大多數人小時候都得過水痘，此水痘病毒永久藏在脊髓或腦神經的背根神經節，伺機待發。當年紀漸長或身心疲累，對此病毒的免疫力降低，病毒趁機活化，從神經節沿著感覺神經往外傳至末稍神經，造成神經痛，最後抵達皮膚，出現紅疹、水疱或膿疱，而且通常只出現於單側，俗稱「皮蛇」。
