insulin Trial (POInT) is the first large-scale clinical trial to test whether giving at-risk children daily oral insulin could prevent or delay type 1 diabetes (T1D). Conducted by researchers from Helmholtz Munich and the Technical University of Munich across five European countries, the study enrolled more than 1,000 children with a genetic risk for T1D. Results published in The Lancet show that while oral insulin did not prevent the development of islet autoantibodies—an early sign of diabetes—it was safe and well tolerated. Importantly, researchers found that some children who received oral insulin developed diabetes more slowly than those given a placebo, suggesting potential protective effects in certain genetic subgroups.   Further analysis revealed that the response to treatment depended on the child's insulin gene variant. Children with genetic versions that raise diabetes risk appeared to benefit, showing delayed onset of the disease, while those without the risk variant did not. These findings point toward a future of personalized prevention, where genetic screening could help identify which children might benefit most from oral insulin. Researchers will continue following the participants until age 12 to assess long-term effects. The study marks a major milestone in decades of diabetes prevention research, highlighting both the promise and complexity of developing tailored, early interventions against type 1 diabetes. XX Joint US-Chinese research looking at generating new beta cells from stomach cells. Upon turning on the "genetic switch," the human stomach cells were converted to insulin-secreting cells within the mice and resembled pancreatic beta cells with respect to gene and protein expression. Encouragingly, when those experiments were done with diabetic mice, insulin secreted from the transformed human cells helped control blood sugar levels and ameliorated diabetes. The scientists hope that a similar approach can be taken to convert cells from a patient's own stomach into insulin-secreting cells directly within the body. Importantly, additional studies are needed to address if this approach is safe and effective to be used in patients. https://www.technologynetworks.com/cell-science/news/human-stomach-cells-tweaked-to-make-insulin-406694 XX A new study in Nature Metabolism may help explain why children born to mothers with type 1 diabetes are less likely to develop the disease early in life compared to those whose fathers or siblings have it. Researchers looked at nearly 2,000 mothers and their children and found that  kids whose moms have type 1 diabetes show changes in their DNA that may actually help protect them. These aren't genetic mutations, but epigenetic changes — chemical tags that turn certain genes on or off. The study found these changes in genes tied to the immune system and type 1 diabetes risk, suggesting that a mother's condition during pregnancy can shape her child's immune response in a protective way. Scientists identified more than 500 areas of DNA where these changes occurred, many in regions that control how the body's immune system works. Most of the changes appeared to calm down the kind of overactive immune response that leads to type 1 diabetes. Researchers even created a "methylation score" to help measure this protective effect. They say the next step is to confirm these results in more diverse groups and figure out exactly how these DNA changes help prevent early diabetes. https://www.news-medical.net/news/20251110/Maternal-type-1-diabetes-may-protect-children-from-developing-the-disease.aspx XX A new study from Karolinska Institutet and Stockholm University reveals that sons born to mothers with type 1 diabetes may develop early vascular dysfunction—independently of metabolic health. The finding may help shape future strategies to prevent cardiovascular disease early in life.     Children of women with type 1 diabetes are known to be at increased risk of developing cardiovascular diseases. This new study, published in Cell Reports Medicine, is the first to show that the risk is linked to early dysfunction in blood vessel cells in sons, even before any metabolic issues arise. The team is now investigating the long-term effects of maternal diabetes, with a particular focus on why sons seem to be affected earlier than daughters. https://medicalxpress.com/news/2025-11-sons-mothers-diabetes-early-vascular.html XX A new study presented at Kidney Week 2025 has shown that the drug finn-uh-near-own  a nonsteroidal mineralocorticoid-receptor antagonist, significantly reduced albuminuria—a key marker of kidney damage—in people with type 1 diabetes (T1D) and chronic kidney disease (CKD). This is the first major breakthrough for this population in more than 30 years. Researchers found that patients taking finerenone saw a 25% average reduction in albuminuria compared to placebo, an improvement that suggests a lower long-term risk for dialysis or kidney transplant. The phase 3 FINE-ONE trial involved 242 adults with T1D and CKD, and results showed benefits as early as three months. The drug was generally well tolerated, with side effects similar to those seen in patients with type 2 diabetes, though mild hyperkalemia (high potassium levels) was slightly more common. Experts say the findings could change the way doctors treat kidney complications in type 1 diabetes, an area that hasn't seen new therapies since the early 1990s. Currently, treatment options rely on blood pressure and blood sugar management, along with renin-angiotensin system (RAS) inhibitors. Finerenone, which is already approved for type 2 diabetes-related CKD, targets overactivation of a receptor that drives kidney damage. Based on these results, Bayer plans to seek FDA approval in 2026 for use in people with T1D and CKD. San Diego-based Tandem revealed that it received FDA approval for the Android version of its Mobi mobile app. Clearance brings Mobi — which the company describes as the world's smallest, durable automated insulin delivery system — to more users. The pump, which pairs with Tandem's Control-IQ+ algorithm, previously worked with iOS software.   Tandem — one of the largest diabetes tech companies in the world — expects to begin a limited rollout next month, followed by full commercial availability in early 2026. This marks the latest milestone for the company, which continues to expand its offerings and widen its reach within the diabetes patient population.   We had a great interview with Tandem on our previous episode, but as I said at the time, it was coming before their earnings call. So here's an update: The company plans to submit the tubeless mobi to the fda before the end of this year.. possible approval and shipping date is hoped for by middle of 2026. Trials for their fully closed loop next-generation algorithm which we tlkaed abou ton the show should be launched in 2026 The Sigi patch pump will be developed and launched as a next-generation version of the Mobi Great job by Dr. David ? Ahn – he posted on IG after getting a message from tandem CEO John Sheridan? 1. First, the Tandem X3 *is* still absolutely in development, contrary to my speculation In yesterday's video. As many of you appropriately pointed out, there is definitely a market for a 300 unit pump, a pump with a screen, and a pump that does not require smartphone control. So from our brief chat, the sense I got that is that the X3 would be more of a refresh of the X2 with newer components, such as a USB-C connector and better memory, rather than a total redesign from the ground up. In terms of timing, all I could get was that it was "not too far distant in the future," which could mean anything I guess, but at least it's still on the way! 2. Next up, he also reassured me that they are working closely with Dexcom to support the G7 15 Day sensor within the next few months. I suspected as much, but it's always good to hear confirmation. 3. Lastly, he did confirm that Tandem is far along in developing a Caregiver/Follow app to allow the remote viewing of glucose and insulin data from a Tandem pump. He explained that it will be based on Sugarmate, the popular diabetes data dashboard app that Tandem acquired back in Jun 2020. While I don't know if every feature will make it into the Tandem caregiver app, Sugarmate is well-liked for its highly customizable dashboard and highly configurable alerts. Sugarmate even has the option to send a text message or phone call for urgent lows. Eversense will be integrated with the sequel twist pump – again I'm hearing soon but no timeline. These include improvements in blood vessel health, physical fitness, and the body's ability to regulate blood sugar. Since 2006, doctors have typically encouraged patients with elevated blood sugar levels to combine metformin with exercise, expecting that the two proven treatments would produce stronger results together. However, the new research suggests this may not be the case. In this study, Exercise alone improved vascular insulin sensitivity, meaning blood vessels responded better to insulin and allowed more blood flow to muscles. This matters because insulin's ability to open blood vessels helps shuttle glucose out of the bloodstream and into tissues, lowering blood sugar after meals. But when metformin was added, the improvements shrank. The drug also diminished gains in aerobic fitness and reduced the positive effects on inflammation and fasting glucose. The findings don't mean people should stop taking metformin or exercising, Malin said. Instead, it raises urgent questions for doctors about how the two treatments can be combined and the need for close monitoring. The issue could cause users to miss alarms, alerts or notifications related to estimated glucose values, according to a Food and Drug Administration database entry posted Oct. 30. The glucose sensor and the app are still available, but Dexcom required users to update the app to a new version. Dexcom began the recall on Aug. 28. The FDA designated the event as a Class 1 recall, the most serious kind. The result will provide researchers with an unprecedented dataset to accelerate the development of new clinical guidelines, next-generation diabetes technology, and personalized care models.   Recruitment is expected to launch in early 2026 through an IRB-approved study. These phase 3 trials will investigate whether the drug can delay T1D onset or progression and will open for recruitment soon. Baricitinib has the potential to extend the "honeymoon period" of T1D, meaning that it could preserve remaining insulin-producing beta cells earlier in disease progression. More beta cells mean better blood sugar management—and potentially reduced long-term complications. JAK inhibitors, including baricitinib, are already FDA-approved for other autoimmune diseases, such as rheumatoid arthritis, alopecia, and more. JAK signaling pathways are associated with overactive immune responses, so blocking this pathway may turn down the immune response. The phase 2 Breakthrough T1D-funded BANDIT study was key in showing that this drug is safe and effective in T1D. Having found 79% of people with diabetes have faced bias or misunderstanding at work, the medtech giant is rolling out a range of resources intended to trigger changes in how workplaces approach the condition. Insulet's "The Day Diabetes Showed up to Work" campaign. based on a survey of almost 10,000 people 79% of people with diabetes have faced bias or misunderstanding at work,.   Almost 90% of people with diabetes surveyed reported experiencing barriers at work due to their condition, and more than 40% of people with diabetes and caregivers said they have workplace-related anxiety tied to the metabolic disease. Around one-quarter of respondents reported fears that diabetes could limit opportunities or lead to workplace discrimination and judgment, and a similar proportion of people said they conceal their condition. https://www.fiercepharma.com/marketing/widespread-workplace-challenges-people-diabetes-spark-insulet-campaign XX New directive issued by the Trump administration could mean people seeking visas to live in the U.S. might be rejected if they have certain medical conditions, including diabetes or obesity.   The guidance, issued in a cable the State Department sent to embassy and consular officials and examined by KFF Health News, directs visa officers to deem applicants ineligible to enter the U.S. for several new reasons, including age or the likelihood they might rely on public benefits.   The guidance says that such people could become a "public charge" — a potential drain on U.S. resources — because of their health issues or age.   The cable's language appears at odds with the Foreign Affairs Manual, the State Department's own handbook, which says that visa officers cannot reject an application based on "what if" scenarios, Wheeler said.   The guidance directs visa officers to develop "their own thoughts about what could lead to some sort of medical emergency or sort of medical costs in the future," he said.
