Jillian’s connection to the mission was shaped by her own family’s experiences with cancer—within a single year, her mother, father, and stepfather each faced the disease.     Back to Basics: What You Need to Know About a Death Event at Home

    This week’s episode features hospice nurse Shelley Henry in an in-depth discussion about what should happen when a death occurs at home.  Many hospice patients chose to die at home, in familiar surroundings with their family members.  It’s important that patients, caregivers, and end of life workers understand the things that happen at the time of a death. Here are some highlights to know: • A hospice team member might not be there when your Person dies.  • Hospice agencies don’t provide ongoing bedside services when a person is dying unless there are symptoms that need to be managed. • The hospice team supports the family when there’s a death, notifying authorities as required, and providing grief support for those at the bedside. • If the death occurs inside a facility like an assisted living facility or a nursing home, the hospice team will coordinate with the facility staff.  • Post-mortem care (respectful, gentle cleaning of a body after death) is often provided. • Narcotics should be destroyed in the home by the hospice nurse according to federal law. • Care at the time of death should never be hurried.  Family members and caregivers should feel supported and never rushed.  Get more information about hospice and connect with hospice nurse Shelley Henry at The Amity Group.     Back to Basics: What to Know About General Inpatient Care

    We're getting back to the basics of hospice care, taking a deep dive into hospice General Inpatient Care. Patients (and providers!) need to know the what/when/where of the GIP level of care inside hospice care. Here are some highlights of this episode: • General Inpatient Care is used for crisis symptom management that can't be done in the patient's home. • GIP is provided in a Medicare-certified facility (so there's 24/7 nursing care). • Medicare does not consider actively dying a crisis. • Every Medicare-certified hospice agency is required to furnish GIP care when it's needed. • Hospice team members (nurse, social worker, chaplain) continue to see the patient while they’re in GIP care.     We’re deep diving into advance care planning with hospice Social Worker Rosie Gaston, talking about diagnosis specific advance directives.  With diseases like Alzheimer’s and other types of dementia on the rise, the need to plan for end of life situations for neurodegenerative diagnoses is increasing.  It’s hard to predict  every scenario, but considering basic care needs and wants is important.  Some of the things that should be considered include where the person wants to live out their life, what treatments they want/don’t want, who’ll be the primary caregiver, and how care will be paid for.  A dementia specific directive might include directions about treating infections like UTI’s and pneumonia, and choices about feeding, feeding tubes, and VSED options.  Even though it’s not possible to think of every situation that might happen, it’s important to have conversations about needs, wants, and wishes for end of life.     How to Cope With Grief That No One Understands

    International End of Life Doula Association is Making Dying Better

    The International End of Life Doula Association believes everyone deserves compassionate, respectful care at the end of life.  Executive Director Doug Simpson is working with the INELDA team to make that happen.  Doug’s journey to EOL doula work began with the death of his own father, followed by the birth of his son.     How to Find Hope When the Burnout is Real

    Palliative physician Emma Jones knows the real effects of burnout, and now she’s coaching others to rise from the ashes. While working in a high-pressure pediatric palliative care job, she found herself suffering the fallout of severe burnout. Emma was told to go home and figure out what was going on, but she realized there was no guide for how to do that. She began dealing with depression, trying to rebuild her self care. The care started with yoga, and grew to include healthy habits including daily movement, therapy, rest, and boundary setting.     Back to Basics: What You Need to Know About Live Discharges

    Back to Basics: What You Need to Know About Complaints

    We’re getting back to the basics of hospice care, sharing information about complaints, when to make them, and what should happen when you do. Hospice Social Worker Lisa Pahl (creator of The Death Deck and End of Life Deck) helps drive this discussion. Here are a few highlights from our discussion: •Patients and/or caregivers have a right to make a complaint.  •There are numerous reasons a complaint might be needed: visits aren’t being made, symptoms aren’t being managed, response time is slow, or the care is out of alignment with the beliefs or needs of the patient. •Care of a hospice patient should never be rough or insensitive - this is always a reason to make a complaint. •If delivery of medications or medical equipment (DME) is very late or doesn’t happen, a complaint should be made. •A complaint can be given directly to the case manager or director/administrator of the agency.  •The hospice agency should immediately investigate, talk to everyone involved, and work to resolve the issue to the satisfaction of the patient or caregiver. •If the hospice agency doesn’t resolve the complaint in a timely manner or to the point the patient/caregiver is satisfied, the complaint can be elevated to the state organization that provides licensure to hospice agencies. It might be the Health and Human Services Department for the state. •The hospice agency is required to furnish the patient/caregiver with contact information about making complaints, including the information for their accrediting organization.     Back to Basics: What You Need to Know About Transfers

    On-call, after hours, 24/7 services - no matter what you call it, hospice care is available to patients and caregivers every hour of every day. Regardless of the day of the week, whether it’s a holiday, or the office is closed, a member of the hospice team is accessible all the time. Some hospice agencies have answering services or triage teams, but the final result of a call is connection with a nurse. If another discipline is needed, that’s going to be coordinated by the agency. No question or concern is dumb. Hospice agencies offer this service to make sure patients and caregivers feel comfortable in their homes while experiencing the end of a life.  Here are some of the reasons a caregiver or patient might want to contact the hospice team when the office is closed.
