Novartis AG (NVS) Q2 2026 — Novartis (NVS), the ~$285B Swiss pharma giant, reported Q2 2026 before the open on July 21: net sales of $14.4B rose +1% in constant currency (+3% USD) — a RETURN TO GROWTH after Q1's -5% cc decline — and core operating income of $5.94B beat the ~$5.3B consensus at a 41.2% margin. Core EPS of $2.41 beat the $2.17 estimate. But reported (IFRS) EPS fell -17% to $1.71, net income dropped -19% on higher taxes and interest, and free cash flow slipped -12% to $5.6B as net debt DOUBLED to $39.4B on M&A (Avidity, Myricx). Entresto fell -50% to $1.18B as US generics launched, while Kisqali (+43%), Kesimpta (+32%), Scemblix (+89%), Pluvicto (+43%) and Leqvio (+59%) more than filled the hole. FY2026 guidance reaffirmed (net sales grow LSD, core op income decline LSD). The ADR fell ~2.7% to ~$149.56 on the print. Novartis (NVS) is one of the two or three largest drugmakers on earth — a ~$285B Swiss pure-play innovative-medicines company (Aa3/AA-) led by Vas Narasimhan. Q2 2026 was, on the surface, a strong quarter: net sales of $14.4B returned to growth (+1% cc, +3% USD), core operating income of $5.94B beat the ~$5.3B Street estimate at a superb 41.2% margin, core EPS of $2.41 beat $2.17 — and the crown-jewel drug Kisqali won a surprise 6-month FDA pediatric exclusivity. The growth engine (Kisqali +43%, Kesimpta +32%, Scemblix +89%, Pluvicto +43%, Leqvio +59%, Fabhalta +88%) more than absorbed the Entresto patent cliff (-50% to $1.18B on US generics; EU LOE in November). Yet the ADR FELL ~3% on the print. Why? Underneath the polished 'core' numbers, reported IFRS EPS fell -17% to $1.71, net income dropped -19%, and free cash flow slipped -12% — because net debt DOUBLED from $21.9B to $39.4B in six months funding a $15.3B M&A spree. Management reaffirmed a trough-year guide (core operating income to DECLINE low-single-digits in 2026), and both Goldman and Jefferies say the premium multiple now hinges on second-half pipeline readouts. Our owner-earnings DCF (base ~$15.5B FCF, +2%/+5% growth, 7-8-9% discount, less $39.4B net debt, ~1.91B shares) centers near $128 vs the ~$150 price — about 15% below. Our call: HOLD, 3/5. Quality is not the question; the price is. We ALIGN with the Street's Hold (avg target ~$155, ~+4%) but are a shade more cautious. We'd own this fortress cheaper — under ~$120 — not here.
