Understanding FND and Pain: A New Paradigm with Prof Jon Stone
Send us Fan MailIn this episode, i got the opportunity to interview a person I hugely admire. He is the person most responsible for putting Functional Neurological Disordeer (FND) on the scientific map. Professor Jon Stone is Professor of Neurology at the University of Edinburgh. Since 1999, he has published over 400 papers, built the world's most widely used patient resource for FND — neurosymptoms.org — and led on international diagnostic criteria. As you may know, FND is a real, disabling, neurological condition. It affects how the brain processes and generates movement, sensation and — critically for this audience — pain. It is not feigning. It is not malingering. And for many patients, it is treatable. keywordsFND, pain, neurology, diagnosis, treatment, neuroimmune, predictive processing, multidisciplinary rehabkey topicsFND as a neurological condition affecting movement and sensationThe overlap between FND and chronic pain conditionsDiagnostic signs like Hoover sign and tremor entrainmentThe neuroimmune and predictive processing models of FNDMultidisciplinary approaches to FND and pain managementChapters00:00 Understanding Functional Neurological Disorder (FND)03:15 The Intersection of FND and Pain07:44 Defining FND in Modern Medicine11:28 Clinical Signs and Diagnosis of FND13:29 FND as a Software Problem15:52 Prevalence of Chronic Pain in FND Patients19:08 Mechanisms Behind FND and Pain Overlap21:10 Reconciling CRPS and FND23:08 Understanding the Overlap in Pain Conditions30:26 Predictive Processing and Its Role in FND35:58 Interdisciplinary Approaches to FND Treatment43:31 The Role of Pain Physicians in Managing FND46:15 Resources for Learning About FND Resourcesneurosymptoms.org - https://neurosymptoms.orgInternational FND Society - https://fndhope.orgFND in chronic pain clinics: 2023 - DOI: 10.1111/ene.15892 CRPS and FND : 2018: Popkirov S, Hoeritzauer i, Colvin L, doi:10.1136/jnnp-2018 318298 Pain and FND 2024 review: Steinruecke M, Mason I, Keen M, et al. Beyond Flexibility: Hypermobility, Chronic Pain, Movement and Hope with Jeannie Di Bon
Send us Fan MailIn this episode, I sit down to chat with Jeannie Di Bon—movement therapist, educator, founder of The Zebra Club, author, and creator of the Integral Movement Method. Through both professional expertise and lived experience, she has helped thousands of people rethink movement not as something to fear, but as a pathway towards confidence, resilience and recovery.. When most people think about hypermobility, they imagine someone who's simply very flexible. But for many people, hypermobility comes with persistent pain, fatigue, autonomic symptoms, gastrointestinal problems, anxiety, neurodivergence, repeated injuries, and years of being misunderstood.Today we're exploring a movement that is changing that narrative.keywordshypermobility, EDS, Zebra Club, pain management, movement therapy, neurodivergence, chronic pain, patient support, NHS, integrative movementkey topicsHypermobility and EDS diagnosis journeyThe Integral Movement Method and its principlesThe role of breath, proprioception, and nervous system regulationCommunity support through the Zebra ClubImpact of misconceptions and the importance of early diagnosisThe overlap between hypermobility, neurodivergence, and mental healthChallenges within NHS pathways and the need for integrated careResearch and future directions in hypermobility and pain managementChapters00:00 Understanding Hypermobility and Its Challenges03:01 Jeannie Di Bon's Personal Journey with EDS05:58 The Birth of the Integral Movement Method08:55 The Zebra Club: A Community for Hypermobility12:01 The Role of Education and Awareness in EDS14:49 The Biopsychosocial Model in Pain Management18:04 Innovative Approaches to Movement Therapy20:56 The Importance of Breath and Proprioception23:52 Challenges in Traditional Rehabilitation for Hypermobility26:57 The Evolving Landscape of EDS Awareness29:58 Future Directions for Hypermobility Care33:38 Fragmentation and Community Support34:45 Hypermobility and Neurodivergence36:41 Understanding Misconceptions in EDS38:46 The Role of Clinicians in EDS Care43:00 Community Collaboration with Clinical Models48:30 Transformative Patient Stories51:34 Future Directions in EDS ResearchResourcesWebsiteThe Zebra Club : https://thezebra.club/Disclaimer:This podcast is for education and general information only and does not replace personalised medical advice. Mental Wealth, Trauma and Pain Management: Speaking with Dr Rani Bora
Send us Fan MailSummaryIn this episode of Pain Speak, Deepak Ravindran and Dr. Rani Bora explore the concept of mental wealth, emphasizing the importance of cultivating mental health beyond merely reducing symptoms. They discuss the intersection of mental health and chronic pain and the evolving perspectives on mental health treatment. The conversation delves into the definitions of mental wealth, the mind-body-spirit connection, and the trauma-informed paradigm, while also addressing the challenges of nervous system dysregulation and the impact of medication on mental health. In this conversation, Rani and Deepak explore the complexities of deprescribing medications, particularly in the context of pain management. They discuss the challenges patients face when trying to come off medications, the role of trauma in pain perception, and the importance of understanding the kindling effect. Additionally, they touch on the role of spirituality in well-being and the importance of empowering patients in their healing journey.TakeawaysMental health should focus on cultivating mental wealth, not just symptom reduction.Chronic pain is deeply connected to mental health and trauma. The conventional approach often focuses on fixing people rather than empowering them.Mental wealth encompasses mind, body, and spirit, not just the mind-body connection. The trauma-informed paradigm shifts the focus from 'what happened to you' to 'what happened inside of you.'Nervous system dysregulation can manifest in various physical and psychological symptoms.Medication tapering must be approached with caution to avoid dysregulation.Deprescribing can be challenging, especially for long-term users. The kindling effect can make withdrawal more difficult for some patients.Recovery can begin before symptoms fully resolve.Spirituality plays a crucial role in overall well-being and healing.Mental health recovery involves more than just symptom reduction.Keywordsmental health, mental wealth, chronic pain, trauma, resilience, holistic psychiatry, nervous system dysregulation, medication, healing, well-being deprescribing, pain management, trauma, medication withdrawal, kindling effect, diagnostic labels, recovery, spirituality, mental health, holistic psychiatryDr Rani Bora's Clinic is both online and Brick and mortar and she can be found at https://holisticpsychiatryclinic.com/Disclaimer:This podcast is for education and general information only and does not replace personalised medical advice. Lifestyle Medicine: Beyond Disease Management with Prof.Sunil Kumar
Send us Fan MailSummaryIn this episode of Pain Speak, I speak to Prof. Sunil Kumar and discuss the pressing challenges in healthcare related to long-term conditions and the concept of 'waiting well.' We explore the journey of Dr. Kumar from anesthesia to becoming a thought leader in lifestyle medicine, emphasizing the importance of health capacity, resilience, and social connection. We delved into the significance of prehabilitation in preparing patients for surgery and the need for a shift from compliance to patient ownership in health management. In this conversation, Sunil discusses the importance of behavior change, autonomy, and motivation in healthcare. He emphasizes the need to address burnout among healthcare professionals, particularly junior doctors, and shares insights on developing resilience through personalized approaches. Sunil advocates for lifestyle medicine as a foundational element in healthcare, aiming for a shift from reactive disease management to proactive health creation. He highlights the significance of auditing one's life and integrating purpose and passion into daily routines to combat burnout and enhance well-being.TakeawaysThe growing number of people living with long-term conditions is a significant challenge for healthcare systems.Waiting for treatment can be transformed into an opportunity for building strength and resilience.Lifestyle medicine is an evidence-based clinical discipline, not just lifestyle advice.Health should be defined as the capacity to live the life you want, not merely the absence of disease.Social connection is a critical determinant of health, with evidence showing its impact on longevity.Prehabilitation is an underutilized therapeutic window that can improve patient outcomes before surgery.Empowering patients through small, manageable changes can lead to significant health improvements.Behavior change requires capability, opportunity, and motivation, as outlined in the COM-B model. Lasting change is built on autonomy, competence, and connection.People resist being changed, not change itself.Lifestyle medicine should be a first-line therapy, not an add-on.The future of healthcare should focus on proactive health creation.Keywordshealthcare, lifestyle medicine, prehabilitation, chronic conditions, social connection, patient empowerment, waiting well, health capacity, resilience, behavior change behavior change, autonomy, motivation, burnout, healthcare professionals, junior doctors, resilience, lifestyle medicine, clinician well-being, health systemsFind more about Dr Sunil Kumar at https://drsunilkumar.co.uk/Disclaimer:This podcast is for education and general information only and does not replace personalised medical advice. VIP -The Future of Pain Services in the UK: A Conversation with Dr. Helen Makins
Send us Fan MailMy guest today is Dr Helen Makins, a Consultant in Pain Medicine at Gloucestershire Hospitals NHS Foundation Trust, UK and someone whose career has taken her across clinical practice, education, commissioning, global health and now national leadership.Helen is currently the National Clinical Lead for the Chronic Pain Programme at GIRFT – Getting It Right First Time, a major NHS initiative focused on improving clinical outcomes, reducing variation and redesigning services across the country. But what makes Helen particularly interesting is the breadth of her work — from teaching pain education internationally with Mercy Ships, to developing innovative social prescribing programmes for young people with pain, to exploring the links between neurodiversity, childhood experiences and chronic pain. She has also worked closely with the Faculty of Pain Medicine, championing better communication skills, clinician wellbeing and the integration of biopsychosocial approaches into training. So today we’re going to explore:• how someone ends up leading national pain transformation work• what GIRFT might mean for the future of pain services• why communication and de-medicalisation matter so much in pain consultations• and how we might better support both patients and clinicians in this space.keywordsPain management, NHS, chronic pain, social prescribing, communication skills, lifestyle medicine, neurodiversity, healthcare commissioning, GIRFT, patient-centered carekey topicsEvolution of NHS pain servicesImportance of communication skills in pain managementRole of social prescribing and lifestyle medicineImpact of neurodiversity on chronic painGIRFT program and healthcare commissioning resourcesGIRFT website - https://www.england.nhs.uk/girft/Stephen Rolnick's Motivational Interviewing Course - https://www.motivationalinterviewing.org/Essential Pain Management Program - https://www.rcoa.ac.uk/faculty-of-pain-medicine/education/essential-pain-managementHelen Makins' LinkedIn - https://www.linkedin.com/in/helenmakins/Disclaimer:This podcast is for education and general information only and does not replace personalised medical advice. Voices in Pain - Sebastian Senociain
Send us Fan MailTransforming Pain Management: An Innovative Approach from the NHSSebastian is talking live at the Live well with Pain conference in Warwick in March 2026. In this podcast recorded live in Brighton at the BSLM Annual conference in September 2025, i spoke to Sebastian Senociain, An integrated wellness practitioner with a variety of skills and knowledge helping people in primary care manage their pain. Innovative approaches to pain management are more crucial than ever now and we can see how Sebastian is redefining pain management within the NHS. His transformative work focuses on integrating holistic methods and fostering community, significantly improving the lives of those living with chronic pain. Sebastian’s journey began in Chile, where he trained as a medical doctor. His experience in primary care, filled with frustrations over the conventional disease-driven approach, led him to explore alternative methods of healing - including yoga, ayurveda, functional medicine and other approaches in physical and mental health. At the NHS, Sebastian recognized a pressing need for a more integrative approach to chronic pain management. He noted that Cornwall had a higher than national average prescription rate for opioids, which sparked his initiative to shift the focus from medication to self-management. He piloted a program aimed at helping patients manage their pain through education and community support rather than solely relying on prescriptions. The pain management program, started in 2023, was meticulously designed to address the diverse needs of patients. Sebastian emphasized the importance of validation and understanding patients' stories. Each 30-minute consultation allowed patients to express their experiences and concerns, forming a foundation for co-designing their care plans. This patient-centered approach not only improved their mental health but also aimed to reduce reliance on medication. He has presented his work now regionally and nationally and this was an option to see his poster and talk to him. The video can be seen on youtube. Pain Reprocessing Therapy Part 3 - Separating the hope and hype
Send us Fan MailIn this solo episode, I explore Pain Reprocessing Therapy (PRT), a novel approach to managing chronic pain that focuses on changing the brain's interpretation of pain signals. I compare PRT with existing therapies like Cognitive Behavioral Therapy (CBT) and Acceptance and Commitment Therapy (ACT), discussing the neuroscience behind pain and the effectiveness of PRT. The episode emphasizes the importance of understanding pain as a complex emergent interplay of neurological and psychological factors, advocating for an integrated approach to pain management that includes PRT alongside traditional therapies.TakeawaysChronic pain may not indicate physical damage but a nervous system issue.PRT offers a new perspective on pain management.Changing the brain's interpretation of pain can lead to recovery.PRT is gaining attention due to its potential effectiveness. The nervous system plays a crucial role in chronic pain.Evidence supports PRT's effectiveness in specific pain populations.PRT differs fundamentally from CBT and ACT in approach.Predictive processing theory explains how the brain perceives pain.PRT can lead to significant reductions in pain intensity.Integrating various therapies can enhance pain management outcomes.KeywordsPain Reprocessing Therapy, Chronic Pain, Neuroscience, CBT, ACT, Predictive Processing, Pain Management, Pain Neuroscience, Emotional Awareness, Therapy TechniquesDisclaimer:This podcast is for education and general information only and does not replace personalised medical advice. Pain Reprocessing Therapy Part 2 - A Lived Experience View
Send us Fan Mail In this deeply human episode of PainSpeak, I’m joined by Fiona Symington, who shares her lived experience of chronic fatigue and persistent pain symptoms — and, importantly, what recovery felt like from the inside and what pain reprocessing therapy (PRT) meant to her. This is not a conversation about quick fixes, techniques, or “getting it right.” Instead, Fiona gently walks us through the emotional and nervous-system journey of living in a body that no longer felt safe, predictable, or trustworthy. We explore the early signs that something was wrong, the impact of not being believed, and how fear, hypervigilance, and constant symptom-monitoring can quietly become part of the suffering itself. Fiona reflects on the slow, often non-linear shifts that marked her path toward healing — moments of understanding, softening, and rebuilding trust rather than pushing or fixing. Together, we talk about identity, grief, and the unexpected challenges of recovery, including how unfamiliar safety can feel after long-term illness. This episode is especially for anyone who feels exhausted by trying, overwhelmed by advice, or quietly worried they’re “behind” in their healing. Fiona’s story offers reassurance rather than instruction, and hope without pressure — a reminder that recovery does not begin with effort or perfection, but often with understanding, compassion, and a nervous system learning that it is no longer under threat. This conversation is shared as one person’s experience, not medical advice, and listeners are encouraged to listen gently, at their own pace. I am really gratefu to Fiona for her time and deeply impressed that she is now training to be a clinical psychologist in order to do the research and help people like her. Pain Reprocessing Therapy Part 1 - Pain Protection and Safety
Send us Fan MailWhat if chronic pain isn’t a sign of damage — but a learned alarm the nervous system can unlearn? In this episode of PainSpeak, I’m joined by Caroline Davies, a specialist pain physiotherapist with over 25 years of experience working across the NHS, the US healthcare system, and now in independent practice in the UK. Caroline’s work sits at the cutting edge of modern pain care, blending deep physiotherapy expertise with contemporary neuroscience and emotion-focused therapies.Together, we explore one of the most important — and often misunderstood — shifts happening in pain medicine today: the move from viewing chronic pain purely as a problem of tissue damage, to understanding it as a problem of persistent threat signalling in the brain and nervous system. From Physiotherapy to Pain Reprocessing Therapy - This episode follows close on the heels of the Channel 4 documentary on 8th Jan featuring Dr. Rangan Chatterjee - The drug Free doctor as he interviewed a patient with chronic pain who had used Pain Reprocessing therapy to reduce her pain significantly. Of course this has given us the chance to understand in more detail what PRT is and isnt!!About Caroline DaviesCaroline began her career in traditional musculoskeletal physiotherapy, working extensively with complex pain presentations including CRPS, pelvic pain, cancer-related pain, and trauma-associated pain. Over time, she noticed a familiar pattern many clinicians will recognise: people whose scans looked “reassuring,” whose tissues had healed, yet whose pain continued — sometimes worsening — despite doing everything they were told.In this conversation, We talk candidly about the limits of conventional approaches, the frustration clinicians can feel when progress stalls, and the curiosity that eventually led her to train in Pain Reprocessing Therapy (PRT), Emotional Awareness and Expression Therapy (EAET), and more recently, Internal Family Systems-informed approaches. As always on PainSpeak, the aim is not to tell you what to think — but to help you understand pain differently, so that change becomes possible.Disclaimer:This podcast is for education and general information only and does not replace personalised medical advice. Overcoming pain - Why a Lifestyle medicine focussed approach could help
Send us Fan MailGenerated by Notebook LM. Pls forgive the initial hyperbole generated by the AI about my credentials!!! (sounds very cringy when I heard it but cant do much about that as the rest was very good!)Recently presented at the 2024 Lifestyles Summit organised by Modality and Pathfinder CIC at Google HQ in London, my presentation outlines the need for a trauma-informed approach to pain management. It argues that the conventional biomedical model is flawed and insufficient in addressing the complex interplay of biological, psychological, and social factors influencing pain experience. The presentation advocates for a Lifestyle Medicine Systems Approach to pain management, which focuses on the role of lifestyle factors, such as diet, exercise, stress management, and sleep, in pain relief and how that can massively help in all kinds of chronic pain especially nociplastic pain . It emphasizes the importance of addressing Adverse Childhood Experiences (ACEs) as they significantly contribute to chronic pain and calls for integrating lived experience into clinical practice. Overcoming Pain - Chronic pelvic pain - An overview
Send us Fan MailChronic Pelvic Pain and Lifestyle FactorsThis Notebook LM summary provides an overview of chronic pelvic pain based on a couple of key resources and a presentation that I gave on Chronic pelvic pain to health care professionals. I highlight the importance of a holistic approach that considers lifestyle factors, psychological well-being, and trauma-informed care. Remember, seeking professional medical advice and support is crucial for effective diagnosis and management of CPP.Chronic pelvic pain (CPP) is a common condition that affects millions of women worldwide. It is defined as intermittent or constant pain in the pelvic area for at least six months, unrelated to menstruation, intercourse, or pregnancy. CPP is a symptom, not a diagnosis, and can be caused by various factors, including endometriosis, fibroids, irritable bowel syndrome (IBS), and psychological factors.Challenges and Complexity of CPPDiagnosing and treating CPP can be challenging. A significant proportion of women with CPP have more than one diagnosis, and pain often increases with multisystem symptoms. Additionally, a substantial number of diagnostic laparoscopies yield negative results, and adhesions, while common findings, are not always the root cause of pain. In many instances, the underlying cause of CPP remains unclear so what is paramount is a validation of the patient symptoms and a multidisciplinary holistic approach. This involves a combination of medical treatments, therapies, and lifestyle modifications. Pain management programmes can be beneficial, providing guidance, coaching, and support from a pelvic pain specialist. Trauma-Informed CareA significant proportion of women with CPP have experienced adverse childhood experiences (ACEs), which can have long-term health consequences, including chronic pain. Trauma-informed care is an important aspect of CPP management. This approach involves understanding the impact of trauma on patients, validating their experiences, and providing sensitive and supportive care.Resources for Further InformationEndometriosis UK: www.endometriosis-uk.orgPelvic Pain Support Network: www.pelvicpain.org.ukDepartment of Health Expert Patient Initiative: www.expertpatients.co.uk/www.livewellwithpain.co.ukCurable App – on all platformsThe Pelvic Pain Network - A collaborative of Physiotherapists with an active research interest in pelvic painDisclaimer:This podcast is for education and general information only and does not replace personalised medical advice. Overcoming Pain - Diving deep into CRPS
Send us Fan MailDr. Ravindran's presentation, "Complex regional pain syndrome: Trauma informed CRPS Care An integrated approach," focuses on trauma-informed care for CRPS patients. This was presented at the UK annual conference organised by CRPS UK. This podcast was a summary of that talk and the source files used by Dr Ravindran for his presentation and generated using the Noteook LM by Google.Dr. Ravindran emphasises the importance of communication and understanding in a trauma-informed approach to CRPS care. He points out common issues that can arise, including miscommunication between care teams, a loss of trust between the patient and medical staff and a lack of consultant review, leading to patient anger, frustration and feelings of being disbelieved.Dr. Ravindran's M.I.N.D.S.E.T approach to CRPS acknowledges the different kinds of pain experienced by CRPS patients, including nociceptive pain, nociplastic pain and neuropathic pain. This approach incorporates various treatment methods to address the physical and mental aspects of CRPS:Dr. Ravindran also discusses Scrambler Therapy, its history, the reasons why it might be effective and the existing evidence base. He provides links to resources for further information about Scrambler Therapy. Pain Speak Special - The Painful Truth with Dr Monty Lyman
Send us Fan MailWelcome to a Pain Speak Special focussing on a new book on pain "The Painful Truth" by Oxford based junior doctor and academic scholar Dr Monty Lyman released on 15th July. This becomes the 7th book on pain released in the UK in just the 6 months of this year! We're currently experiencing a Renaissance in pain science and it is great to add Monty's book to this fantastic list of resources for our patients and general public and for anyone interested in knowing more about pain science.Order your copy of the Painful Truth hereIn recent years our understanding of pain has altered so radically it's fair to say that everything we thought we knew about pain is wrong. I made the case in my book as well. As Dr Monty Lyman reveals, we misunderstand pain - with harmful consequences. This is Dr Monty Lymans second book. His first one " the Remarkable life of the skin" was shortlisted for the Royal Society Science Book Prize". He is a remarkable story teller and in this book , The Painful Truth, he explores and presents cutting-edge research, encompassing phantom aches to persistent pain, and interviews survivors of torture as well as those who have never felt pain. It shows the myriad ways in which pain acts as a protector sometimes unnecessarily. He not only provides hope for reducing and managing pain, but takes us to a deeper understanding of what it means to be human. In the end he presents the hopefuly side of taking this knowledge and using it for improving our health and I am glad that i got a chance to be featured as well! This is the untold story of pain - our most elusive feeling and I am glad that I got a chance to be interviewed for the book and he has agreed to come on to this podcast. Longcovid - Viewpoints from occupational health services
Send us Fan MailIt has been a hugely gratifying and humbling experience to get the chance to set up the Longcovid Service for Berkshire and work with a great team from primary and community care. We have learnt a lot and are continuing to learn as we help our patients suffering from Longcovid and offer them support, a listening ear, appropriate testing and treatments where available in a trauma informed manner. In this episode, I speak to my consultant colleague Dr Blandina Blackburn, Occupational Health Consultant. Dr.Blackburn has been working with staff at the Royal Berkshire for many years and is a hugely respected national figure in Occupational Health and represented her profession at the highest levels. We had recently collaborated to write an article for the Journal Occupational Health@work. In the podcast episode, I wrongly mention it as an editorial so apologies for the error. Please find the link at https://www.atworkpartnership.co.uk/journal/issue/17_5/contents/long-covid-and-its-implications-at-workShe has been invaluable in providing a clear sounding board for many patients who havent been able to go back to work and this includes a large number of NHS workforce. There is a lot of confusion prevailing right now as to how return to work can be established when we are still learning about the long terms effects of Post Covid so I am glad that Blandina can inform us about the complexities and suggest a way forward for now. Rulings are likely to change and we might have her coming back once we know more about the condition and laws change and courts make further announcements. Longcovid - What do patients feel and want from a service?
Send us Fan MailIt has been a hugely gratifying and humbling experience to get the chance to set up the Longcovid Service for Berkshire and work with a great team from primary and community care. We have learnt a lot and are continuing to learn as we help our patients suffering from Longcovid and offer them support, a listening ear, appropriate testing and treatments where available in a trauma informed manner. In the first episode of this series, I speak to 2 patients who are part of the Longcovid SOS group .Each tell their story from their perspective and it is more than important in a condition like Longcovid to know what the lived experience of these patients are and their symptoms to be taken into account when designing a service that can meet their needs. Each persons symptoms are unique and thei experiences are different and I have learnt a lot from talking to my patients on how varied their presentations have been and how they have told their story. In future episodes, I will be talking about our present upto date understanding around Longcovid and newer studies and research and my team at Reading Berkshire. Please provide a feedback and comments on the podcast and any questions that you may have and recommend guests.
