    The World Thought Flat and the Dance of the Dinosaurs Rise and Fall of Consolidation: The resurgence of the Pyramid Age “We need to get rid of the mom and pops . . . . Theyare inefficient and poorly run small businesses, and we can build on economies of scale and knowledge of how to succeed in business without even trying.” Such was the strategy of the captains of the real estate business who began tearinginto nursing homes in the 1980s and overnight built giant conglomerates that I call pyramids. These pyramids cost America its personal patient centered care and hollowed out the very core of quality for the elderly.It all started with a small-time owner of three nursing homes in California that grew into Beverly Enterprises, which onceowned 1,200 nursing homes across the nation and has since imploded due to substandard care and violation of the minimum standards of quality. This paved the way for the classic strategy of acquiring real estate with a business attached—in this case, servicing the growing needs of an aging society. Other operators followed, including Manor Care, Sun Health, Mariner, Kinder Care, National Health, Vencor, Hillhaven, Genesis, and Integrated Health Services. Now it is Avi and Kindred. Currently, 50 percent of the nursing home industry is owned by chain operators having 20 or more homes. Another 25 percent is owned by religious-based groups who own three or more (Good Samaritan, for example, at one time owned over 200). What that means is that 75 percent of nursing homes have absentee ownership.What is absentee ownership? It is the organizational structure in which the money decisions are made off-site. The on-siteadministrator has all the responsibility, limited authority on operational policies, and no authority on money issues.With the emergence of Obama Care giving rise to moreenforcement and money driven care consolidation is inevitable as enterprise fights the dominance of the Monopsony. It didn’t work in the 1980s and is destined to fail again. If allowed to evolve it will destroy competition as a Monopoly does.

But the consumption of the health-care dollar is predominately not paid for by the consumer. The middlemen, so to speak, make the forces of free enterprise moot. Consumers are bystanders in the relationship between purchase and quality because they do not directly pay forthe product.This phenomenon is called monopsony. The consumer is not the buyer. But the buyer is almost singly dominant. This is the reverse of monopoly, where the seller has the last say. This is not Microsoft at work. It is the federal and state Medicare and Medicaid programs that buy upward of seventy five percent of all health-care service products. Under the Obama nation, government will be the purchaser of last resort for all Americans.Ingeniously, the insurance industry convinced the federal government in 1966 that the only fair and equitable manner to fund and pay for health care for the elderly and disabled was to pay reimbursable costs including a markup for overhead and return on equity. Most of the providers, in those days, were not-for-profit hospitals and sole-practitioner doctors. Thismethod lasted until 1989, when it was changed, in terms of payment, for hospital care and the emerging continuum of services for the elderly population.

    REMEDY ELDERCIDE AND RESTORE ELDERPRIDEWhy we Must Fix HealthCare Funding! Currently health care funding is a $4.2 trillion annual cost (add $9 trillion dollars with the pandemic and another $5 trillion for baby boomer chronic illnesses), political football. Proposals from both parties are suspect since they aren’t based on performance or outcome. Medicare for All, the Public Option, Obama Care, private insurance do not deal with prevention as a policy or have health preservation as an objective. It is treatment and medication driven. It is estimated that the prescription drug business in America accounts for $600 billion to $900 billion per year (growing at the rate of 25% per year (just look at the dirge of pharmaceutical commercials pitching pills not prevention) in contributing to an overdosing society.  (With supply lines controlled by outsourcing to China, USA was at the mercy of our own failure to foresee such an event). Ordinarily, the dollar appreciates in times of global stress. Our research develops a framework to understand why the dollar has emerged as a global safe-haven and anchor currency and how this role might change under a nationalist trade policy. Our analysis yields two key insights: First, the United States enjoys several financial privileges stemming from the dollar’s status as the global safe-haven currency. Second, the dollar’s safe-haven property, in turn, depends on the link between the US economy and the rest of the world through free trade. A trade war that isolates the United States from global markets severs this link. Our framework identifies a critical threshold: If average tariffs and retaliations exceed roughly 26 percent for an extended period, the euro could become the world’s new primary anchor currency.
