An ordinary near miss is not sentinel merely because serious harm was possible. Life-sustaining rescue is one qualifying severe-harm example, not the entire definition. (2) In the audio: A never event is a much narrower category that sits inside the broader bucket of sentinel events. — Correct: September 2026 policy clarification: The 2025 NQF update, published May 2026, has 28 SREs in four categories. Joint Commission adopts these plus three workforce events January 1, 2027. Medication-error SREs can include serious temporary harm; recovery does not exclude them. CMS selected HAC policy limits additional IPPS payment for selected conditions not present on admission, not the entire bill; IRFs are exempt. Payment and reporting rules must be checked separately.
