Complex Patients Need Simple Interventions
What if the answer for your most complex patient...was actually simpler than you think? In this episode, we share a favorite clinical principle and one that we train on at PelvicSanity...The more complex the patient, the simpler the intervention.Because when someone has seen multiple providers, tried countless treatments, and feels completely overwhelmed by their symptoms...The last thing they usually need is a more complicated plan.We discuss:✅ Why newer clinicians shouldn't be shielded from complex patients✅ The foundational interventions most clinicians overlook✅ How to choose treatment priorities with your patient ✅ Metaphors that help patients understand why you're starting simple ✅ Why patience is required—for both clinician and patient ✅ Why no patient is too complex to helpIf you've ever felt intimidated by a long history, a thick intake packet, or a patient who's "tried everything"...This episode is for you. Birth Prep Isn't Birth Education: An Interview with Gina Conley of Mamaste Fit
Are we trying to do too much as pelvic rehab providers? That might be an uncomfortable question.Because when it comes to birth prep, many of us genuinely want to help our patients have the best experience possible. But where does our expertise end?And where does someone else's begin? In this episode, Nicole sits down with Gina Conley of MamasteFit—perinatal fitness provider, birth doula, educator, and someone who has attended hundreds of births—to talk about the role pelvic PTs actually play in pregnancy and birth preparation. This conversation challenges a lot of assumptions and asks us to reflect on how we're showing up for our pregnant patients. We dive into:👉 The difference between being a perinatal professional and a birth professional 👉 Why pelvic PTs bring huge value in birth preparation—but may not need to be teaching full childbirth education classes 👉 The difference between rehabilitation exercises and true fitness programming 👉 Why "never push on your back" and "never hold your breath" may be oversimplifications 👉 Birth plans vs birth preferences 👉 Why collaboration across professions leads to better care 👉 How our own experiences and biases can unintentionally shape how we treat patientsOne of the biggest takeaways:Birth prep isn't about controlling outcomes. It's about helping patients feel informed, adaptable, confident, and supported. And the more we collaborate instead of compete…the better care we're going to provide.About Gina ConleyGina Conley is co-founder of MamasteFit, a prenatal and postpartum fitness company that blends perinatal fitness, birth preparation, postpartum recovery, and professional education. How many of your patients might have endometriosis… and no one has said it out loud yet?Because that’s happening. A lot.In this episode, Nicole sits down with one of the only physicians in the country trained in both endometriosis excision surgery and reproductive endocrinology—Dr. Sadikah Behbehani. And this conversation pulls back the curtain on what’s really happening with endo, fertility, and patient care.We cover: 👉 Why endometriosis is still massively underdiagnosed (and often missed—even in fertility clinics) 👉 The difference between suspecting endo vs actually confirming it 👉 Why surgery is still the gold standard—and when it matters most 👉 The role pelvic PT plays (before AND after surgery) 👉 What REIs are often missing—and why 👉 How endometriosis impacts fertility, miscarriage risk, and pregnancy outcomes 👉 When conservative care is helpful… and when it’s delaying necessary treatmentBecause as pelvic rehab providers…👉 We’re often the first ones to connect the dots 👉 The first ones to say the word “endometriosis” 👉 And the first chance patients have to get the right careAbout Dr. Sadikah BehbehaniDr. Sadikah Behbehani is a double board-certified OB/GYN and reproductive endocrinology and infertility specialist with additional fellowship training in minimally invasive gynecologic surgery. She is one of only a handful of physicians in the U.S. with expertise in both fertility treatment and advanced endometriosis excision surgery. She practices at Endometriosis & Fertility Center, where she focuses on complex endometriosis care, fertility preservation, and helping patients build their families.👉 Website: https://endometriosis-fertility.com/ 👉 Instagram: https://www.instagram.com/dr.sadikahbehbehani/Downtraining MasterclassIf you want to be able to better help your pelvic pain patients, you need to be working on the nervous system. Revenue vs Profit - Where you Need to Be
Revenue is vanity. Profit is reality. And most business owners are focused on the wrong number. In this episode, we break down one of the biggest mindset shifts in business:👉 Revenue is how much money your business brings in👉 Profit is how much actually ends up in your pocketAnd the difference between those two numbers determines everything about your life. This conversation started after a high-level business conference in Vegas where someone asked:“How do you improve profitability in an insurance-based business?”The answer?“If you can’t control your pricing… there’s only one game to play: you have to ruthlessly cut costs.”And honestly? That’s not a business most of us want to run.We break down: Why profit margin is the metric serious business owners actually care about The huge mistake most people make when trying to improve profitability Why you cannot save your way to a successful business The difference between finite expense cuts and uncapped revenue growth How small increases in patient volume massively outperform tiny cost savings Why business model determines lifestyle more than almost anything else And then we get into the contrast that changes everything:🏥 Insurance-based model ~10% margins ~42 visits/day to make $100k💰 Cash-based model ~66% margins ~3 visits/day to make $100kSame income.Completely different life. This episode is about more than money. It’s about: 👉 Burnout 👉 Freedom 👉 Control 👉 And building a business that actually works for your lifeIf you're ready to build a business with stronger systems, better margins, and more freedom…The Business Accelerator is where we help pelvic rehab business owners grow intentionally—without burning themselves out.👉 Learn more here: www.pelvicptrising.com/acceleratorAbout Nicole & Jesse CozeanPelvic health is hard. What Trauma-Informed Care Really Means: An Interview with Krystyna Holland
Are you truly practicing trauma-informed care… or just assuming you are?Because there’s a big difference. And most of us were never actually trained in it.In this episode, Nicole sits down with pelvic PT, researcher, and educator Krystyna Holland to break down what trauma-informed care really looks like—and where our field is still falling short. And this conversation goes deeper than you expect. We’re not just talking about being empathetic or “good with patients.”We’re talking about: 👉 The intentional choices that either build safety… or unintentionally cause harm 👉 Why “I’m a caring provider” isn’t the same as being trauma-informed 👉 The missing clinical frameworks that should have been part of our training 👉 How even well-meaning clinicians can get this wrongKrystyna shares insights from her current research—including the development of actual, tangible guidelines for trauma-informed care in pelvic health. Not theory. Not buzzwords.Real, actionable practices. We also get into: Why ongoing consent matters (and how to actually do it) How intake forms can unintentionally create harm What to say when a patient discloses trauma The biggest mistakes clinicians make when trying to “protect” patients Why trauma-informed care applies to every interaction—not just internal exams The surprising gap between what patients want… and what providers think matters This episode will likely challenge how you think about your care. And that’s the point.About Krystyna HollandKrystyna Holland is a pelvic health physical therapist, trauma-informed care researcher, author, and founder of Inclusive Care LLC. Beyond the Obvious: An Interview with Anna Hartman
Are you actually treating the right thing… or just what’s obvious?Because those are very different. And if you’re missing the real driver of your patient’s symptoms…No amount of exercises, techniques, or experience is going to fix it.In this episode, Nicole sits down with athletic trainer and educator Anna Hartman to dive into a completely different way of thinking about assessment, treatment, and what it really means to be an effective clinician.Anna brings a unique blend of: Orthopedic rehab Osteopathic and visceral approaches High-level work with professional athletes and Navy SEAL candidates And the result is a perspective that challenges a lot of what we’ve been taught. We cover: Why assessment—not treatment—is the most important skill you have The difference between treating symptoms vs finding the true driver Why “results in 1–3 visits” isn’t about shortcuts—it’s about precision How ego gets in the way of clinical progress (more than you think) What it actually means to “trust what you feel” as a clinician Why repetition—not information—is the key to real skill development This conversation will push you.It might even make you a little uncomfortable. But if you’re willing to question your assumptions and level up your clinical thinking…There’s a lot here.Anna HartmanA huge 'thank you' to Anna for being on the podcast and sharing her wisdom with all of us. Is Your Problem Supply or Demand?
Is supply or demand the bigger problem in your business?Because if you don’t know the answer to that…You’re probably working on the wrong thing. We see this all the time. Business owners are busy, putting in effort, trying to grow……but focusing on an area that isn’t actually the bottleneck. In this episode, we simplify everything down to one core concept: supply vs demand.👉 Supply = how many patient visits you can actually provide 👉 Demand = how many people want to be on your scheduleSounds basic. But most people never apply it to their own business. We walk through: How to identify whether you're supply or demand limited Why you’ll always be limited on one side (and why that’s normal) What to do if you’re supply limited (efficiency, adding time, hiring, or raising prices) What to do if you’re demand limited (leads, conversion, or clinical buy-in) How focusing on the wrong side wastes time, energy, and money If your business feels scattered or stuck…This is the lens that brings clarity.Business Accelerator ProgramIf you’re ready to stop guessing and start focusing on what actually moves your business forward…The Business Accelerator is where we help you identify your biggest constraint—and fix it.No more spinning your wheels on things that don’t matter.👉 Learn more and apply here: www.pelvicptrising.com/acceleratorAbout Nicole & Jesse CozeanPelvic health is hard. Prolapse is one of the most talked-about conditions in pelvic health right now.And also one of the most misunderstood. In this episode, we take you behind the scenes of the PelviCon Prolapse Symposium — why we created it, how it’s different, and why this topic needed a deeper dive.Because the truth is:Most of us were taught a one-size-fits-all approach to prolapse. And it doesn’t work.Prolapse is one of the fastest-evolving areas in pelvic rehab, with more research, more perspectives, and more complexity than ever before. That’s exactly why we built this symposium differently.Instead of staying within our silo…We brought together:Pelvic PTs and OTs Surgeons Researchers Mental health professionals And experts from around the worldTo answer one question:👉 What does it actually mean to treat prolapse well in 2026?In this episode, we talk about:The origin story of the symposium Why prolapse was the obvious choice What most clinicians are still getting wrong And what you’ll actually learn from attendingIf you’ve ever felt like prolapse treatment is more complicated than it “should” be…You’re right.Prolapse SymposiumJoin us for the 2026 PelviCon Prolapse Symposium here: www.pelvicon.com/symposiumAbout UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Why Is Prolapse So Hard to Treat?
Prolapse should be simple… right?Strengthen the pelvic floor, support the organs, and move on.Except that’s not what actually happens. In this episode, we break down why prolapse is one of the most complex conditions we treat — and why so many clinicians feel stuck. We cover:Why prolapse is often taught incorrectly from the beginningThe disconnect between symptoms and anatomical findings Why you can’t “Kegel your way out” of structural defectsHow the nervous system, identity, and fear all play a roleThe impact of hormones, birth trauma, and connective tissueAnd why prolapse is really a multi-system condition, not just a pelvic floor issueThis isn’t about doing more. It’s about understanding the full picture — and knowing when pelvic PT is just one piece of the solution.Want to Go Deeper? 7 Reasons Nicole Never Wanted to Start a Business
Starting your own practice gets talked about a lot.But what doesn’t get talked about?👉 The reasons not to do it.In this episode, we break down the 7 reasons Nicole never wanted to start her own business — and why, for a long time, staying an employee was actually the better choice. We cover:What it looks like when a job gives you all the good (and shields you from the bad) The uncertainty and complexity of starting from scratch The role of loyalty — and when it matters Career growth without ownership Financial risk, student loans, and lack of safety net The appeal of a predictable schedule And the very real fear of failureThis is a nuanced conversation about business ownership — not everyone should do it, and that’s okay. But it’s also about what changes when those reasons start to fall apart. The Bigger TakeawayIf you’re thinking about starting a business…It needs to be for something bigger than just frustration with your current job.And if you’re a business owner…Your job is to create a place where it’s genuinely hard for great people to leave.Ready to Start Your Own Practice? If you’re serious about starting your own business the right way…👉 Check out the Business Kickstart Program (step-by-step guide): www.pelvicptrising.com/kickstartAbout UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Do You Dread Certain Patients? Let's Fix That
Ever looked at your schedule and thought…“Ugh… that patient.”You’re not alone.Dreading certain patients is something almost every pelvic PT or OT experiences — but it’s not something we talk about enough. In this episode, we break down:The subtle signs you might be dreading certain patients (even if you don’t realize it)Why it happens — from tricky diagnoses to personality challenges to specific stages of careAnd what to actually do about itBecause it’s not about being a better “perfect” clinician. It’s about building the confidence to handle whatever walks through your door. The truth is:👉 You’re never going to have it all figured out.And the sooner you accept that……the more freedom (and enjoyment) you’ll have in treating your patients.Resources & Upcoming EventsPelviCon Prolapse Symposium Waitlist: www.pelvicon.com/SymposiumNicole’s Free Core Assessment Training (April 16): www.pelvicptrising.com/abdominalAbout UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Stop Chasing Letters - Certificates in Pelvic Health
Will getting another certification actually make you a better clinician?Or just give you more letters after your name?In this episode, we tackle one of the most common questions we hear — which certifications should I get?Because in pelvic health, there are a lot of options. And a lot of confusion. We break down what certifications really are… and what they’re not.At the end of the day, a certification is simply someone saying: “You’re certified.”That’s it.We also talk about the uncomfortable truth — that many certifications are primarily a marketing tool. They’re designed to package courses together, increase completion, and yes… sell more education. That doesn’t make them bad.But it does mean we need to think about them differently. We also dive into:• Why certifications don’t bring patients through the door• Why they won’t fix imposter syndrome• The difference between learning for growth vs. chasing letters • Why even the most recognized certifications don’t guarantee better clinical skills • How to actually choose the right con-ed for your careerAt the end of the day, the question isn’t:“What certification should I get?”It’s:“Would I take this if there were no letters attached?”About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Offer Benefits To Appeal and Retain Your Team
Most smaller practice owners think they need to be “bigger” before they can offer real benefits. But that’s not actually true.In this episode, we break down what providing benefits as an employer really looks like — especially for small, growing pelvic health practices.Because benefits aren’t just about checking boxes. They’re about attracting the right people… and keeping them.We talk about the biggest misconception owners have — that benefits need to be expensive or complicated to matter. They don’t.In fact, some of the most meaningful benefits have very little to do with money. We also dive into:• What employees actually value (hint: it’s not always what you think) • Why benefits are really a retention strategy, not just a hiring tool • How to think about flexibility, growth, and mentorship as benefits • Why you don’t need to offer everything — just a few things done really well • How your benefits reflect what you truly value as a businessAt the end of the day, your benefits send a signal. To your team.To future hires. And to the kind of business you’re building. If you’re an owner trying to grow your team — this episode will help you think about benefits in a completely different way.Free Training - Should I Be Running Ads?Join us for our quarterly free training all about Google, Instagram and Facebook Ads. Advocate for Yourself and Stop Accepting These 3 Things in Our Field
Why do so many pelvic rehab clinicians love treating patients… but feel frustrated with their jobs?In this episode we dig into the results from our State of Pelvic Rehab Survey and talk about three things every pelvic PT and OT employee should be advocating for in their workplace. The data was eye-opening.67% of clinicians are doing unpaid documentation. That means many rehab providers are literally working for free so their employer can be reimbursed by insurance. We also found that 79% of clinicians don’t have a regular clinical mentor, and 74% receive no paid mentorship time in their job.At the same time, most clinicians also report very small continuing education budgets. You can’t grow without support — which means you should have either strong mentorship or a meaningful con-ed budget. And then there’s raises. The average raise reported in our survey was just 1.8%.Nearly 81% of clinicians received raises of 3% or less, which means many pelvic rehab providers are effectively taking a pay cut every year after inflation. In this episode we break down three things every employee deserves — and give scripts you can use to advocate for yourself professionally and respectfully.Pick one.Start the conversation. And see what kind of workplace you’re really in.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. The Power of Your Circle: How Lauryn Beecher Built Her Practice
Some practices grow into something incredible.Others burn the owner out along the way. What makes the difference? In this episode we sit down with Lauryn Beecher, owner of Pelvic Health and Wellness, to talk about the real journey of building a pelvic health practice that actually aligns with your values.Lauryn didn’t start with a big clinic. She started by renting a treatment room two days a week, juggling multiple jobs and slowly building something of her own.Today she runs a thriving multi-clinician practice serving one of the most diverse pelvic health populations we see anywhere — men, women, kids, fertility patients, and the trans community. But the real secret behind what she’s built?Intentionality.Lauryn shares why she’s incredibly intentional about the people around her — her team, referral partners, mentors, and even the patients she works with.Because the people in your circle shape your business far more than most owners realize. We also dive into:• Why certifications alone don’t bring patients in the door • The myth of the “magic number” before starting a practice • How adaptability becomes a business superpower • The overlooked skill every business owner should developYou can learn more about Lauryn and her practice at www.pelvichealthandwellness.com Instagram: @pelvichealthandwellnessBusiness Accelerator ProgramIf you're a business owner looking to grow your practice, we are currently accepting applications for the Pelvic PT Rising Business Accelerator Program. We've helped 900+ business owners build practices aligned with their mission.Learn more at: www.pelvicptrising.com/acceleratorAbout UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. 6 Surprising Results from Our State of Pelvic Rehab Survey
We recently wrapped up our State of Pelvic Rehab Survey, with hundreds of responses from pelvic therapists working as employees. And a few things stood out.First, the paradox.Despite a number of structural challenges in these jobs, most pelvic therapists report being pretty happy. Nearly 70% rated their job satisfaction a 4 or 5 out of 5.We (generally) love the work. But when you dig into the details of how these jobs are structured… the numbers get interesting. In this episode, we walk through six of the most surprising findings from the survey, including:Why mentorship still hasn’t improved in the past five years (and why nearly 80% of clinicians still report having none)Why most pelvic therapists are not treating anywhere close to 40 hours per week anymoreHow documentation is still largely unpaid — with 63% of clinicians reporting they aren’t fully compensated for itWhy raises across the field remain extremely small, with 80% of clinicians receiving 3% or less last yearWhy continuing education budgets are surprisingly low — with nearly 60% of clinicians receiving less than $1,000 per year, despite the lack of in-house mentorshipAnd what clinicians say is actually driving burnout in pelvic rehab (spoiler: it’s mostly administrative)The survey paints a fascinating picture of the field right now.Experienced clinicians.Complex patients.A specialty people genuinely love. But also a lot of clinics still figuring out how to structure these jobs in a way that actually supports the clinicians doing the work.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. 5 Financial Advantages Every Business Owner Should Be Using
Most pelvic rehab business owners work incredibly hard… and then accidentally leave money sitting on the table. In this episode, we walk through five financial advantages that smart business owners use to build stability, reduce taxes, and create long-term wealth — without doing anything risky or complicated. This isn’t about fancy investing strategies or crypto speculation. It’s about boring, high-leverage fundamentals that add up dramatically over time.💰 1. High-Yield Savings Accounts (Stop Giving Away 3%)If you’re letting large amounts of money sit in your business or personal checking account, you’re losing out.We break down:Why your checking account should only hold operational cashHow high-yield savings accounts can earn 3–5% instead of ~0%Why we use Ally (and other strong options)How to create “buckets” for taxes, vacations, payroll buffer, and big expensesWhy there is no meaningful difference between business and personal finance at this level🏥 2. Health Savings Accounts (HSA)If you’re eligible for an HSA, you should understand how powerful it is.We discuss:2026 contribution limitsWhy HSAs allow tax-free healthcare spendingHow to use it as a financial bufferWhy this is not just a retirement accountHow business owners can strategically use high-deductible plans🏦 3. Retirement Accounts (IRAs, SEP, SIMPLE, 401k)We give a clear breakdown of:IRA vs SEP IRA vs SIMPLE IRA vs Solo 401kContribution limitsWho each option is best forHow business structure affects your optionsWhy many owners underutilize these dramaticallyIf you’re not deliberately funding retirement as a business owner, you’re missing one of the biggest long-term advantages of entrepreneurship.🏠 4. Business Use of Home DeductionIn today’s world, nearly every business owner qualifies. We cover:What qualifies as a dedicated spaceHow to calculate itWhy this can be a meaningful annual tax breakCommon misconceptions that prevent people from using it🧾 5. S-Corp Election (When It Makes Sense)If your profit exceeds ~$75,000 per year, you should at least explore this.We explain:How S-Corp status reduces payroll taxWhy this is one of the biggest structural tax advantagesWhen it makes sense (and when it doesn’t)Why you need a good CPA involved🚀 The Bigger ThemeBusiness ownership gives you more than income. It gives you financial tools that W-2 employees don’t have. But simply owning an LLC doesn’t automatically create freedom. You have to use the advantages available to you. These five strategies:Increase your returnLower your taxesImprove cash flowBuild long-term wealthCreate true financial confidenceIf you’re building a business, you should be building wealth alongside it.Get in Touch!Learn more at www.pelvicptrising.com, follow Nicole @nicolecozeandpt (www.instagram.com/nicolecozeandpt) or reach out via email ([email protected]).Check out our Clinical Courses, Business Resources and learn more about us at Pelvic PT Rising...Let's Continue to Rise!

The '6-8 Visit' Lie
There’s a unspoken soundtrack running in pelvic rehab:“Patients should be better in 6–8 visits.”Most of us don’t say it out loud. But it shapes how we plan care, how we discharge patients, and how we judge ourselves. In this episode, we break down:Where this belief actually comes from (hint: insurance, not biology)Why it doesn’t hold up under scrutinyHow it quietly contributes to burnoutAnd how it impacts patient outcomesWe ask three important questions:Is it true?Is it helpful? Is it kind? When you look at how other specialties practice.....or how long research studies are conducted for....or tissue healing timelines, motor learning, scar remodeling, behavior change, and nervous system regulation… none of them peak at visit 6. The soundtrack isn't true. And it's certainly not helpful. Not for your patients. Not for yourself with the extra pressure it creates. And not for our field.Lastly, it certainly isn't kind. You would never tell another therapist they aren't any good because they couldn't get a complex patient better in 6-8 visits - yet we're telling ourselves that all the time.So we need to retire this broken soundtrack (as Jon Acuff would say).We also give you three ways to check whether you’ve actually internalized this — or whether the soundtrack is still running the show:Review your last 50 plans of careNotice how you feel at visit 4 if someone isn’t dramatically betterPay attention to your response when someone asks, “How long until I get better?”This episode kicks off a deeper exploration of time in pelvic rehab — because patient outcomes aren’t just about skill. They’re about skill × time. And if we artificially limit the time we spend, we cap the results for our patients.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Stop Looking for the "Magic Bullet" in Business
If there were a magic bullet in business, we’d all be using it.And we’d all be successful. But there isn’t.As pelvic rehab providers, we get frustrated when patients come in looking for the one magic pill, surgery, technique, or device that will instantly fix everything. We know better. We know outcomes are about consistency, skill, time, and effort. And then we turn around and do the exact same thing in our businesses. In this episode, we talk honestly about the ways practice owners distract themselves chasing shortcuts instead of doing the work that actually grows a business. We break down common “magic bullet” traps:Facebook or Instagram ads will fix everythingSelling a course will create passive incomeTelehealth will magically expand your reachBuying expensive equipment (Shockwave, RTUS, Emsella, red light therapy) will generate leadsRenting space from a physician who will “definitely” refer patientsGetting another certification that patients aren’t asking aboutNone of these are inherently bad.But none of them are magic. At the end of the day, business success is a simple formula:Clear direction × Hard workYou have to know where you’re going. You have to know what actually matters. And you have to be willing to put in the work.If you do that, you’ll build something meaningful. If you keep chasing shortcuts, you’ll stay stuck. This episode is tough love — but it’s the kind that moves you forward.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Are Cash-Based or Insurance-Based Providers "Better"?
There’s been a recent flare-up in the pelvic rehab world around cash-based versus insurance-based care. And while emotions are running high, we wanted to pause the noise and add something that feels increasingly rare: context. In this episode, we start with what we all agree on:Every pelvic PT and OT is doing their best for their patientsWe all want clinicians to be paid fairly and sustainablyMore practice models = more options for both clinicians and patientsPatients benefit from having real choicesWe should be referring to one another, not competing against the field itselfThe current medical system requires both insurance and cash-based care — in every country we’ve seenFrom there, we address the two biggest areas of disagreement head-on.First, the claim that cash-based practices reduce access to care. We walk through the data around deductibles, waitlists, and real-world barriers to care — and why more options actually increase access, not reduce it.Second, the idea that one model inherently produces better care. We’re very clear here: you can get excellent or poor care in any setting. But when you take the same caliber clinician, the environment matters — and we explain why time, autonomy, documentation burden, and feedback loops change outcomes. This episode isn’t about picking sides. It’s about understanding incentives. And it’s about recognizing that the growth of pelvic rehab depends on all of us moving forward together.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Patient Outcomes = Skill x Time
In this episode, we break patient outcomes down into a simple but powerful formula:Patient Outcomes = Skill × TimeYou need:The skill to understand what’s actually driving someone’s symptomsAnd the time for healing, behavior change, tissue adaptation, relationship-building, and clinical reasoning to unfoldMiss either one — and outcomes suffer. We talk about:Why outcomes can’t be separated from both clinician skill and available timeHow testing your hypotheses requires both technical ability and space to observe changeWhy healing, strength, and behavior change don’t happen on insurance-driven timelinesHow different practice models dramatically affect the “time” variableThis conversation was sparked by recent rhetoric criticizing cash-based care — and instead of taking the bait, we zoomed out and asked a better question:What actually creates great outcomes? This episode sets the foundation for future discussions about systems, incentives, and why clinicians often feel frustrated despite doing their best work.🎧 If you care about results — for patients and clinicians — this one matters.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. How We Get Paid - Why Clinicians & Clinic Owners are Getting Squeezed by Insurance Companies
In this episode, we break down how money actually moves in the insurance-based PT and OT world, why so many clinicians and clinic owners feel financially stuck, and how the landscape has quietly but dramatically changed over the last 20 years. We cover:How insurance companies make money — and why they’re incentivized to pay out as little as possibleThe reality of declining reimbursement and rising administrative costsWhy clinic owners feel squeezed from both sidesWhy clinicians deserve raises — and why insurance doesn’t support that realityHow this system unintentionally pushes clinics toward lower-quality care modelsMost importantly, we talk about why the rise of cash-based care isn’t a trend — it’s a correction.Cash-based models:Allow small, independent clinics to survive and thrive againCreate real earning upside for skilled cliniciansRestore transparency and accountability in careExist in every healthcare system — including countries with universal healthcareThis episode isn’t about villainizing anyone. It’s about understanding the system we’re operating in — so we can make better, more informed decisions about our careers, our clinics, and our patients.🎧 Whether you’re an employee, a practice owner, or somewhere in between, this is required context for the modern pelvic health landscape.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. What to Do With a Long Waitlist
Long waitlists are incredibly common in pelvic rehab — and they’re only becoming more common as awareness of pelvic health grows. But a long waitlist isn’t automatically a good thing… or a bad thing. It’s a decision point. In this episode, we break down what it actually means to have a long waitlist and how to think about it strategically and tactically, whether you own a practice or work for someone else.We discuss:Why waitlists longer than ~3 weeks lead to significant patient drop-offThe three strategic paths when demand exceeds capacity:Providing gold-standard care to fewer peopleSeeing more people with lower intensity careExpanding capacity through hiring or program growthWhy a waitlist alone is not a reason to hireHow solopreneurs and small practices should think about waitlists differentlyTactical best practices to:Improve fill ratesReduce cancellationsUse your waitlist efficiently without burning out your teamIf you’ve ever felt stressed, guilty, or stuck because your waitlist keeps growing — this episode will help you make intentional decisions instead of reactive ones.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Goal Setting for 2026 — Diversify Your Portfolio
Most goal-setting focuses on one or two areas of life — usually work and maybe health — and then we wonder why things feel off balance by February. In this first episode of 2026, we’re sharing the framework we teach every year in our business programs, but one that applies just as much if you work for someone else:Diversify your goal-setting portfolio. Before you set career or business goals, we walk through other areas that matter just as much — and often get overlooked:RelationshipsFun and enjoymentPersonal financesHealth and wellnessCareer or businessWe share real examples from our own lives — what we’re prioritizing, what we’re not prioritizing, and why sustainable goals matter more than aggressive ones that collapse by mid-January.This episode is about creating goals that actually support the life you want — not just another list of things to chase.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Crafting Your 2025 Narrative: Fighting Negativity Bias & Giving Yourself Credit
As the year wraps up, it’s tempting to jump straight into planning what’s next — without ever stopping to reflect on what just happened. In this episode, we talk about why crafting a clear narrative for your year is one of the most powerful (and overlooked) tools for preventing burnout, building perspective, and entering the next season grounded instead of reactive. We explore:Why high performers default to focusing on what didn’t go wellHow negativity bias distorts your view of the yearThe clinical parallel between helping patients make sense of their story — and doing the same for yourselfWhy even experienced CEOs struggle to give themselves creditOur one-sentence narrative for 2025, shaped by becoming a family of four while expanding our impact in pelvic healthHow reflection creates psychological closure and protects against burnoutWe’ll walk you through a simple exercise to help you name your own one-sentence narrative for the year — before you start planning the next one.If you’re feeling tired, behind, or like the year slipped by too fast, this episode is your invitation to pause, zoom out, and recognize what you’ve actually built.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. How a 0-10 Scale Will Transform Your Communication
Binary questions are easy — and often misleading. In this episode, we break down one of the most powerful (and underused) communication tools in pelvic health, leadership, and life: the 0–10 scale.Instead of asking questions that are heavily influenced by power dynamics, assumptions, or people-pleasing (“Are you okay with this?”), the 0–10 scale opens the door to nuance, curiosity, and honesty. We discuss:Why yes/no questions shut conversations downHow a 0–10 scale creates psychological safety and intellectual humilityHow defining the ends of the scale matters just as much as the numberWhy the scale naturally leads to better follow-up questionsHow this tool keeps conversations collaborative instead of adversarialWe shares real-world examples of using the scale:Gaining consent for internal vaginal or rectal treatmentAsking about comfort with students or shadowingClarifying goals like returning to running or biking (and whether those are real goals)Interviewing clinicians and understanding true interest in patient populationsNavigating employee requests without jumping straight into actionThis simple shift turns communication into interaction — and helps you gather better information, build trust faster, and avoid unnecessary conflict.🎓 48-Hour Clinical Course Sale To celebrate this episode, we’re running a 48-hour flash sale on all Pelvic PT Rising clinical courses.Use code RISING26Lifetime access to all coursesThis is the only sale like this we’ve done all yearIf you’ve been thinking about taking one of Nicole’s clinical courses, this is the time.👉 Learn more and grab your course at www.pelvicptrising.com/clinicalAbout UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. From Surviving to Thriving in Business as a Pelvic PT/OT Owner
A lot of pelvic health business owners are just surviving. They’re working hard. Seeing patients. Paying the rent and bills, with maybe a bit left over for themselves. But surviving isn’t why most people started a practice. In this episode, we talk about what it actually means to thrive as a business owner—and why the success you want is often on the other side of the work you’ve been avoiding. We walk through the five core areas we see in truly thriving practices:Financial confidenceLifestyle and time freedomClinical excellenceDaily enjoymentCommunity impactWe also discuss why willpower alone rarely gets people there, why grinding harder often backfires, and how getting the right support can completely change the trajectory of your business. If you’ve ever felt stuck between “it’s fine” and “this isn’t what I imagined,” this episode will give you clarity—and a framework to think differently about what’s next.👉 Want to see where you fall right now? The 4 Types of Clinical Mentorship (And Why Most Clinicians Are Missing At Least One)
Mentorship has become a buzzword in pelvic PT and OT — but most conversations around it are incomplete. In this episode, we break down the four distinct types of mentorship that actually shape clinical confidence, patient outcomes, and long-term career satisfaction. Most clinicians receive some mentorship — but very few receive all four.We unpack:Hard Skills Hands-on techniques, assessments, and interventions — what most people think of as “mentorship,” and what many con-ed courses focus on.Soft Skills Communication, motivation, managing difficult personalities, and helping patients truly buy into care. Often overlooked, but frequently the difference between average and exceptional outcomes.Information Understanding diagnoses, testing, medical management, and knowing what actually matters for the patient in front of you — not just what’s theoretically interesting.Professional Development Career guidance, burnout prevention, time management, and mentorship that considers you as a person — not just a clinician. We also talk honestly about what to do if you’re not getting this level of mentorship where you work — and how to approach your career strategically instead of hoping things improve on their own.🔍 Looking for a clinic that prioritizes mentorship? We’ve built a Hiring & Mentorship Map featuring pelvic PT/OT practices — many within the Pelvic PT Rising community — that value training, growth, and long-term career development.👉 Explore the map: www.pelvicptrising.com/mapWhether you’re early in your career or years in, this episode will help you identify what kind of mentorship you’re actually receiving — and what you may be missing.Business Accelerator ProgramAre you a business owner looking to take things to the next level in 2026? Stop Borrowing Your Patient's Story - Help Them Create a Better One
We dive into one of the most overlooked clinical skills in pelvic rehab: 👉 helping patients make sense of their own story.Patients often arrive with narratives that are fragmented, confusing, or shaped by years of mixed messages. When clinicians accidentally adopt that narrative, both the patient and the plan of care end up lost.We explore:Why so many patients lack a cohesive understanding of their conditionHow unclear stories lead to frustration, fear, and chasing answersOur role in helping patients build a clearer beginning–middle–future arcWhat it means to become the trusted “guide” in their journeyKey questions that help uncover what’s really keeping patients stuckThis episode will help you reframe the way you approach the patient story — and how to use it to improve buy-in, clarity, and outcomes.Business Owner Free Training - How to "Convince" Patients to Come to a Cash PracticeIf you're a business owner, don't miss our free training on Thursday (12/11) all about converting patients. Why 90% of Business Owners Don't Reach Their Financial Goals
🎙️ Why 90% of Pelvic Health Business Owners Aren’t Hitting Their Financial GoalsWe break down exactly why so many pelvic health business owners struggle financially — and what to do about it.This episode is based on data from our community and our experience working with more than 800+ pelvic rehab business owners. And the truth? Most owners aren’t missing their financial goals because 'people are afraid of spending money' or 'it just isn't working in my town or they’re “bad at business.”They’re missing them because the system they’re operating in makes financial success nearly impossible without addressing 5 key issues:1️⃣ Time — The #1 Business KillerWhether you’re a solopreneur drowning in admin or a multi-clinician owner still seeing too many patients, time constraints choke your revenue more than anything else.2️⃣ Pricing — Starting Too Low and Staying Too LowMost owners undercharge for years… sometimes forever. Your pricing has to support your life — not just cover rent.3️⃣ Marketing — “People Find Me on Google” Is Not a PlanIf you can’t tell us exactly where you’d go to find five new leads this week, you don’t have a marketing system. And without one? You can't control the growth of your business.4️⃣ Conversions — You Don’t Need More Leads. You Need to Convert the Ones You Have.If you’re not converting at least 70% of inquiries, you’re leaking money. This is one of the highest ROI skills you can fix.5️⃣ Consistency — The Most Underestimated Skill in BusinessLife will happen. Burnout will happen. Systems are what allow your business to survive when you can’t run at 100%.The Big Picture:A pelvic health business is a get-rich-slow scheme. It works — but only if you fix the system and run it consistently over time.🚀 Ready to Stop Guessing and Start Growing? The Business Accelerator opens soon! Six months. Weekly coaching. Pay-per-patient vs. guaranteed hours can mean a 2–3x difference in annual income.2️⃣ Optimizing for FlexibilityCan you adjust your start and end time? Number of days? Being off for school pickups? These are HUGE quality-of-life factors and often matter far more than an extra dollar per hour.3️⃣ Optimizing for Mentorship + Career GrowthThe reason many residencies pay peanuts is because mentorship is invaluable. Look for structured mentorship and a program that serves you. You’re not just taking a job — you’re building a career.4️⃣ Optimizing for Ethos + Patient Care ModelAsk yourself:Will I see multiple patients per hour or one-on-one?Does this match the clinician I want to be?Your fulfillment comes from doing work you believe in.🔍 Looking for great pelvic health positions?Use our free Hiring Map: www.pelvicptrising.com/map It’s full of vetted pelvic health practices actively hiring PTs and OTs.🔥 PelviCon Live Virtual + RecordingToday is the final day to get PelviCon 2026 Live Virtual + Recording for the lowest price! 👉 www.pelvicon.comAbout UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health.
