Sebagian konsumen narkoba cenderung bermasalah mengendalikan perilakunya yang berhubungan dengan komoditas itu. Mereka terlibat persoalan dengan keluarga, teman, sekolah atau pekerjaan, bahkan hukum akibat konsumsi narkobanya. Sebagian lagi terus mengonsumsi narkoba meski berbagai persoalan tadi telah menimpa mereka. Orang-orang seperti itu didiagnosis mengidap gangguan konsumsi zat (substance use disorder). Meski statistiknya kecil, diperkirakan 13,18 persen dari seluruh konsumsi narkoba ilegal sejagat pada 2018, mereka yang bermasalah ini masih memiliki orang-orang yang peduli akan kehidupannya. Keluarga atau teman pengidap gangguan ini tetap “membantu” mereka, misalnya secara finansial walau tahu uangnya akan digunakan untuk apa. Di negara-negara Barat, kecuali yang kaya raya dengan asuransi kesehatan yang bisa digunakan untuk perawatan ketergantungan zat, anggota keluarga yang ketagihan narkoba biasanya diusir dari rumah. Banyak dari mereka yang menggelandang karena sudah tidak ada lagi teman atau kerabat yang percaya dan sudi rumahnya mereka tumpangi. Mengemis di jalanan dengan penampilan kumal lazim mereka lakukan.
