You must switch mechanisms and initiate gabapentin. — Correct: Failure of a treatment is not a diagnostic test. Antihistamine nonresponse does not establish that the remaining itch is neuropathic. Postburn itch commonly runs on a histaminergic and a neuropathic generator at once, and dryness, scar tension and wound irritation will defeat an antihistamine on their own. Reassess the skin and the scar, keep moisturizers, cooling, scar massage and pressure garments running, and add gabapentin when the itch carries neuropathic features such as burning, tingling, allodynia, or itch confined to a healed graft or donor site.
