The U.S. Food and Drug Administration identified slivered onions as the likely source, prompting the removal of Quarter Pounders from menus. The outbreak affected at least 75 individuals across 13 states and resulted in one death and multiple hospitalizations. McDonald's confirmed Taylor Farms supplied the onions from a facility in Colorado Springs, Colorado, which has since stopped sourcing from that facility. The Colorado Department of Agriculture confirmed beef patties were not linked to the outbreak, as E. coli tests returned negative. Taylor Farms has recalled yellow onions from the Colorado facility and is working with health officials on the investigation. Other fast-food chains have also taken precautionary measures related to onion supplies. Colorado has reported the highest number of illnesses linked to the outbreak, with many cases associated with travel.
