Sensory Diets & Fragile X Syndrome (Bev Kadish)
Presented by Bev Kadish, occupational therapist, from WriteStart OT. Discussion of sensory diets as a personalized plan of sensory activities designed to help individuals better regulate their sensory systems and improve focus and attention. It is a structured approach to providing the right amount and type of sensory input throughout the day. Individuals with Fragile X syndrome often have difficulties with sensory processing and regulation, and sensory diets may assist with helping the child or adult remain calm, focused and organised. The webinar outlines: * how to create sensory diet * an example of a proactive sensory diet * activating and calming movement activities * deep pressure input/proprioception * heavy work activities * maintaining regulation (fidget and focus) * vibration * oral input *breathing cards * auditory system * visual input *olfactory input *withdrawal area * calming sensory room or space * community based activities * list of sensory resources Bev is an experience OT who has supported many children and adults living with Fragile X syndrome, through her own private practice and the Fragile X Alliance Clinic in Melbourne. Many thanks to the Qantas Side by Side Foundation grant for supporting this webinar.

ASD and FXS (Dr Marcia Braden)
Where an individual has a co-diagnosis of Fragile X syndrome (FXS) and Autism Spectrum Disorder (ASD) it’s essential to ensure that effective and targetted strategies are put in place. Dr Marcia Braden says “While there are some similarities in behaviours associated with the two conditions, it's important that allied health, disability support teams, educators and parents clearly understand the differences between FXS and ASD. It's often assumed that the diagnosis of ASD drives programming of supports for a person with a dual diagnosis, but the anecdotal evidence doesn't support that." In this webinar Dr Braden compared and contrasted learning style, social interaction and behavioural strategies taking into account FXS and ASD.

Cannabidiol and Fragile X syndrome (Dr Helen (Honey) Heussler MD)
r Honey Heussler describes the scientific rationale underpinning the RECONNECT cannabidiol (CBD) FX clinical trial currently underway in Adelaide, Brisbane, Melbourne & Sydney. The RECONNECT study is looking to understand whether the (Zynerba) ZYGEL CBD transdermal gel product helps in some behavioural issues associated with Fragile X syndrome. Dr Honey Heussler outlined the working of the human endocannabinoid system, the impact of Fragile X syndrome on that system, and how cannabidiol (non-THC CBD) can provide regulation and control. Dr Heussler also outlined the operation and findings of the earlier clinical trial phases which have preceded Zynerba’s current RECONNECT study. The RECONNECT study remains open in Australia until 31 March 2024. Individuals with a diagnosis of Fragile X syndrome and who are aged 3-22 years old and have behavioural symptoms may be eligible to participate in the study. Communicating And Connecting part 1 (with Bev Sher)
Bev Sher discusses the topic of communicating and connecting, focusing on highly verbal versus less verbal individuals. She explains that speech refers to the clarity and precision of speaking, whereas language is about what is being said and its social usage. She highlights that individuals with Fragile X syndrome face speech and language difficulties due to motor delays, cognitive delays, sensory motor integration difficulties, social anxiety, and motor planning difficulties. The pathologist emphasizes the importance of multidisciplinary collaboration between speech language pathologists and occupational therapists to address these difficulties. She suggests that a strength-based approach should be employed, tailoring interventions to suit the learning style of the individual, and managing anxiety to improve fluency. The pathologist offers various strategies for supporting comprehension in Fragile X syndrome, such as slowing down speech rate and using visual aids.

Educational Strategies (with Dr Marcia Braden)
Dr. Marsha Braden discusses educational strategies for those with Fragile X Syndrome, which is caused by a lack of the Fragile X protein that plays a role in brain development and regulates other genes and proteins. The lack of the protein leads to overactive and improper neuronal connections, affecting learning in various areas of the brain. Individuals with Fragile X Syndrome may exhibit hyper extendable joints, low muscle tone, and other physical features. Strategies that use visual supports, such as pictures and schedules, and accommodations for anxiety and social interaction, can be effective for learning. However, direct instruction and fast-paced methods may not work well with this population without adaptation. Overall, targeted instruction and accommodations can help individuals with Fragile X Syndrome achieve success in learning.

She presents two case studies of men with Fragile X, one of whom lives in a group home and the other with his mother, and discusses the difficulties they face in becoming independent. The speaker emphasizes the need for structure and practical strategies to support independence, and acknowledges the emotional challenges faced by parents and caregivers of individuals with Fragile X, including planning for their long-term care.

Cognitive Profile (with Dr Marcia Braden)
Dr. Marsha Braden discusses the cognitive profile of individuals with Fragile X Syndrome. There are strengths in vocabulary, long-term memory for meaningful information, and face emotion recognition. However, weaknesses include inattentional control, linguistic processing, visual spatial cognition, and visual motor impairments. Biological and environmental factors affect outcomes, and the simultaneous processing strength should be utilized for teaching whole word reading. Visual cues and associative learning are effective, but focus and concentration deficits, executive functioning deficits, and inconsistent reaction to stimuli are prevalent. Females may mask learning difficulties, and math is difficult for both genders due to its sequential nature. Incidental learning is common, and understanding the cognitive profile can help individuals with Fragile X Syndrome be more productive and enjoy learning.

When to Consider a Diagnosis of Fragile X (Dr Jonathan Cohen MD)
When should a GP consider a diagnosis of Fragile X, and why is a diagnosis important?Dr Jonathan Cohen from the Fragile X Alliance clinic discusses developmental delay and other symptoms which warrant referral for a DNA test for Fragile X, and the role of family history in a diagnosis.Dr Cohen notes that GPs play a very important role in managing the health of families who are affected by Fragile X.New expanded carrier screening offers women the option of being screened for a large number of genetic conditions including Fragile X, before pregnancy or in the early stages of pregnancy. With thanks to ThinkGP (GPCE) for making the short film for Fragile X Association of Australia to raise awareness of Fragile X.
