Ep 122: Before You Take On A GP Partner, Check These Four Things
Most Practice Owners don't decide they need a partner, then go looking for the right one.Someone asks to buy in.And suddenly the question becomes, “Should I say yes?”That's where things can go sideways.Because taking on a partner isn't just selling equity. You're giving someone access to risk, decision-making, profit, and the future value of the business. In this episode, Todd and Sachin share the four things they look at before bringing someone into ownership, why similarity is often overrated, and how to test a potential partner before you're locked in together. What you'll get from this episode:The four things you're really giving away when you sell equity in your PracticeWhy a partner who thinks exactly like you can be a liability, not an advantageHow to test disagreement, temperament and decision-making before you commitWhy selling equity for cash can be far more expensive than borrowing moneyWhat happens when one Owner drives the growth and everyone else shares the upsideResources MentionedWhy GP Partnerships Fall Apart (And How To Fix Them)How a Junior GP Chose the Right Practice to Buy Into.....Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 121: He Built a Practice That Doesn't Need Him
Only 20% of GPs would consider Practice Ownership today. It used to be 45%. The RACGP asked the ones with doubts why, and 80% said the same thing: they're not sure it's viable.Mark Sujecki has been a Practice Owner for 30 years. Two multi-doctor Practices in Mount Martha, ten doctors running through the main one at any time, and he still surfs, plays tennis, and volunteers overseas most years. He built the business so it can run without him in the room.Todd sits down with Mark to get into how he actually engineered that – what he handed over, what he held onto, and why the Owners who get their time back are usually the ones who stopped trying to run everything themselves. What You'll Get From This EpisodeWhy Mark bought in for control, not money, and what that means for choosing your own partnersThe two-clinics-one-culture model, and how it runs without micromanaging either siteHow Mark made himself less essential to the Practice without shrinking the businessThe team every new Owner needs before they buy in, and why "smarter than you" is the standardWhy 80% of GPs think ownership isn't for them, and the one question that changes their answer.....Mark Sujecki: Mark Sujecki is a GP and co-owner of two multi-doctor Practices in Mount Martha, Victoria, and has been a Practice Owner for over 30 years.Connect with Mark: LinkedInConnect with Todd: LinkedIn.....P.S. Ep 120: How GP Owners Should Navigate the Surcharge Ban
On 1 October 2026, surcharging card payments becomes illegal in Australia. The RBA says the change is about transparency and lower costs for consumers. The problem for GP Practices is that the underlying card fee doesn't disappear. Someone still has to pay it.And depending on how your money flows, that could be your Practice or your GPs.Todd recently taught this live to our Project X Members. One Owner thought the impact would be tiny. Once she ran the numbers, she realised it affected seven or eight of her GPs.In this episode, Todd and Sachin walk through what actually changes, where the cost can land, the maths behind the fee increases needed to break even, and how to handle the conversation with your team. What You'll Get From This EpisodeWhy removing the surcharge doesn't remove the fee, it just changes who pays itThe maths behind a 1.5% surcharge becoming a 4.3% fee rise for your Practice, or 2.3% for the GPThe hidden exposure many Owners haven't checked: GPs and tenants paying service fees or rent by cardWhy cash, bulk billing and direct bank transfers can create problems of their ownTodd's four rules for navigating the change with your team.....Free ResourceDownload The Surcharge Reset™ Playbook to work out what card collection is actually costing your Practice, compare the four ways to handle it, catch the edge cases most Owners will miss, and take the right conversation to your GPs before you make a change.Download the Playbook here: https://smc.expert/The-surcharge-reset-playbook-d.....Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 119: How Five GP Owners Make Decisions Without Getting Stuck
Dr Annelies Geissler joined Robina Town Medical Centre and EZT Medical Centre as a contractor GP in 2008.Sixteen years later, she's one of five equal Owners running both Gold Coast Practices, with around 25 GPs across the group.Five Owners brings more experience and more capacity, but it also means more strong opinions in the room. Annelies breaks down how they choose the right people for ownership, split responsibility across two sites, and keep things moving when agreement doesn't come easily. What You'll Get From This EpisodeWhat they look for before inviting a GP into ownership, and why clinical skill alone isn't enoughHow five equal Owners divide roles without losing shared accountabilityWhy separate communication channels matter when you're running two PracticesHow treating decisions as experiments helps strong personalities move forward without endless debateWhy they changed their fee structure to better recognise longer and more complex consults.....Connect with Todd: LinkedInConnect with Annelies Geissler: LinkedIn.....P.S. Ep 118: How a Junior GP Chose the Right Practice to Buy Into
When Dr Luke Ward started looking for a Practice, he wasn't only looking for somewhere to work.He was looking for somewhere worth buying into. That changed the questions he asked.Who owned it? How long had the team stayed? Was the Practice still being built from scratch, or had it already proved it could run well without chaos?Luke eventually joined Robina Town Medical Centre and EZT Medical Centre on the Gold Coast. Today, he's one of five equal Owners across the two Practices. He breaks down how he assessed the opportunity, prepared himself for ownership, and what junior GPs should look for before buying into a Practice. What You'll Get From This EpisodeThe questions Luke asked before choosing which Practice to joinWhy long-term staff retention told him more about culture than a polished pitch ever couldHow he prepared for ownership before anyone had formally offered him a shareWhat Practice Owners should look for before inviting the next GP into the ownership groupResources MentionedHow to Grow a Multi Million Dollar Property Portfolio by Michael YardneyRich Dad Poor Dad by Robert KiyosakiThe Barefoot Investor by Scott PapeThe Armchair Guide to Property Investing podcast with Ben Kingsley and Bryce HoldawayAustralian Institute of Company Directors course.....Connect with Todd: LinkedInLearn more about Luke Ward: Profile.....P.S. Ep 117: From Opera Singer to Practice Owner
Helen Barnett's path to Practice ownership was anything but conventional. After 17 years as a professional opera singer with Opera Australia, she returned to university in her late 30s, studied science, won the University Medal, and earned a place in medical school. But by then, she knew medicine wasn't the path she wanted.Preventative care was.Nutrition was.And that decision eventually led her to co-own Dubbo Health Hub, a multidisciplinary Practice in rural NSW, alongside GP Dr Heather Malham.Helen brings a rare perspective to this episode. She has been the contractor, the clinician, and the Owner, often at the same time. That means her view on allied health isn't theoretical. It comes from years of seeing where these partnerships work, where they fall apart, and what Practice Owners often miss. What you'll get from this episode:Why allied health works best when it's built into the patient journey, not bolted onto the sideWhat allied health practitioners are really looking for when they choose a Practice, starting with being treated as an equalWhy an allied health room can cost around a quarter of what it costs to run a GP room, and what that means for pricing it fairlyWhy values fit matters more than simply filling a gap in your service mixHow to find the right practitioner by starting with your patients and the referral patterns already inside your PracticeWhy "here's your room" isn't onboarding, and what weak onboarding quietly costs everyone.....Connect with Todd: LinkedInConnect with Helen Barnett: LinkedIn.....P.S. Ep 116: He Trained 60 GPs to Build Six Practices
Dr Tim Koh opened his first clinic just three years after qualifying as a VR GP.Not to build an empire. To teach.Nearly 20 years later, he's built six Practices, trained more than 60 registrars, and retained some team members for 25 years. In this episode, Tim shares how registrar training became a serious growth strategy for his group, and why many GP owners may be overlooking one of the best ways to build culture, strengthen recruitment, and grow a Practice for the long term. What You'll Get From This EpisodeWhy many GP owners avoid taking on registrars, and why Tim thinks they've got the maths backwardsHow he recruits for cultural fit, including what he tells every registrar candidate about how long he expects them to stayHow to position registrars with patients so training becomes a strength of the Practice, not something the community has to tolerateWhy the most valuable thing supervisors teach isn't clinical knowledge, but critical thinkingHow registrars can lift standards, challenge pessimism, and become part of a stronger long-term GP teamTim's advice for anyone considering Practice Ownership or supervision:"Be brave. Ep 115: From Nearly Closing to Out of Rooms
Reservoir Medical Group has been open since 1960. Four generations of patients. A fifth now starting. For 10 years the Medicare freeze made it hard to keep the doors open. Then COVID hit on top of it. When Sachin asked Lindsay Hyde what she was trying to solve when she first joined Project X, her answer was three words: "We were trying not to go under."She didn't. Six years after taking a leap of faith and moving from 9 consult rooms to 13, they're at capacity. A nurse recently couldn't find a free room to run a health assessment. Now Lindsay is in conversations with the shopping centre about knocking on the neighbours' door. What You'll Get From This EpisodeHow a four-person management team ended the era of good ideas dying quietly in the hallwayFive previous registrars returned as fellows -- the retention number that tells the whole story"If you don't do your business CPD your Practice might die" -- the reframe that cuts through every vocation argumentThe Roger Federer principle Lindsay borrowed from Todd and never put downHow the leap from 9 to 13 rooms worked, and why 12 months later they were nearly out of room again.....Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 114: How to Unlock a Practice Your Partners Won't Grow
Some multi-owner Practices have partners who are comfortable with what they've built and have no interest in what it could become. They show up, collect their share, and defer every decision to the practice manager. If you're the partner who can see exactly what the Practice could become while the others collect a cheque, this episode is about how you change that. This isn't just a stuck Practice. It's a wealth event no one is turning up to collect. What You'll Get From This EpisodeWhy going from 25% to 15% might be the best financial move you ever make as a passive senior partner, and why Todd says the math almost always works in your favourTodd's one obligation for passive senior partners who want the upside: exactly what they must commit to and exactly what they must stop doingThe Planet Saturn Practice: why a business with 20-plus years in the community has more upside than a greenfield start-up, and why the junior partner showing up is a gift to everyone at the tableWhy you should set a hard time limit on stuck-partner conversations, and what you put on the table when nothing has moved in six monthsThe doctor who sold her stake for $1, built a Practice worth over $3M from scratch, and says the best thing that came out of it had nothing to do with money.....Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 113: Your Crusty Accountant Is Killing Your Exit
An owner at the Practice Owners Conference thought their Practice was worth $100K. Their wife had already done the planning. After the sale, they would need to work nine more years to reach their retirement goals. When Sachin and Todd unpacked the structure, the Practice should have been worth $600K to $700K. The problem was not the medicine, and it was not even the business. It was a billing structure designed in the 1980s by an accountant who has probably since retired into the same situation. The associateship model makes one thing very clear: whoever built it was thinking about keeping 100% of their personal billings, not about what the thing would be worth when the time came to hand it on.What you'll get from this episode:Why the associateship structure is engineered to produce a Practice worth nothing at exitTodd's ready reckoner: average Practices trade at 3-4x on a 6-8% margin. Project X Practices run 10-15% margin and sell closer to $1.5M on the same billingsHow loss aversion keeps doctors trapped in a structure they know is wrongThe 18-month versus 3-year restructure roadmap and what determines which path you takeWhat to say to your fellow owners first, and why this conversation takes six to twelve sittings, not oneThe doctor who sold her stake for $1, walked away, and built a Practice worth over $3M...........Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 112: The Cost of Not Yet
Most Practice Owners have a decision they keep pushing to "later."It might be a structural change, a hard conversation, a team decision, or a piece of Owner work that never quite makes it to the top of the list. Not because it doesn't matter, but because the Practice is full, the team needs attention, and the next quiet window feels just around the corner.Sachin had one of those decisions in 2013: a simple trust structure question for his accountant about a Vanguard ETF investment. The form got put aside, ended up in the laundry, and didn't get picked up again until November 2020. That delay may have cost close to seven figures in compounding returns. The same pattern showed up again and again at the Practice Owners Conference in Sydney. Across 62 one-on-one conversations, Todd and Sachin kept hearing different versions of the same thing:"I'm not ready yet.""I've got a few things to fix first.""Once this is sorted, I'll get to it."That's the waiting game. And for Practice Owners, it can get expensive fast. What You'll Get From This EpisodeWhy waiting for the "right time" can quietly cost more than the decision itselfThe three reasons Practice Owners stay stuck in the waiting gameHow the "train as you play" principle helps Owners make progress inside real Practice conditionsWhy your team may be ready for growth before you feel ready to lead itHow one Practice valued at $3.2M exited for $10.3M in two years with the same team, same two sites, and no expansionWhat finally breaks the pattern and gets important Owner work moving.....Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 111: The Chaos Coordinator Badge
Alana was once gifted a "chaos coordinator" badge at her Clinic. She wore it. At the time, it felt like recognition. Looking back, she sees it differently.Because in General Practice, there is a culture of badges and mugs and shared trauma. The shit show supervisor. The captain of chaos. An identity built around how many fires you put out each day. That identity feels like belonging. But it can also become the trap that keeps Owners, Practice Managers, and teams stuck below the line. For Owners, this episode is a reminder that a Practice cannot scale if its leadership identity is built around rescuing, reacting, and absorbing every problem. In this episode, Todd sits down with Alana Flohr -- 13 years in General Practice, former Practice Manager of a large Tweed Coast Clinic -- to name the three behaviours that keep owners and PMs stuck in the drama, and the above-the-line alternatives that change everything. What you'll get from this episode:The three "below the line" roles -- victim, hero, and villain -- and how each shows up inside a real PracticeWhy the hero role is often the most seductive trap for Practice Managers and OwnersAlana's honest reflection on owning her own hero behaviour, including the chaos coordinator badge she once acceptedHow to coach your team through difficult conversations, even while you're still learning to have them yourselfThe "shut the door" system Alana used to protect one hour each afternoon for above-the-line thinking and improvement workTodd's recommended action for this week: share the episode with your PM, create a shared language, and agree to call it out when you see itResources mentioned:Dare to Lead by Brené Brown.....P.S. Ep 110: The Paying Tenant Trap: What to Charge for Spare Rooms
Most Practice Owners with a spare room will let someone in before they've worked out what the room is actually worth. The logic is familiar: a paying tenant is better than an empty room. In the short term, it feels true. But below a certain rate, a tenant starts costing you more than the room generates -- in admin time, in blocked space, in the leverage you've quietly handed away. Todd and Sachin call this the keep-open cost, and most Owners don't know where theirs sits.In this episode, they walk through how to calculate it, who actually belongs in a spare room and in what order, and the agreement mistakes that lock Owners into the wrong arrangement for years. What you'll get from this episode:The hierarchy of what belongs in a spare room -- from a GP first down through allied health, and why the order mattersWhy the allied health tenant who treats your space like a business beats the sessional who comes twice a weekSachin's locked-in mistake: the five-year sublease that handed over front desk services and left no leverageWhy Todd watched a $700-800k allied health operation return almost nothing -- and what that taught him about keep-open costThe cosmetic injector scenario: why it sounds like easy income and why it almost always ends badlyThree mistakes to avoid: room block, below-cost rates, and the agreement that locks you out of your own best use.....Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 108: The GP Helmet
Most GP Practice Owners don’t have a time problem. They have an identity problem. After 20 years as a doctor, the GP role becomes hard to step out of. Patients need you. Staff pull you back in. The work feels familiar, useful, and immediately rewarding.Owner work doesn’t feel like that. It’s slower. Messier. Less obvious. And because it doesn’t fit neatly between patients, it gets pushed to the side.In this episode, Todd and Sachin unpack why the GP “helmet” is so hard to take off, what it costs the Practice, and what needs to change if you want to actually lead your Practice. What you’ll get from this episode:The three roles every GP Owner carries: GP, shareholder, and ownerWhy the GP identity is the hardest one to step out ofThe martyrdom pattern: “Only I can do this,” and what it quietly costsWhy a “quick half-hour job” can become 20 hours over 12 weeksHow unfinished projects pile up when owner time isn’t protectedThe Whitney Houston test for imposter syndromeWhy reducing clinical time is the first structural move most Owners avoidResources mentioned:Feel the Fear and Do It Anyway by Susan Jeffers.....Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 107: The Competitor Living Rent Free In Your Head
Worried about the practice down the road?In this episode, Sachin and Todd unpack how competitor fear can push Practice Owners into rushed decisions, bad positioning, and scarcity thinking – and how to get back to playing your own game. You’ll hear two real stories: Todd’s bulk-billing competitor opening across his car park, and Sachin’s team nearly changing their billing model because a small practice opened 100 metres away. What you’ll get from this episode:How to tell the difference between a real competitor and a neighbouring practice. Why changing your model in response to someone else can damage your own positioning. Why ideas are not the thing to protect – execution is.The four reactions owners have when they feel threatened: leave, ask them to leave, hold back, or play full out.How to stop letting competitors live rent free in your head and focus on improving your own practice.Resources mentioned:Mindset by Carol DweckThe Infinite Game by Simon Sinek.....P.S. Ep 106: When Your Practice Manager Won't Budge
Most Practice Owners hit the same wall at some point. They ask their Practice Manager to do something. It doesn't happen. They ask again. Still nothing. And slowly, the frustration builds. The honest answer? Usually both sides are contributing.Todd and Sachin break down the three root causes behind a Practice Manager who won't perform:A lack of understanding (they don't actually know what you want)A lack of will (no amount of cash will fix this)A lack of capacity (the environment, not the person, is the problem)What you'll get from this episode:The three-layer diagnostic: why your PM might not be delivering and how to tell which problem you're actually dealing withWhy paying a bonus for poor performance is one of the costliest moves an Owner can make, and what to do insteadThe Eisenhower Matrix applied to your PM's week: how to identify what should be cut, delegated, or protectedA real case study: how two Owners turned an underperforming PM into an A-grader over 18 months without replacing themThe 30-minute weekly meeting that creates accountability and moves projects forward in six weeks or less…..Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 105: The Budget Just Raised the Price of Passivity.
The Federal Budget just dropped. Most of the coverage won’t matter much to GP Practice Owners.Four things do.Capital gains tax. Family trust distributions. Urgent care clinic funding. Commercial real estate.Todd and Sachin break down what changed, what didn’t, and what Owners should be thinking about now.The headline is simple. The structures that gave small business owners a tax edge are being squeezed. The only real response is to build something bigger. What you’ll get from this episode:The four Budget changes that actually matter for GP Practice Owners, without the political noiseWhat the CGT and family trust changes mean in plain English, and why your structure needs proper adviceWhy commercial real estate may now be a stronger long-term play for Practice OwnersWhy earning one-third more is the only honest response – and how $250K in added annual profit creates $2M+ in income and a higher exit multiple.....Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 104: The New Owners Playbook. What To Do In Your First 90 Days.
Settling on a Practice is one of the biggest moments in ownership. The temptation is to walk in and start making changes. New systems. New structure. Your stamp on everything. The owners who build the most value from acquisitions do the opposite. They slow down, listen, and diagnose before they prescribe. And they know that the order in which you do things in the first months shapes everything that comes after.Todd and Sachin walk through their complete first-steps playbook for new owners: what to prioritise, what to never skip, and how to set yourself up for a return that goes well beyond the purchase price. What you'll get from this episode:The three-lens diagnostic for any Practice you’ve just acquired: GP capacity, patient demand, or operational chaosWhy talking to everyone under the roof before you change anything is non-negotiableThe financial system tasks that need to reach 90–95% within your first four weeks, and why they can’t waitHow to run a tools audit and the one simple trick for finding out what’s actually being usedThe meeting cadence that keeps a newly acquired Practice from falling into chaosWhat a realistic bedding-in timeline looks like, and why slow is fast when it comes to people.....P.S. Ep 102: When More Clinics Makes You Less Money
Most Practice Owners dream of owning more clinics. More locations, more GPs, more revenue. Proof you've made it.The data tells a different story. If you own five clinics, roughly one is producing 80% of your results while the others drain your cash, your time, and your attention. That's not growth. That's a rescue operation you're funding. What you'll get from this episode:Why the first clinic almost always funds the others, and why that's a trapThe vanity metrics that signal a Practice is run on ego, not economicsTodd's banking wake-up call: what happens when the bank asks you to leave by tomorrowThe four questions Keith Cunningham uses before any major decision (most owners only ask one)Why moving your best people to fix failing clinics is one of the most expensive mistakes in expansionThe real numbers to track: cash profit, GP retention, patient retention, and unit economicsWhy opening a new site is sometimes just avoiding the hard work in the one you already have…..Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 101: The 3 Lies Holding Your Practice Back
Most Practice Owners know what needs to change. The recruitment process is broken. The fee structure hasn't moved in three years. The front desk culture is costing them patients every week. They know. And nothing happens. It's rarely about knowledge. It's about the gap between intention and action, and the specific things that widen it.Emotional bandwidth.Fear of upsetting the team. The pressure to get it perfect before moving. And delegation that goes nowhere because nobody closed the loop.Scale My Clinic's implementation coach Stevie Landgraf has seen both sides: Practices stuck in the same loop for years, and practices that turn a plan into action in a week. This is about what makes the difference.…..What you'll get from this episode:- Why "I'm too busy to implement" is a sign you need to act now, not later- The cost of not acting: how unresolved problems compound in time, money, and energy- Why paralysis by analysis hits hardest in high-performing practices, and what breaks it- The difference between messy delegation and clean delegation, and why one kills follow-through every time- How to get your reception team on board with fee changes (without a fight)- The money buckets exercise that shifts front desk mindset in a single conversation- One thing that separates practices that fly from those that stall: share the why, early and often.....Connect with Stevie: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 100: Why Multi-Owner Practices Get Stuck
Multi-owner Practices have some of the biggest untapped potential in General Practice. They also have some of the most common, avoidable problems. No one is leading. No one is accountable. Quarterly meetings that turn into complaint fests. And a Practice Manager left to figure it all out alone.Todd and Sachin have seen this play out across dozens of Practices – from two-owner partnerships to groups of nine. In this episode they break down why it happens, what it costs you, and exactly how to fix it.What you'll get from this episode:Why multi-owner groups default to passivity and what it does to profitability and cultureThe five hats every Practice owner needs to understand (and why mixing them up costs you)The "chosen one" vs "divide and conquer" approach – when to use eachWhy paying your leading owner fairly is the difference between a $2M and a $9M businessThree actions to take if you recognised your Practice in this episodeResources mentioned:Burnout, Betrayal, and the $1 Sale with Dr Nerida McDonald…..Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 99: Costs Are Rising. What Practice Owners Should Do Next.
Everything is getting more expensive. Rent, consumables, insurance, fuel, wages. And it's not slowing down.Interest rates are likely heading up. Patients are starting to pull back. Staff are feeling the pinch personally and some are quietly looking at their options. This is the episode for Practice Owners who want to get ahead of it rather than react when it's too late. Todd and Sachin walk through how the current environment is hitting owners, Practices, staff and patients, and what to do about each one. What you'll get from this episode:How rising costs, inflation and global uncertainty flow through every layer of your PracticeWhy now is actually the time to invest, not retreat, and what "strategically contrarian" means in practiceHow to have the conversation with your team before they come to you asking for moreThe whiffle: a simple framework for hearing what's really on your team's mindWhy extending hours and going deeper in your community beats cutting backResources mentioned:Episode 97: The Most Expensive Room In Your PracticeEpisode 43: The GP Pay Debate: Stop Playing Into Their Hands!…..Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 98: How To Fraud-Proof Your General Practice
It happens more than anyone wants to admit. And most Practice Owners wouldn't even know it was going on.Phantom billing at reception. Personal expenses paid through the business. Cash disappearing from the drawer. One person running everything with no oversight. Todd sits down with Kelly Chard, accountant and founder of GrowthMD, to walk through exactly where Practices are exposed and how to lock it down.Kelly has seen it all, from small-scale theft that built up over years to full-blown Medicare fraud discovered too late. This episode is the playbook for making sure it doesn't happen to you. What you'll get from this episode:The red flags that should make every Practice Owner nervousWhy having one person run everything is your biggest riskHow to separate duties so no single team member controls the full chainThe fortnightly finance meeting that takes 20 minutes and changes everythingWhat to do about cash, petty cash, and why Practices should go cashlessHow phishing emails and AI-generated invoices are targeting Practices right nowThe tool that replaces petty cash and credit cards with controlled digital spending…..Resources mentioned:Kelly's LinkedIn video: The 3 most important documents Practice Owners need to know.……Not sure where to start? Ep 97: The Most Expensive Room In Your Practice
It's not the waiting room. It's not your biggest consulting room. It's the one room most Practice Owners never look at financially. One owner audited theirs and found a $200,000 annual loss hiding in plain sight. Overstaffed nurses, cupboards full of expired stock, and zero revenue being generated from nurse-led services. Six months later, the loss was eliminated and they were on track for $100K profit from the same room. What you'll get from this episode:Which room is quietly draining your profits and why most owners never noticeHow losses of $100K–$200K a year build up without anyone realisingThe four moves to stem the bleeding: rostering, stock systems, waste tracking, and task delegationHow to turn nurse-led services into a revenue streamWhy one owner's team went from being a cost centre to saying "this has to be a revenue generating activity"The consumable charging models that work without patient pushbackConnect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 96: How To Recruit International GPs (And The Pitfalls To Avoid)
Most Practice Owners know they need more GPs. The problem is finding them, getting them through the system, and keeping them once they arrive.International recruitment is one of the fastest ways to solve a GP shortage, but the pathways are confusing, the timelines are longer than most owners expect, and the wrong approach can cost you months.Sachin sits down with Jane and Alex from WaveLength, one of Australia's leading GP recruitment agencies, to break down exactly how it works. What makes a Practice attractive to overseas doctors, which pathway gets them working fastest, and what good orientation actually looks like. What you'll get from this episode:Which Practices can recruit international GPs and which ones can'tWhy team size, culture, and lifestyle matter more than you think when attracting overseas doctorsThe two main pathways explained simply: expedited specialist vs college streamHow long it actually takes from first contact to a GP starting in your clinicWhat effective orientation looks like and how to stop losing doctors in their first yearConnect with Jane and Alex:Jane Stanke is a GP Talent Advisor with over 17 years at WaveLength International. Ep 95: How To Sell Your Practice For Life-Changing Money
Most GP Practice Owners sell too late and for too little. The timing's wrong. The valuation's inflated. The due diligence is skipped. And decades of hard work get handed over for a fraction of what it's worth. This episode covers the full exit conversation:What you'll get from this episode:Why most owners sell too late in life and what that actually costsThe "would I buy my own clinic?" question and why your answer mattersThe order of improvements that drive the biggest value upliftWhy profit margins in coached Practices sit at 10-15% versus 6-8% in the wildThe dumb tax: what happens when buyers skip due diligenceHow one Practice Owner turned a $3.2M valuation into a $10.3M sale in exactly two yearsResources mentioned: Die With Zero by Bill PerkinsConnect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. Ep 94: Why GP Partnerships Fall Apart (And How To Fix Them)
Most GP partnerships don't blow up overnight. They drift apart slowly. Decisions stall. Side conversations replace real ones. One partner wants growth, another wants to coast. And everyone pretends it's fine.Sachin sits down with Craig and Anna from Leadership Creativity to unpack what alignment actually looks like, what happens when it breaks down, and how to fix it before it's too late.Craig and Anna have worked with ownership groups for nearly a decade, inside and outside General Practice, and they've seen everything from partnerships that haven't spoken in 12 years to groups that aligned before merging and got it right from day one. What you'll get from this episode:What a healthy ownership group actually feels like versus a misaligned oneThe warning signs most partners ignore until it's too lateWhy decisions get made in meetings but never actually happenHow to have the reset conversation without blowing things upWhen to bring in external facilitation and why doing it yourself rarely worksThe one thing Craig and Anna would tell every Practice Owner to do this weekResources mentioned:Episode 50: From 2 GPs to 13 in a Decade; A Journey by Two Fabulous Female GP Owners with Dr Petrina Barson & Dr Arete KouterisEpisode 64: The Clinic Was Running Well – Then This Business Manager Raised the Standard for Everyone with Karlea WoottonEpisode 72: The GP Recruitment Crisis: How One Clinic Turned a 6-Week Waitlist into a Hiring Engine with Julie ThomsonConnect with Craig and Anna:Craig Morris is a Leadership Mentor and Consultant and co-founder of Leadership Creativity. How do you scale a clinic without burning out or breaking what made it special? In this episode, Dr Todd Cameron sits down with rural GP owner Dr Ben Nally for a deeply honest, practical conversation about what it really takes to scale a General Practice, especially when you’re doing it in a small, regional community with limited workforce, tight margins, and very public accountability. You’ll enjoy a grounded walkthrough of what happens when a clinic starts in the red, runs on goodwill instead of systems, and slowly — deliberately — transforms into a thriving, sustainable business that serves both patients and the people running it.Ben shares the uncomfortable early lessons, the small strategic wins that created momentum, and the leadership mindset shifts that allowed the clinic to grow from 2–3 FTE GPs to a team of 14+, without burning out the owners or losing the soul of the practice. This conversation speaks directly to owners who feel the weight of responsibility — to patients, staff, and community — but don’t want to sacrifice their health, family, or sanity to keep the doors open. It reinforces a core truth: sustainable growth doesn’t come from doing more yourself; it comes from better systems, better people, and clearer leadership.Stop reacting, start leading intentionally, and build a practice that actually supports your life, not consumes it.In this episode of the Scale My Clinic Podcast, you’ll learn:00:57 - The realities of running a practice in a rural community03:30 - GP Ownership as community leadership, not just business10:31 - Buying into a legacy practice (and knowing what to change and what not to break)13:16 - Mechanics of turning a struggling clinic around and how small wins create momentum16:44 - How intentional investment in people, leadership, and the physical clinic environment unlocks growth from 2.3 FTE GPs to 8.5+ FTEABOUT BEN NALLYDr Ben Nally, JCCA, MBBS, FRACGP is a Practice Owner, General Practitioner, Post Graduate Diploma in AnaestheticsWHERE TO FIND BEN NALLYWebsite: https://cgpmansfield.com.au/doctors-and-staff/#ben Website: https://cgpmansfield.com.au/ Missed the previous Episode?Listen to Episode 87 here: Scaling Clinics Without Losing Lifestyle: How Mary & Sean Did It.....P.S. Ep 86: From Transactional to Transformational: The GP Standard That No App, Pharmacy or Walk-In Clinic Can Replace
General Practice is under pressure like never before. Pharmacies, nurse-led clinics, telehealth platforms and government-funded walk-in centres are all positioning themselves as “GP alternatives” - and it’s leaving many Practice Owners anxious, frustrated, and questioning how they stay relevant and sustainable. In this episode, Dr Todd Cameron sits down with Dr Sachin Patel for a candid, at times uncomfortable, but deeply important conversation about standards in General Practice and why the future of GP clinics won’t be won through speed, volume, or transactional medicine.Instead, they unpack what actually makes General Practice indispensable: transformational care, deep relationships, strong professional standards, and leadership at the patient, practice, and community level. This is not just theory. It’s a call to lift the bar — clinically, culturally, and commercially — so your clinic becomes the place patients choose, teams want to work in, and communities trust. If you’re feeling pressure from competition, workforce challenges, or a system that seems stacked against General Practice, this conversation offers something rare: clarity and control. It reminds you that the future of your clinic isn’t decided by politicians or platforms — it’s shaped by the standards you choose to lead by.Listen in, reflect honestly, and ask yourself: Are we building a transactional clinic … or an indispensable one?In this episode of the Scale My Clinic Podcast, you’ll learn:00:00 - The threat of transactional medicine and why GPs are playing into it02:59 - Why transformational care is something no shortcut provider can replicate or replace12:05 - Professionalism inside the practice and why ownership starts with standards20:58 - Thinking beyond the consulting room and understanding the strategic advantage of genuine community leadership24:06 - Defining a 10/10 GP experience and raising the bar year after yearMissed the previous Episode?Listen to Episode 85 here: NFE: The Leadership Rule That Transforms the Owner–PM Relationship Overnight.....P.S. General practice is under pressure — and unless you act now, ‘free’ could cost you everything.”In this episode, Dr Sachin Patel unpacks the real story behind the November 1st Medicare changes — and what they mean for the future of your practice. With government funding lagging, rebates shrinking, and costs rising faster than ever, Clinic Owners across Australia are facing one question: How do I stay viable without burning out?Dr Sachin reveals why “free” is never truly free — and how bulk billing incentives, MyMedicare registration, and rebate reductions could quietly erode your clinic’s autonomy if you’re not strategic.
