Episode 63: A Whole-Body Approach to Perimenopause with Dr. Julie Sarton and Dr. Alexa Charbonneau

Dr. Mary interviews pelvic floor physical therapists Dr. Julie Sarton and Dr. Alexa Charbonneau from Sarton Pelvic Healing about common perimenopause/menopause issues they treat, including urinary leakage (stress and urge), dyspareunia (pain with sex), nocturia (peeing at night), constipation, and broader musculoskeletal problems like SI joint pain and frozen shoulder. We discuss how symptoms can reappear or become “unmasked” in midlife and emphasize that while hormones matter, one-size-fits-all HRT messaging misses crucial factors such as nervous system regulation, lymphatic and gut function, fascia changes (including estrogen receptors in fascia), old injuries, breathing mechanics, and visceral/organ mobility. The conversation highlights nuanced decision-making for cancer survivors and people with conditions like endometriosis, genetic detox differences, and mast cell activation (MCAS), stressing quality of life and individualized, team-based care. What you will learn in this episode: Perimenopause can start as early as 35 and last a decadeMany symptoms they've normalized (leakage, joint pain, painful sex, brain fog, peeing at night) are treatable, not inevitableHRT is not one-size-fits-all — especially for those with endometriosis, cancer history, or genetic variants like MTHFR or APOE4How the pelvic floor is connected to the feet, jaw, neck, diaphragm, tailbone, and even cranial bonesHow there are estrogen receptors exist in fascia which may be why old injuries resurface in perimenopauseConstipation can recirculate estrogen, making hormonal symptoms worseThe lymphatic system is deeply underaddressed and may have real implications for breast healthNervous system regulation is foundational for lasting healing00:00 Episode Overview02:00 Meet the Guests05:36 Common Menopause Symptoms08:12 Dr. Julie’s Personal Journey12:43 Beyond One Size Fits All15:56 Nuanced Hormone Decisions17:48 Detox Constipation MTHFR19:08 Cancer Care and Quality20:30 Mast Cell and Gut Clues23:06 Lymph Histamine Overlap24:01 Estrogen Swings Explained24:37 Pelvic PT Full Body Care25:53 How Fascia Line Connect the Head to Feet27:48 Estrogen Receptors In Fascia29:35 Detective Work Success Stories30:33 Diaphragm And Visceral Work32:25 Constipation And Tailbone Keys34:28 Glymphatics Lymph And Drainage37:09 Nervous System Lifestyle Reset39:13 Menopause Recalibration Closing42:13 Where To Find Pelvic Healing43:25 Podcast Outro And SupportYou can learn more about Dr. Julie and Dr. Alexa’s practice: Sarton Pelvic Healing in Irvine and Southern California here:https://www.pelvichealing.comYou can follow their instagram below:https://www.instagram.com/pelvichealing/Want support for perimenopause that goes beyond hormones?Whether you (or your clients) are taking HRT but still having symptoms, not ready for it, or can’t tolerate it...there’s more we can do.I’ll be sharing practical tools from a movement and rehab perspective, focusing on the nervous system, fascia, lymph, viscera, and musculoskeletal system. If you want updates on workshops, resources, and strategies to support this phase of life more fully, would love you to join my email list.Subscribe to my email list hereI’ll see you in 2 weeks!PS:If you are interested in being a guest on the show, please fill out the form here.

Episode 62: It’s Not Willpower: The Real Reasons We Struggle With Food with Gina Worful, RD

In this conversation, we unpack the hidden layers of our relationship with food—how it’s shaped by culture, childhood, trauma, biology, and the nervous system. Gina Worful, Founder of the Mastering Mindfulness Institute, is a registered dietitian who watched countless people struggle inside the conventional health model—white-knuckling their way through diets, blaming themselves when their body disagreed, and feeling more confused than empowered. She took a different approach: one that helps people reclaim their power with food, rebuild trust in their body, and understand the deeper patterns that I have seen often surface during perimenopause. What you will learn Why diets and willpower often fail and how shame keeps people stuck in cycles of control and “falling off the wagon.”The difference between blood sugar crashes and nervous system dysregulation, and how each uniquely drives cravings. How childhood experiences (like food scarcity, “clean your plate” rules, or cultural beauty ideals) can deeply shape eating patterns. The powerful role of self-trust, pleasure, and embodiment in healing our relationship with food.Practical tools for tuning into the body—like breath, awareness, and mindful eating practices—that bring regulation and freedom back to the table. Why dropping into the body isn’t always comfortable (especially with trauma) and how to gauge your capacity so you don’t overwhelm your system.Real-life stories, from mindful indulgence to scar tissue release, that show how body awareness can be both healing and empowering.Timestamps:00:00 Introduction to TMI Talk with Dr. Mary00:03 Understanding Perimenopause and Food Relationships00:40 Introducing Gina Worful and Her Approach00:56 Exploring the Layers of Food Relationships04:32 The Impact of Diet Culture and Shame05:25 The Role of Control and Willpower11:14 The Brazilian Perspective on Body Image21:54 The Science Behind Cravings and Stress33:34 Practical Tips for Managing Eating Habits37:41 External Reminders and Executive Function37:58 Emotional Eating and Childhood Wounds39:09 Scarcity Mindset and Food Habits43:04 Reconnecting with Your Body52:49 Understanding Capacity and Discomfort01:00:56 Mindful Eating and Pleasure01:07:23 Conclusion and Contact InformationYou can learn more about Gina below:https://www.masteringmindfulness.instituteinstagram.com/gina.worfulWant support for perimenopause that goes beyond hormones?Whether you (or your clients) are taking HRT but still having symptoms, not ready for it, or can’t tolerate it...there’s more we can do.I’ll be sharing practical tools from a movement and rehab perspective, focusing on the nervous system, fascia, lymph, viscera, and musculoskeletal system. If you want updates on workshops, resources, and strategies to support this phase of life more fully, would love you to join my email list.Subscribe to my email list hereI’ll see you in 2 weeks!PS:If you are interested in being a guest on the show, please fill out the form here.

For example, if you’ve had back pain for seven years and you go to a clinic or a healthcare professional, chances are they will ask you to have an X-ray, MRI, and some blood work. If they don’t find anything, you will continue to have symptoms and you will be dismissed. This is what I see in the clinic day in and day out when people come to me for help. With alternative treatments, we look into the mental health component and your lifestyle to hopefully help with your management for chronic pain and illness. I personally experienced going through chronic pain and illness that’s why I want to help you get through it the way that I did! Give this episode a listen to learn more about how physical therapy and other alternative treatments can help you! Let’s talk about:  0:35 – Introduction to today’s topic        0:54 – Some truth about western medicine 1:46 – The components that are not often looked into for people who have chronic pain and illness 2:41 – My own experience with going through chronic pain and illness and how it all started 4:40 – What I find in majority of people I see that have chronic pain and illness and the effects of being in a fight or flight mode all the time 7:01 – One of the biggest things that helped me with trauma that I went through growing up 8:38 – When did my first symptoms of chronic pain and illness appeared 9:24 – Gaslighting: What is it and how I was gaslit before 11:48 – How a psychotherapist helped me and how it can help you too 13:06 – Looking into the central nervous system: What happens when you’re in fight or flight mode, symptoms that you might be in that mode, why is it important to realize if you’re in this mode, and how you can get out of this mode 17:39 – Looking into your nutrition and lifestyle: How food and lifestyle affects your chronic pain and illness 22:21 – How does therapy actually help you: Breaking down the benefits 24:21 – Therapy and medication: When does medication usually come into play 27:30 – Looking into the people that you’re around with all the time, how they affect you, and how to bring back joy in your life 29:36 – The importance of understanding how you’re talking to yourself day in day out and understanding what triggers your chronic pain and illness 30:23 – Importance and benefits of tracking your period cycle 32:06 – How can you support yourself once you’ve identified your triggers 33:16– Why people with IBS tend to get triggered more 34:43 – How will the emotional diary and tracking help you in managing your symptoms 37:15 – Conclusion Want support for perimenopause that goes beyond hormones?Whether you (or your clients) are taking HRT but still having symptoms, not ready for it, or can’t tolerate it...there’s more we can do.I’ll be sharing practical tools from a movement and rehab perspective, focusing on the nervous system, fascia, lymph, viscera, and musculoskeletal system. If you want updates on workshops, resources, and strategies to support this phase of life more fully, would love you to join my email list.Subscribe to my email list hereI’ll see you in 2 weeks!PS:If you are interested in being a guest on the show, please fill out the form here.
