🩺 Klinik: chronisch vs. akutChronische Anämie (oft unspezifisch):- Müdigkeit, Leistungsabfall, Blässe, Schwindel, Konzentrationsstörungen- bei schwerer: Dyspnoe, Tachykardie, Palpitationen, Angina-ähnlich- geriatrisch: Stürze, funktioneller Abbau, „depressive“ SymptomatikAkute Blutungsanämie (potenziell dramatisch):- Hypotonie, Tachykardie, Kaltschweißigkeit, Synkope, Schock- Cave: Hb/Hkt initial noch normal direkt nach akutem Blutverlust🕵️ Anamnese & Status: worauf achten?- Blutungsquellen: Menorrhagie, Teerstuhl/Hämatochezie, Hämaturie, OP, okkult GI- Chronische Erkrankungen: Entzündung, Infektion, Tumor, Niere- Ernährung/Malabsorption: vegan, bariatrisch, Zöliakie, Alkohol- Medikamente: Antikoagulanzien/ASS, MTX, Hydroxyurea, Zidovudin, PPI, Chemo- Familien-/Herkunftsanamnese: Thalassämie, hereditäre Hämolysen- Untersuchung: Blässe, Ikterus, Lymphknoten, Hepatosplenomegalie, neuro. Zeichen, Herzinsuffizienzzeichen🧪 Blutbild-Logik (MCV/MCH + Retikulozyten)- Mikrozytär/hypochrom → oft Hb-Syntheseproblem (v.a. Eisenmangel/Thalassämie/ACD)- Normozytär → oft ACD, Niere, Blutung (breite DD)- Makrozytär → v.a. B12/Folat, Alkohol, Leber, MDS, Medikamente🧲 Mikrozytär: Eisenmangel vs. ACD vs. ThalassämieEisenmangel:- Ferritin ↓ (aber Akute-Phase!), immer mit CRP interpretieren- typischerweise: Transferrinsättigung ↓, Transferrin ↑, sTfR ↑, Retikulozyten-Hb ↓- Ursachensuche Pflicht:- prämenopausal: häufig Menorrhagie- Männer/postmenopausal: GI-Blutung bis zum Beweis des Gegenteils (ggf. Gastro-/Koloskopie)Therapie Eisen:- oral meist 1. Wahl (z.B. ~100 mg elementares Eisen täglich oder jeden 2. Tag; Verträglichkeit beachten)- i.v. bei Unverträglichkeit, Malabsorption, CED, hohem Bedarf, fortgeschrittener CKD, Bedarf an rascher AuffüllungACD:- Entzündung/Hepcidin, verminderte Eisenverfügbarkeit, gedämpfte EPO-Antwort- häufig: Ferritin normal/↑, CRP ↑, Transferrinsättigung ↓, Transferrin normal/↓- Therapie: Grunderkrankung behandeln, Mischbilder häufigThalassämie:- ausgeprägte Mikrozytose bei oft moderater Anämie, Ferritin meist normal, Erythrozytenzahl relativ hoch- Diagnose: Hb-Analyse; Eisen nur bei gesichertem Mangel🧯 Normozytär: Blutung/Hämolyse vs. Hypoproliferation- Retikulozyten hoch → Hämolyse-/Blutungsabklärung- Hämolyse: LDH ↑, indirektes Bilirubin ↑, Haptoglobin ↓, ggf. Coombs, Ausstrich- Retikulozyten nicht erhöht → ACD, renale Anämie, endokrin, frühes Eisenmangelstadium, KM-Erkrankungen/Medikamente- Basislabs oft: Nierenwerte, CRP, Ferritin, Leberwerte, TSH, B12/Folat🧠 Makrozytär: B12/Folat & Co.DD: Vitamin-B12-Mangel, Folatmangel, Alkohol, Leber, Medikamente, MDS, Retikulozytose- B12-Mangel: hämatologisch Makrozytose ± Bi-/Panzytopenie; neurologisch (Parästhesien, Gangstörung etc.) teils irreversibel- Diagnostik: B12, Holo-TC, ggf. Methylmalonsäure; Homocystein ↑- Folatmangel: Folsäure ↓, Homocystein ↑, Methylmalonsäure normal- Therapie: B12 je nach Ursache oral hochdosiert oder i.m. (z.B. Hydroxocobalamin 1000 µg initial wiederholt, dann alle 2–3 Monate); Folat oft 5 mg/d über MonateSicherheitsregel: Bei unklarer Makrozytose keine Folsäure ohne B12-Ausschluss.🚩 Red Flags (rasch/weiter abklären, ggf. Hämatologie)- Blasten im Blut- Bi- oder Panzytopenie- ausgeprägte Leukozytose/Thrombozytose- B-Symptomatik- Hepatosplenomegalie/Lymphadenopathie- neurologische Symptome bei Makrozytose- schwere/rasch progrediente oder unplausible Anämie🩸 Transfusion: pragmatisch- Entscheidung nach Klinik, Dynamik, Komorbidität, nicht nur Hb- häufig restriktiv: stabil oft ~7 g/dl; bei KHK/Symptomen eher ~8 g/dl (orientierend)- nach jeder EK-Einheit re-evaluieren; immer kausal mitbehandeln (Eisen/B12/Folat/Blutungsquelle)✅ Take-home- Anämie ist meist Symptom → Ursache finden.- MCV/MCH + Retikulozyten strukturieren die Abklärung.- Mikrozytär: Eisenmangel/ACD/Thalassämie; Normozytär: Retikulozyten entscheiden; Makrozytär: B12 vor Folat.
