Perfect Care Podcast

The 6th episode of the Perfect Care podcast is deeply personal. This Deep Dive blends lessons from leadership experts (such as Zaleznik, Kotter, Goleman, Collins, Heifetz, as well as the US Army's doctrine on leadership) with Dr. Lobdell's journey from the operating room, through a life-changing accident, additional training in critical care, business school, & subsequently leading quality efforts at the Sanger Heart & Vascular Institute (SHVI). Most recently, the focus has been the strategic, digital transformation & world-class results of Perfect Care (PC). These efforts & results require great teammates-such as Shannon Crotwell RN BSN CCRN (the operational lead of PC), Dr. Geoff Rose, President of SHVI, & the terrific teammates at SHVI & Advocate Health. Finally, a special shoutout to Team Lobdell for their love & unwavering support as well as Wayne Sotile, PhD for his wisdom, & coaching on this rich journey.

The 5th episode of the Perfect Care Podcast relates our quality improvement journey & reducing the rates of acute kidney injury (AKI) after surgery. We also reflect on the value of doing common things, uncommonly well, learning faster, and continuously improving. Finally, we consider the potential for integrating novel tools to monitor, treat, and evaluate AKI.

The 4th episode of the Perfect Care Podcast reviews the evolution of cardiac critical care @ the Sanger Heart & Vascular Institute. We think of these efforts, which began in 2004, in two eras: analog & digital. The later, digital era (built on the analog’s culture of data driven learning, teamwork and high-performance) includes tele-rounding and virtual critical care, which led to the more expansive and transformational remote perioperative monitoring initiative.

This episode of the Perfect Care Podcast takes a deeper dive into the foundational role of early extubation to drive learning, continuous improvement, and the development of a culture of high-performance and high-reliability. We are grateful to our patients and their families who inspire us, the interdisciplinary, clinical team of nurses, respiratory therapists, nurse practitioners, cardiac intensivists, cardiac surgeons, and administrative leaders who have supported us for decades.

In our second episode, of the Perfect Care Podcast, we reflect on our history. Leadership, teamwork, & the development data driven, of a high-performance and high-reliability culture. Further, we review the elements of our efforts and results that built the foundation for Perfect Care.

Perfect Care is the brainchild of Kevin W. Lobdell MD, System Director at Sanger Heart & Vascular Institute and Clinical Professor-Wake Forest University School of Medicine (which are a part of Advocate Health, headquartered in Charlotte NC). Shannon Crotwell, RN, BSN, CCRN has led the administrative and clinical implementation efforts of Perfect Care since its inception in 2018. Perfect Care aims to elevate the engagement of patients and their family, reduce barriers to care, & leverage rare, talented clinicians with technology. With the visionary and generous support of The Duke Endowment and leadership of Sanger Heart & Vascular Institute + Advocate Health, we have enrolled > 2250 patients in Perfect Care. Of note, our results with the first 1000 consecutive Perfect Care heart surgery patients are world-class: ZERO mortality. ~17% shorter postoperative length of stay (1 day), 33% fewer 30-day readmissions (4.7%), and 97.8% discharged to home.
