Welcome back to Rad Onc Smart Review: Thoracic Oncology Edition. Episode 15 is a planning-focused guide to definitive radiation for stage III non-small cell lung cancer. We move from simulation and motion assessment to gross disease, nodal volumes, target margins, technique, image guidance, adaptive triggers, and plan appraisal. The core challenge is not simply achieving coverage. It is building a defensible plan that reflects the actual disease map while controlling heart, lung, esophageal, spinal-cord, and treatment-time risks. We also discuss when anatomy or response has changed enough that the original plan should no longer be treated as valid.
