The episode also considers how kidney function and social determinants of health—including access to food, safe housing, walkable environments, and chronic stress—can influence cardiovascular risk. Next, we compare aortic stenosis and mitral stenosis. Aortic stenosis creates resistance as the left ventricle ejects blood through a narrowed valve, producing a harsh systolic murmur and potentially leading to left ventricular hypertrophy, reduced cardiac output, dyspnea, angina, and syncope. Mitral stenosis interferes with ventricular filling during diastole, producing a low-pitched rumbling murmur and causing pressure to build in the left atrium and pulmonary circulation. Heart-transplant candidacy considers medical urgency, compatibility, pulmonary pressures, organ function, cancer screening, psychosocial readiness, medication adherence, substance use, and available support. The episode concludes with pulseless electrical activity, or PEA—the clearest example of why a normal-looking rhythm does not guarantee that the heart is pumping. Starting with the Greek myth of Prometheus, whose liver was eaten and regenerated every day, we explore the real science behind liver regeneration, liver function, bilirubin metabolism, jaundice, hepatitis, and alcohol-related liver damage. The liver is often called the body’s chemical factory because it performs more than 500 essential functions, including detoxification, metabolism, bile production, blood filtering, nutrient processing, and waste removal. One of its most remarkable abilities is regeneration. After surgery, injury, or a living donor transplant, the remaining liver cells can begin multiplying almost immediately. In many cases, the liver can regain its full mass within a few weeks to several months. But liver regeneration has limits. The liver can recover from certain injuries and even repair damage caused by toxins, including alcohol, if the harmful cause is removed early enough. However, regeneration is not invincible. Cirrhosis Isn’t Just a Liver Problem: Bedside Signs of Liver Failure
Cirrhosis is not just a liver problem. Once the liver becomes stiff, scarred, and unable to function normally, the effects show up throughout the entire body — in the brain, kidneys, blood, abdomen, lungs, and vascular system. This episode breaks down how nurses can connect the pathophysiology of cirrhosis to what they actually see at the bedside. The episode begins by comparing the liver to the body’s “chemical factory.” A healthy liver filters toxins, makes important proteins, supports metabolism, and helps the body maintain balance. But with chronic injury from alcohol-related liver disease, fatty liver disease, hepatitis, autoimmune disease, or genetic conditions, the liver tries to repair itself over and over again. Eventually, that healing process becomes scarring, fibrosis, and permanent architectural damage. High-Yield Nursing Pearls Airway beats everything. Cleaning the stoma matters, but losing the airway is what kills the patient fastest. A cuffed trach does not prevent aspiration. Aspiration happens when material passes the vocal cords; the cuff sits below that level. Do not suction on a schedule. Suction based on assessment cues like visible secretions, coarse rhonchi, desaturation, or high peak airway pressures. Preoxygenate before suctioning. Suction removes oxygen along with secretions, so the patient can desaturate quickly. Fresh trach emergency rules are different. If the stoma is less than 7 days old, blindly forcing the tube back in can create a false passage. Trach patients need bowel teaching.
